Efficacy, safety, and population pharmacokinetics of a single 1500mg dose of dalbavancin for short-term therapy in patients with chronic prosthetic joint infections.

The efficacy, safety, and population pharmacokinetics of a single 1,500 mg dose of dalbavancin as a sequencing treatment for Gram-positive chronic prosthetic joint infections (CPJIs) have not been described. We present an observational, retrospective study conducted in two Spanish hospitals including patients with CPJI caused by Gram-positive bacteria susceptible to dalbavancin managed with two-stage exchange, antibiotic-loaded spacers, and a single 1,500 mg dose of dalbavancin. Follow-up visits included measurement of dalbavancin plasma concentrations. Negative intraoperative cultures at seco

Prevalence and risk factors of concurrent bacteremia in children under 5 years of age hospitalized with viral lower respiratory tract infections.

PURPOSE: We aimed to investigate the prevalence and risk factors of concurrent bacteremia in otherwise healthy children under 5 years of age … remia. In the post-pandemic period, a significant shift in pathogen distribution was observed, with gram-negative bacteria, particularly Klebsiella spp., emerging as the predominant cause of bacteremia. CONCLUSION: Although rare, concurrent bacteremia was associated with high fever, elevated CRP levels, the need for advanced respiratory support, and an age of ≤ 6 month …

A Comparative Study of Standard and Direct Susceptibility Testing From Positive Blood Cultures at a Tertiary Care Hospital in Bangladesh.

BACKGROUND: Sepsis is one of the leading causes of death globally. Standard antimicrobial susceptibility testing (AST) takes around 48 hours to get results from a positive blood culture bottle. Advanced diagnostic tools for AST are not available in most of the laboratories in developing countries. Therefore, performing a direct susceptibility testing (DST) might help to reduce turnaround time by 24 hours, reducing high mortality in patients with septicemia. OBJECTIVE: The study aimed to evaluate the accuracy of DST from positive blood culture broth by comparing categorical agreement and error

Antibiotic Resistance Pattern in Intensive Care Unit of a Tertiary Care Hospital, Nepal.

Background Intensive care unit (ICU) is the especial department of the hospital where critically ill patients are treated with the unique type of technologies to revert back to functional by body's own mechanism. Therefore, there are lots of external intervention with chance of getting bacterial infections. Antibiotics are medicines used to prevent and treat such bacterial infections. However, due to selective broad spectrum antibiotic pressure there is great chances to develop antimicrobial resistance at any time during hospital stay in intensive care unit. Objective To find out the antibioti

Bacterial Virus Forcing of Bacterial O-Antigen Shields: Lessons from Coliphages.

In most Gram-negative bacteria, outer membrane (OM) lipopolysaccharide (LPS) molecules carry long polysaccharide chains known as the O antigens or O polysaccharides (OPS). The OPS structure varies highly from strain to strain, with more than 188 O serotypes described in E. coli. Although many bacteriophages recognize OPS as their primary receptors, these molecules can also screen OM proteins and other OM surface receptors from direct interaction with phage receptor-binding proteins (RBP). In this review, I analyze the body of evidence indicating that most of the E. coli OPS types robustly shie…

Evaluation of Sepsis Flow Chip for identification of Gram-negative bacilli and detection of antimicrobial resistance genes directly from positive blood cultures.

Blood stream infections are serious conditions associated with high morbi-mortality. In this study, the new Sepsis Flow Chip (SFC) assay for identification of Gram-negative bacteria and their antimicrobial resistance genes was evaluated in positive blood cultures (BCs). SFC is a microarray with a broad panel comprising the most frequent causative agents of sepsis and antimicrobial resistance genes associated with them. A total of 100 prospective BCs, positive for Gram-negative bacilli, were assessed in the routine of the clinical microbiology laboratory and also applying the SFC assay. Moreove…

A Coxiella burnetii phospholipase A homolog pldA is required for optimal growth in macrophages and developmental form lipid remodeling.

BACKGROUND: Many gram-negative bacteria produce an outer membrane phospholipase A (PldA) that plays an important role in outer membrane function and is associated with virulence. RESULTS: In the current study, we characterized a pldA mutant of Coxiella burnetii, an intracellular gram-negative pathogen and the agent of human Q fever. The C. burnetti pldA open reading frame directs synthesis of a protein with conserved PldA active site residues. A C. burnetii ΔpldA deletion mutant had a significant growth defect in THP-1 macrophages, but not axenic medium, that was rescued by complementation. T…

Lipopolysaccharide reduces the primordial follicle pool in the bovine ovarian cortex ex vivo and in the murine ovary in vivo.

Infections of the uterus or mammary gland with Gram-negative bacteria cause infertility in cattle, not only during disease but also for some time afterward. Even though these infections are in organs distant from the ovary, metritis and mastitis perturb antral follicle development and function in vivo. Although granulosa cells from antral follicles express toll-like receptor 4 (TLR4), and detect and mount an inflammatory response to lipopolysaccharide (LPS) from Gram-negative bacteria, it is not known whether LPS impacts preantral follicle development. The present study tested the hypothesis t…

IMD-0354 optimization generates potent colistin adjuvants with in vivo activity and reduced eukaryotic toxicity.

The development of novel therapeutic approaches to combat infections that stem from extensively drug-resistant (XDR) gram-negative bacteria remains an area of significant unmet need. One such approach is the use of antibiotic adjuvants. Currently, colistin (polymyxin E) is used as the antibiotic of last resort for the treatment of XDR gram-negative bacterial infections. However, resistance to this antibiotic is on the rise. We previously reported that IMD-0354 (an IκB kinase-β inhibitor) and related salicylanilide adjuvants overcome colistin resistance in several gram-negative pathogens. Howev…

Occurrence of extended-spectrum β-lactamase, AmpC, and carbapenemase-producing genes in gram-negative bacterial isolates from human immunodeficiency virus infected patients.

BACKGROUND: Progressive decline of immune response in HIV patients makes them susceptible to frequent bacterial infections. High usage of antibiotics influences the emergence of multidrug-resistant bacteria and worsens the clinical outcomes. In this study, the occurrence of drug-resistant genes in Gram-negative bacterial isolates from HIV patients in South India was analyzed. METHODS: A total of 173 Gram-negative bacterial (GNB) isolates from HIV patients were screened for antibiotic susceptibility profile using the Kirby-Bauer diskdiffusion method. Positivity of drug-resistant genes was anal…

In Vitro Activity of Ceftazidime-Avibactam Alone and in Combination with Amikacin Against Colistin-Resistant Gram-Negative Pathogens.

Aims: Colistin became the critical treatment option for multidrug-resistant Gram-negative bacteria (GNB); however, resistance to colistin is increasingly being reported among clinical isolates. New therapy strategies should be considered nowadays. The aim of this study was to investigate the in vitro activity of a novel β-lactam/β-lactamases inhibitor ceftazidime-avibactam (CZA) alone and in combination with amikacin against colistin-resistant Gram-negative pathogens. Results: Among all the colistin-resistant GNB strains, 30.4% (21/69) were resistant to CZA, which was similar to the resistance…
