Two cases of treated iatrogenic aorto-enteric fistula are presented: one completely successfull, the other less, with exitus after five months due to aortic wall disruption. The late occurrence, operative findings, anatomo-pathological examination of the specimens, as well as the sterile (aerobic) cultures in both cases, rather favour mechanical etiological forces associated with the healing complications of aortic synthetic grafts, with or without secundary infection. Spontaneous external biliary fistula, once common, is now a rare clinical entity. A patient with this lesion is described and attention is drawn to the fact that this rare condition is still being seen in certain patients with neglected cholecystitis. Heroin addiction was thought to have modified his response to gallbladder disease and contributed to the perforation. Diagnosis was suspected on physical examination and confirmed by spontaneous discharge of stones and fistulogram.

The authors report a case of post-traumatic arterioveinous fistula of the dura-mater associated with an extradural hematoma of the posterior fossa. The traumatic nature of the lesion is confirmed by the presence of an occipital fracture facing the fistule responsable for the extradural hematoma. The authors have found on similar case citied in the literature (11).

Disk space sepsis is a recognized complication of spinal surgery, but infection by a penetrating fistula from a retroperitoneal or pelvic abscess is rare. The lack of localizing symptoms, the insidious disease process, and the clinical and myelographic findings simulating a herniated disk syndrome obscure the true diagnosis and make identification of the primary infection difficult. A good result was obtained in this case by laminectomy and drainage of the lumbosacral disk space, followed by a diagnostic fistulogram and evacuation of a large pelvic abscess.

On the basis of three cases, the authors study the problem of perineal urinary fistula secondary to rectal surgery. They first note that little attention has been devoted to these lesions. The clinical and radiological features are described and analysed, in a pathological context. Treatment of these fistulae is difficult if a conservative approach is sought, and the best solution is perhaps nephrectomy or bypass by a Bricker operation carried out either initially or after an attempt at repair has failed.

The second case of septic arthritis resulting from a vesico-acetabular fistula secondary to a penetrating injury is reported. Despite reported good results in blunt trauma with hip and urinary tract drainage combined with appropriate antibiotics, improvement occurred in our patient only after surgical excision of the fistulous tract. It appears that in some cases early fistulectomy can shorten hospital stay and reduce the incident of complete ankylosis.

A relatively simple method for the intra-vascular treatment of a carotid-cavernous fistula is described and illustrated in one case. It consists of proximal ligation of the internal carotid artery and occlusion of the carotid syphon with a balloon catheter above the fistula. In this way, the carotid segment with the fistula is isolated from the circulation and therefore undergoes thrombosis. Close co-operation between radiologist and surgeon is required.

Management of 10 cases of primary perilymph fistula is described. The modified technique of stapedectomy described in this article, seems to prevent primary perilymph fistula; time will tell if it will prevent secondary fistula. The hearing gain obtained with this technique is slightly inferior to the gain obtained with Gelfoam-wire prosthesis, but most significant is the fact that discrimination score is excellent in all cases. The author is convinced that the extra time and dexterity required to perform this procedure are well justified by the safety provided and the results obtained.

A case of iatrogenic subclavian arteriovenous fistula is reported. An intracath had been inserted percutaneously into the left subclavian vein to monitor the central venous pressure during aortic valve surgery. On the second postoperative day a thrill and continuous murmur were detected at the left subclavian region. Left subclavian arteriogram and catheterization studies confirmed the diagnosis of subclavian arteriovenous fistula. The lesion was treated surgically with satisfactory results.

A case of occlusion of the right common carotid artery combined with stenosis of the left common carotid artery is reported. Immediately after surgery a carotico-cavernosus fistula was observed on the right side. The clinical picture and the therapeutical problems are discussed; 4 X-ray and 2 fundus photographs are presented.

The application of a modified Latzko's partial colpocleisis to vesicovaginal fistula occurring after abdominal hysterectomy has been reported. The present method, which is safe and highly effective, can be perfomred with ease by urologists having little experience with transvaginal operations. It is important in performing the present operation to remove the tension on the bladder wall by dissecting the cleavage plane between the vaginal wall and the bladder wall in a distal direction from the denuded area surrounding the fistula.
