Behavioral and motor deficits following traumatic brain injury: implications for post-traumatic epilepsy.

Post-traumatic epilepsy (PTE) is a severe complication of traumatic brain injury (TBI). Patients with PTE often experience comorbid neurobehavioral deficits, which further reduce the quality of life and impede the recovery of patients. This review examines behavioral and motor deficits associated with PTE across clinical and preclinical studies, with an emphasis on the need to identify PTE-specific phenotypes that may serve as biomarkers of disease development and progression, as well as distinguish PTE from other forms of epilepsy. Despite growing evidence of post-TBI behavioral and motor abn

Addressing stigma in epilepsy care: a systematic review of personalized anti-stigma interventions.

Epilepsy-related stigma (ERS) continues to be a significant challenge affecting people with epilepsy (PWE) worldwide and is closely associated with reduced quality of life and poor treatment adherence. Although various anti-ERS interventions have been developed, the effectiveness of personalized approaches has not been systematically evaluated. This systematic review synthesizes the available evidence on personalized anti-ERS interventions in epilepsy, focusing on their effectiveness as a strategy to reduce ERS. We systematically searched for studies evaluating personalized interventions aimed

Patient-reported outcome measures (PROMs) in quality of epilepsy care: A systematic review.

OBJECTIVE: The current lack of an unambiguous and widely applicable quality indicator for epilepsy care could hamper epilepsy care. The main aim of this systematic review is to study patient-reported outcome measures (PROMs) instruments that evaluate the quality of epilepsy care and inventorize the main themes covered as well as the measurement properties of these instruments. METHODS: The protocol was registered in PROSPERO (ID: CRD42025637902) and reporting followed the PRISMA reporting guidelines for systematic reviews. Electronic databases searches were conducted on PubMed, Embase and Web

Cumulative electroconvulsive therapy sessions and focal epilepsy: A nationwide cohort study in Japan.

OBJECTIVE: Electroconvulsive therapy (ECT) is widely used for severe psychiatric disorders, yet concerns remain regarding possible epileptogenic effects associated with prolonged or repeated exposure. We examined the association between cumulative ECT sessions and focal epilepsy in a nationwide Japanese cohort. METHODS: We conducted a retrospective cohort study using a nationwide claims database including 3733 patients who received ECT between 2008 and 2025. For analyses of incident focal epilepsy, patients with a prior diagnosis of epilepsy before the first ECT session were excluded from the

Exploratory epilepsy-derived transcriptomic projection reveals region-sensitive molecular remodeling in Alzheimer's disease.

BACKGROUND: Alzheimer's disease (AD) is frequently accompanied by seizures or subclinical epileptiform activity, but the transcriptomic context linking AD-related neurodegeneration with epilepsy-related network dysfunction remains unclear. OBJECTIVE: To apply an exploratory epilepsy-derived directional transcriptomic projection framework to examine region-sensitive molecular organization in AD brain tissues. METHODS: A directional reference was generated from GSE268714 by defining seizure onset zone (SOZ)-upregulated and SOZ-downregulated genes from an unpaired exploratory SOZ-versus-non-inv

Longitudinal developmental trajectories after epilepsy surgery in early childhood: A biphasic developmental hypothesis.

OBJECTIVE: To evaluate the longitudinal impact of epilepsy surgery on developmental trajectories in young children by comparing developmental rate (DR) during the early postoperative and later follow-up periods. METHODS: This single-center retrospective observational study included 90 children who underwent their first epilepsy surgery before 72 months of age. Development was assessed using the Kinder Infant Developmental Scale (KIDS). To reduce the influence of chronological age, DR was calculated as the change in developmental age divided by the observation period. DR during the first posto

Contraceptive and Enzyme-Inducing Antiseizure Medication Coprescription Among US Medicaid Beneficiaries With Epilepsy.

BACKGROUND AND OBJECTIVES: People with epilepsy of child-bearing potential (PWECP) require access to highly effective contraception, yet many antiseizure medications (ASMs) can reduce the effectiveness of some hormonal methods by inducing hepatic enzymes. In this retrospective cohort study, we aimed to identify the proportion of PWECP prescribed a susceptible hormonal contraceptive method who were concurrently prescribed an interacting ASM and patient characteristics associated with coprescription. METHODS: We obtained Medicaid claims data from 2016 to 2021 for female beneficiaries and define

Drug-resistant epilepsy in an autosomal recessive copper metabolism disorder from SLC33A1 mutation.

UNLABELLED: SLC33A1 mutation (Huppke-Brendel Syndrome) is a rare, autosomal recessive disorder associated with copper metabolism and characterized by global developmental delay, sensorineural hearing loss, congenital cataract and generalized hypotonia. We report two siblings under 6 years with novel SLC33A1 gene mutation, born from heterozygous parents. The first patient developed early onset drug-resistant epilepsy (DRE) secondary to the novel genetic mutation; the second patient did not develop seizures but shared several other phenotypic features of the syndrome, highlighting intrafamilial

Expanding the phenotypic spectrum of FGF12-epilepsy-does prompt precision therapy affect outcomes?

Fibroblast growth factor-12 (FGF12) variants have been associated with developmental and epileptic encephalopathy (DEE) with evidence of modulation of voltage-gated sodium channels Nav1.2 and Nav1.6. We aim to expand the phenotypic spectrum of FGF12-related epilepsy with emphasis on precision therapy. We describe 12 patients: eight with neonatal onset seizures with the recurrent p.Arg52His (c.155 G > A) variant, two with a previously unreported p.Glu153Gly (c.458 A > G) variant, one with a p.Gly50Ser (c.148 G > A) variant, and one with a de novo whole-gene duplication. Atypical absence seizure

Epilepsy-associated digenic variants affecting an actin/mitochondria/glutamate pathway promote seizure susceptibility.

Epilepsy affects approximately 50 million people worldwide, yet more than half of individuals with a presumed genetic cause still lack a molecular diagnosis despite the identification of over 1,000 monogenic epilepsy genes. This diagnostic gap is unlikely to be resolved by improved variant detection alone, suggesting that variants affecting the same biological pathway may combine to cause disease. By studying epilepsy-associated actin regulatory genes, we identified a conserved actin/mitochondria/glutamate (AMG) pathway. We demonstrate that reduced actin polymerization promoted DRP1-mediated m

Thiazolidinedione and reduced epilepsy risk: A population-based study using target trial emulation.

OBJECTIVE: No epidemiological studies have yet explored the relationship between TZD use and epilepsy incidence within a population context. This study aimed to evaluate the association between TZD use and risk of epilepsy in a Chinese population by using a target trial emulation framework with active-comparator new-user (ACNU) design. METHODS: We emulated a target trial comparing the risk of epilepsy between use of TZDs and alpha glucosidase inhibitors (AGIs) in patients with type 2 diabetes mellitus (T2DM). Participants aged between 18 and 100 years, who were new users of TZDs or AGIs were

Sleep stage-dependent distribution of interictal epileptiform discharges in epilepsy: A systematic review.

BACKGROUND: Sleep and epilepsy interact through complex bidirectional mechanisms. Although NREM sleep facilitates interictal epileptiform discharges (IED), the diagnostic contribution of individual sleep stages remains uncertain. In particular, it is unclear whether deeper sleep stages such as N3 provide an advantage over N2 for spike detection or localization in clinical (electroencephalography) EEG practice. METHODS: This systematic review followed PRISMA 2020 guidelines. PubMed and Web of Science were searched for studies reporting quantitative IED measures across sleep stages in patients

Inroads into epilepsy through high-frequency oscillations: Achievements and benchmark areas for improvement.

High-frequency oscillations (HFOs) were discovered more than 20 years ago, and since then they have been studied intensively in the context of epilepsy. HFOs encompass a broad spectrum of oscillations, typically ranging from 80 Hz to several kHz, that include both normal and pathological oscillations, documented in people with epilepsy and animal models. HFOs have drawn considerable attention due to their prominent roles in epileptogenesis, ictogenesis, and functional organization of epileptic tissue. We provide historical background on HFOs in epilepsy and summarize the current state of knowl

VENI, VIDI, VICI: The advantage of early epilepsy surgery in drug-sensitive patients.

OBJECTIVE: Surgical intervention is frequently considered for drug-resistant focal structural epilepsy. However, the impact of early surgery on seizure control, cognitive performance, and quality of life (QoL) across drug-sensitive and drug-resistant patients remains incompletely characterized. METHODS: We conducted a prospective-retrospective study on 214 patients with age > 6 years with focal structural epilepsy (focal cortical dysplasia type II, hippocampal sclerosis, low-grade epilepsy-associated tumors) stratified as: drug-sensitive (DS, n = 72) and drug-resistant (DR, n = 112) patients

Anti-Inflammatory and Antioxidant Strategies in Epilepsy: From Molecular Mechanisms to Threshold Management.

Epilepsy is a multifactorial disorder, yet routine management still focuses on neuronal excitation and insufficient inhibition, with antiseizure medications (ASMs) as the primary therapeutic strategy. This approach fails in roughly one-third of patients who develop drug-resistant epilepsy (DRE). Converging evidence links DRE with neuroinflammation, oxidative stress (OS), and mitochondrial dysfunction-an interconnected distal pathophysiological triad that progressively lowers seizure thresholds yet remains peripheral to clinical epilepsy management. We map this triad mechanistically and show th

Caring Across Languages: Clinician Perceptions of Variability in US Pediatric Outpatient Epilepsy Care.

Little is known about outpatient language access practices in the United States for children with epilepsy whose families use languages other than English (LOE) for medical care. The Pediatric Epilepsy Research Consortium (PERC) aimed to characterize clinician perceptions of institutional practices by developing and conducting a descriptive survey. We weighted statistical outcomes to equally represent clinician opinion from each center and conducted deductive thematic analysis of open-ended responses. One hundred clinicians from 40 institutions (53.3% institutional response rate) completed the
