Chyle leak is a potentially serious complication resulting from violation of the thoracic duct, right lymphatic duct, or one of their terminal branches. It may impair nutrition and immunity, compromise and delay wound healing, prolong hospitalization, and delay adjuvant therapy. After surgery for breast carcinoma, this is a rare occurrence. Here, the author describes a novel application of lymphoscintigraphy showing thoracic duct injury after a wide excision and axillary clearance.

A seven-year-old flat-coated retriever presented with a history of lethargy, dyspnoea and inappetence of several days' duration. Clinical examination revealed pale mucous membranes and tachypnoea, and haematology demonstrated marked autoagglutination. Thoracic radiographs revealed an increased opacity in the perihilar region. The owners declined further evaluation and the dog was treated symptomatically with immunosuppressive doses of prednisolone and azathioprine. The dog's demeanour improved, although it was eventually euthanased seven weeks later because of dysphagia and worsening dyspnoea.

Surgical staging has changed the method by which patients with endometrial cancer are managed. Before the routine use of lymph node dissection, patients were presumed to have nodal disease based on imaging studies, palpation, and biopsy. The move to a surgically based staging system in 1988 created a new subgroup of patients who had documented nodal disease. OBJECTIVE: To test the clinical value of using Tc-99m MIBI scintimammography (Tc-99m MIBI SMM) to differentiate nodular lesions in breasts and detect axillary lymph node metastases of breast cancer.
METHODS: The subjects consisted of sixty female patients exhibiting unilateral palpable breast mass. The early and delay static acquisitions were undertaken in all subjects at 10 and 90 min after injection with Tc-99m MIBI. The acquisition included three positions: anterior supine, left lateral prone and right lateral prone. A special device for SMM which was developed by our laboratory was used.
