Unexplained Giant Esophageal Ulceration in a Pregnant Woman With Severe Immunosuppression.

Esophageal ulceration is a well-recognized cause of morbidity in patients with advanced human immunodeficiency virus (HIV) infection, most commonly from opportunistic infections such as cytomegalovirus, herpes simplex virus, or Candida species. Idiopathic esophageal ulceration (IEU) is a diagnosis of exclusion associated with profound immunosuppression, typically with CD4 counts <50 cells/mm³. Its occurrence during pregnancy, particularly in congenital HIV infection, is exceedingly rare and presents unique diagnostic and therapeutic challenges. We report a 32-year-old pregnant woman at 21 week

Helicase-primase inhibitors as next-generation anti-HSV agents: from current clinical status to structure-activity relationships and patent coverage.

INTRODUCTION: Helicase-primase is an interesting target for small-molecule therapy of orthoherpesviruses in general and, in particular, herpes simplex virus (HSV) and varicella zoster virus (VZV) infections. With amenamevir already approved for the treatment of VZV and HSV in Japan and with pritelivir's granted breakthrough therapy designation and priority review for the treatment of acyclovir-resistant HSV infections in immunocompromised patients, other companies have also expressed interest in small-molecule helicase-primase inhibitors (HPIs). AREAS COVERED: We review the helicase-primase i

Toscana Virus Diagnosis via a Point-of-Care Tool Based on Disposable Optical Fiber Biosensors.

The Toscana virus (TOSV) is an emerging Phlebovirus endemic to the Mediterranean region, with marked neurotropism, responsible for often undiagnosed cases of meningitis and meningoencephalitis. Rapid and precise identification is critical for medical treatment, but traditional approaches like Polymerase Chain Reaction (PCR) and serology have limits in terms of time, cost, and potential cross-reactivity. This proof-of-concept introduces a low-cost point-of-care test (POCT) based on surface plasmon resonance (SPR) in plastic optical fibers (POFs) combined with an antibody selective for TOSV nucl

Transsynaptic viral tools in neural circuit analysis: from anatomical mapping to functional interrogation.

The application of transsynaptic viruses has transformed neural circuit analysis, enabling increasingly precise mapping of neuronal connectivity. These approaches can be broadly divided into polysynaptic and monosynaptic strategies. Polysynaptic retrograde tracers based on pseudorabies virus, including the classically selected Bartha strain and genetically engineered retrograde variants have provided important insights into multisynaptic circuit organization in rodents. For polysynaptic anterograde tracing, herpes simplex virus strain H129 and its derivatives have been used to map output pathw

Emerging and Re-Emerging Viral Pathogens in Oncolytic Virotherapy: A Comprehensive Review.

Oncolytic virotherapy has recently emerged as a promising immuno-oncological strategy, utilising the natural cytolytic potentials of viral pathogens to selectively target and destroy malignant cells. Although various engineered viruses, such as adenoviruses, vaccinia virus, and herpes simplex virus, are currently in clinical evaluation, emerging and re-emerging viruses with intrinsic oncolytic potential, including Zika virus (ZIKV), Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), Dengue virus (DENV), and West Nile virus (WNV), have also shown promising results. These emerging and

An AI-assisted morphoproteomic approach is a supportive tool in esophagitis-related precision medicine.

Esophagitis is a frequent, but at the molecular level poorly characterized condition with diverse underlying etiologies and treatments. Correct diagnosis can be challenging due to partially overlapping histological features. By proteomic profiling of routine diagnostic FFPE biopsy specimens (n = 55) representing controls, Reflux- (GERD), Eosinophilic-(EoE), Crohn's-(CD), Herpes simplex (HSV) and Candida (CA)-esophagitis by LC-MS/MS (DIA), we identified distinct signatures and functional networks (e.g. mitochondrial translation (EoE), immunoproteasome, complement and coagulations system (CD), r

Neurofibromatosis Type 1: Genetic Mechanisms and Advances in Therapeutic Innovation.

Mutations in the NF1 gene cause Neurofibromatosis Type 1 (NF1), one of the most common genetic disorders. This gene encodes neurofibromin, a member of the GTPase-activating protein (GAP) family that functions as a negative regulator of RAS signaling. Loss of NF1 function leads to persistent RAS activation and promotes tumor growth. The clinical manifestations of NF1 mainly include pigmentary changes, benign and malignant peripheral nerve sheath tumors, as well as gliomas affecting the central nervous system. Currently, MEK inhibition is the only approved therapy and is primarily effective in c

The Role of Artificial Intelligence in Herpesvirus Detection, Transmission, and Predictive Modeling: With a Special Focus on Marek's Disease Virus.

Herpesvirus infections, including herpes simplex virus (HSV), Epstein-Barr virus (EBV), and cytomegalovirus (CMV), present significant challenges in diagnosis, treatment, and transmission control. Despite advances in medical technology, managing these infections remains complex due to the viruses' ability to establish latency and their widespread prevalence. Artificial Intelligence (AI) has emerged as a transformative tool in biomedical science, enhancing our ability to understand, predict, and manage infectious diseases. In veterinary virology, AI applications offer considerable potential for

A More Rapid Method for Culturing LUHMES-Derived Neurons Provides Greater Cell Numbers and Facilitates Studies of Multiple Viruses.

The ability to study mature neuronal cells ex vivo is complicated by their non-dividing nature and difficulty in obtaining large numbers of primary cells from organisms. Thus, numerous transformed progenitor models have been developed that can be routinely cultured, then scaled, and differentiated to mature neurons. In this paper, we present a new method for differentiating one such model, the Lund human mesencephalic (LUHMES) dopaminergic neurons. This method is two days faster than some established protocols, results in nearly five times greater numbers of mature neurons, and involves fewer

Portable molecular diagnostic platform for rapid point-of-care detection of mpox and other diseases.

The World Health Organization's designation of mpox as a public health emergency of international concern in August 2024 underscores the urgent need for effective diagnostic solutions to combat this escalating threat. The rapid global spread of clade II mpox, coupled with the sustained human-to-human transmission of the more virulent clade I mpox in the Democratic Republic of Congo, highlights a critical gap in point-of-care diagnostics for this emergent disease. In response, we developed Dragonfly, a portable molecular diagnostic platform for point-of-care use that integrates power-free nucle

A self-amplifying mRNA vaccine expressing PRV gD induces robust immunity against virulent mutants.

Pseudorabies virus (PRV) causes an acute febrile infectious disease of pigs. Since 2011, PRV variants have appeared and spread nationwide in China. mRNA vaccines present a safe alternative and can stimulate humoral and cellular immunity. We constructed a self-amplifying mRNA (saRNA) vaccine containing gD gene of a highly virulent PRV variant. In mice, the PRV saRNA gD vaccine induced high levels of neutralizing antibodies. In piglets, vaccination with the PRV saRNA gD vaccine at a dose of as little as 5 μg induced very strong humoral immunity and cellular immunity, providing full protection ag

Role for NF-κB in herpes encephalitis pathology in mice genocopying an inborn error of IRF3-IFN immunity.

Herpes simplex encephalitis (HSE) is a devastating disease with high mortality and serious sequelae. Genetic defects in the IFN-I pathway predispose individuals to HSE, but underlying mechanisms remain unclear. Using transgenic mice with the IRF3 R278Q mutation, ortholog to HSE-associated IRF3 R285Q, and iPSC-derived CNS cells from a pediatric patient carrying the variant, we investigated mechanisms in HSE. IRF3 R278Q transgenic mice exhibited aggravated HSV-1 brain disease and elevated CNS viral loads. Accordingly, microglia from the IRF3 R278Q mice showed reduced HSV-1-induced IFN-I expressi

Alpha-herpesvirus US1 interacts with cGAS to suppress type I IFN responses and antiviral defense.

Alpha-herpesviruses, including pseudorabies virus (PRV) and herpes simplex virus type 1 (HSV-1), cause severe diseases in a wide range of hosts. However, the precise mechanisms of immune evasion by alpha-herpesviruses remains elusive, hindering the development of broad-spectrum antiviral vaccines and drugs. Here, we demonstrate that the immediate early protein US1, encoded by alpha-herpesviruses, directly interacts with cGAS, suppressing its dsDNA binding and enzymatic activity. Structural analysis using AlphaFold reveals a conserved overlapping region within PRV and HSV-1 US1 proteins. Deleti
