Evaluation of patients via colonoscopy who underwent positron emission tomography/computerized tomography due to colon involvement.

OBJECTIVE: Fluorodeoxyglucose is not a tumor-specific agent and it can also be involved in benign conditions, which may cause diagnostic confusion. This research aims to elucidate the colonoscopic findings of patients who underwent colonoscopy due to colon involvement in positron emission tomography/computerized tomography. METHODS: A total of 71 patients who underwent colonoscopy due to colonic involvement in positron emission tomography/computerized tomography at SBU Keçiören Training and Research Hospital Gastroenterology Clinic Endoscopy Unit have been analyzed retrospectively. Demographi

Peritoneal Metastases of Colorectal Origin Treated with Complete Cytoreduction and Hyperthermic Intraperitoneal Chemotherapy: The Efficiency of Mitomycin C.

BACKGROUND: Survival of patients affected by colorectal cancer peritoneal metastases (CRC-PM) can be improved with combined complete cytoreductive surgery (CCRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). Two chemotherapeutic agents are mainly used: mitomycin C (MMC) and oxaliplatin. A recent prospective randomized clinical trial showed that oxaliplatin-based HIPEC does not improve survival compared with CCRS alone. The purpose of our study was to compare the survival effectiveness of MMC versus oxaliplatin-based HIPEC using a homogeneous surgical technique and drug protocol. METH

Colectomy and desmoid tumours in familial adenomatous polyposis: a systematic review and meta-analysis.

Desmoid tumours (DT) are one of the main causes of death in patients with familial adenomatous polyposis (FAP). Surgical trauma is a risk factor for DT, yet a colectomy is inevitable in FAP to prevent colorectal cancer. This systematic review and meta-analysis aimed to synthesize the available evidence on DT risk related to type, approach and timing of colectomy. A search was performed in MEDLINE, EMBASE and the Cochrane Library. Studies were considered eligible when DT incidence was reported after different types, approaches and timing of colectomy. Twenty studies including 6452 FAP patients

Role of Negative Pressure Wound Therapy When Performing Elective Open Colectomy.

Background: The impact of negative pressure wound therapy (NPWT) as an adjunct to colorectal surgery is largely unknown. The purpose of this study was to determine whether NPWT impacts wound complications during elective open colectomy. Methods: The American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) and colectomy targeted procedure databases were queried from 2012-2018 for patients undergoing non-emergent planned open colectomies. Groups were propensity score matched for anastomosis type (ileo-colic, colo-colic, colo-rectal), age, body mass index (BMI), diab…

Preoperative Circulating Tumor DNA in Patients with Peritoneal Carcinomatosis is an Independent Predictor of Progression-Free Survival.

BACKGROUND: Next-generation sequencing (NGS) is a useful tool for detecting genomic alterations in circulating tumor DNA (ctDNA). To date, most ctDNA tests have been performed on patients with widely metastatic disease. Patients with peritoneal carcinomatosis (metastases) present unique prognostic and therapeutic challenges. We therefore explored preoperative ctDNA in patients with peritoneal metastases undergoing surgery. METHODS: Patients referred for surgical resection of peritoneal metastases underwent preoperative blood-derived ctDNA analysis (clinical-grade NGS [68-73 genes]). ctDNA was

The selective use of splenic flexure mobilization is safe in both laparoscopic and open anterior resections.

AIM: Splenic flexure mobilization (SFM) is standard practice in anterior resections. No previous studies have compared outcomes with and without SFM in laparoscopic and open colorectal cancer surgery. This study aimed to determine whether routine or selective SFM should be advised. METHOD: Data were collected prospectively on all elective anterior resections for cancer in our unit between October 2006 and November 2009. RESULTS: Of 263 resections, SFM data were recorded in 216; 138 were laparoscopic (32% with SFM, 3.6% converted) and 78 open (68% with SFM). Eighty-eight were low anterior res

Endoscopic submucosal dissection for residual/locally recurrent lesions after endoscopic therapy for colorectal tumors.

BACKGROUND AND STUDY AIM: Residual or locally recurrent lesions may occur after endoscopic therapy for epithelial colorectal tumors. Additional endoscopic mucosal resection is difficult for large lesions. Endoscopic submucosal dissection may be useful for such lesions, but may be more technically difficult for residual/locally recurrent lesions than for primary lesions. This study evaluated the efficacy of endoscopic submucosal dissection for residual/locally recurrent lesions in comparison with primary lesions. METHOD: This retrospective case-control investigated 34 residual/locally recurren

Factors influencing hematological toxicity and adverse effects of perioperative hyperthermic intraperitoneal vs intraperitoneal chemotherapy in gastrointestinal cancer.

BACKGROUND: Intraperitoneal (IP) chemotherapy (IPC), including hyperthermic intraperitoneal chemotherapy (HIPEC), has emerged as a promising approach to control peritoneal metastases in gastrointestinal (GI) cancers. However, the safety profile and toxicity spectrum of IPC remain incompletely understood. This study aimed to evaluate the incidence of hematologic and biochemical adverse reactions following surgery with or without IPC and to compare the toxicity profiles of normothermic IPC and HIPEC. Additionally, potential risk factors for liver injury were investigated to guide clinical manage

Safety and efficacy of robotic repeat hepatectomy compared to laparoscopic and open repeat hepatectomy: a retrospective comparative study.

Repeat hepatectomies are challenging due to adhesions and altered anatomy. Robotic surgery, with enhanced dexterity and visualization, may expand the role of minimally invasive liver surgery (MILS) in this setting. The aim of this study was to explore the safety and efficacy of robotic repeat hepatectomy. We conducted a retrospective cohort study comparing repeat robotic (R-RLR), laparoscopic (L-RLR), and open liver resections (O-RLR) performed between January 2019 and December 2024. The primary outcome was postoperative morbidity. Secondary outcomes included conversion to laparotomy, use of P

Impact of MMR status on preoperative CT-based lymph node overstaging in right-sided colon cancer: a retrospective analysis.

BACKGROUND: Neoadjuvant treatment for colon cancer has recently gained more and more attention. Given the imprecision of radiologic staging, low-risk patients may be exposed to the toxicities and inconvenience of chemotherapy when surgery alone might have been considered sufficient. Overstaged lymph nodes via computed tomography (CT) may lead to unnecessary neoadjuvant treatment. However, the clinical features and prognostic factors of preoperative CT-based lymph node overstaging in right-sided colon cancer, particularly the impact of mismatch repair (MMR) status, are still not well understood

Prognostic and Treatment-Associated Survival Effects of Microsatellite Instability Across Disease Stages in Colon Cancer: An NCDB Analysis.

BACKGROUND: Microsatellite instability (MSI) is an established molecular biomarker in colorectal cancer (CRC). However, its stage-specific prognostic value and treatment-modifying role in colon cancer remain incompletely defined, particularly across contemporary multimodality treatment strategies. METHODS: We conducted a retrospective cohort study using the National Cancer Database, including patients aged ≥ 20 years diagnosed with colon cancer between 2004 and 2020 with documented tumor stage and MSI status. MSI was categorized as microsatellite stable (MSS), MSI-low (MSI-L), or MSI-high (MS

Standard-Based Port-Sharing Workflow for Simultaneous Robot-Assisted Distal Gastrectomy and Right-Sided Colectomy: A Case Report.

INTRODUCTION: Synchronous gastric and colorectal cancers occur in approximately 4% of patients. Although simultaneous resection is common in laparoscopic surgery, robot-assisted simultaneous procedures are less frequently reported because of logistical constraints in multi-quadrant targeting and port geometry. Herein, we describe a standardized and reproducible workflow using the da Vinci Xi system (Intuitive Surgical, Sunnyvale, CA, USA) that focuses on strategic port sharing and planned redocking. CASE PRESENTATION: An 86-year-old man presented with gastric cancer (cT3N0M0, Stage II), ascen

Endoscopic Ultrasound-Guided Radiofrequency Ablation for Pancreatic Metastases: Safety and Clinical Outcomes in a Single-Center Case Series.

Background/Objectives: Pancreatic metastases are uncommon, and local treatment is reserved for selected patients with limited disease. Surgery remains the standard option when feasible, but may be inappropriate in patients with small, multifocal, anatomically challenging, or medically high-risk lesions. This study evaluated endoscopic ultrasound-guided radiofrequency ablation (EUS-RFA) as an organ-preserving local treatment for pancreatic metastases. Methods: Consecutive adult patients undergoing EUS-RFA for histologically confirmed pancreatic metastases at a tertiary endoscopy center between

Beyond Simple Ratios: Can the Preoperative HALP Score Provide a More Effective Inflammatory Biomarker for Colon Cancer Survival?

Background and Objectives: Previous findings have suggested that inflammatory and nutritional biomarkers may help predict the prognosis in colorectal cancer patients; however, their predictive value and optimal use remain uncertain. Therefore, this study aimed to investigate the effects of several preoperatively measured inflammatory and nutritional biomarkers on the overall survival in patients undergoing surgery for colon cancer and to compare their predictive ability between themselves and with other clinical parameters. Materials and Methods: This retrospective cohort study included 411 pa

Machine learning-based prediction model and web calculator for postoperative LDVT in colorectal cancer.

BACKGROUND: Lower limb deep vein thrombosis (LDVT) is a common but often underdiagnosed complication after colorectal cancer (CRC) surgery. Its early symptoms are subtle, and delayed detection can lead to post-thrombotic syndrome or even life-threatening events. However, effective tools for early risk assessment are lacking. OBJECTIVE: To identify risk factors for postoperative LDVT in CRC patients and develop a machine learning (ML)-based risk prediction model with an accessible web calculator. METHODS: This retrospective study included 1,200 CRC patients undergoing radical surgery. A model
