High-velocity penetrating wounds of the gravid uterus: review of 16 years of civil war.

OBJECTIVE: To evaluate the value of selective laparotomy in pregnant women with penetrating abdominal injuries. METHODS: A retrospective survey was carried out at our center over 16 years of civil war, extending from 1975 to 1991. Fourteen pregnant women had uterine injuries secondary to high-velocity abdominal penetrating trauma. The corresponding management was evaluated carefully with respect to maternal and fetal outcomes. RESULTS: Two maternal deaths occurred, neither resulting solely from intra-abdominal injuries. Visceral injuries were present when the entrance of the missile was in e

DRESS induced by amoxicillin-clavulanate in two pediatric patients confirmed by lymphocyte toxicity assay.

BACKGROUND: Drug reaction with eosinophilia and systemic symptoms (DRESS) is a rare but serious delayed hypersensitivity reaction that can be caused by antibiotic exposure. The reaction typically develops in 2 to 6 weeks. The pathophysiology is thought to involve toxic drug metabolites acting as a hapten, triggering a systemic response. The diagnosis is made clinically but can be confirmed using assays such as the lymphocyte toxicity assay (LTA), which correlates cell death upon exposure to drug metabolites with susceptibility to hypersensitivity reactions. CASE PRESENTATIONS: Case 1 involves

Association of Intrapartum Drugs with Spontaneous Intestinal Perforation: A Single-Center Retrospective Review.

OBJECTIVE: Spontaneous intestinal perforation (SIP) occurs commonly in extremely low gestational age newborns (ELGANs; <30 weeks' GA). Early, concurrent neonatal use of indomethacin (Neo_IN) and hydrocortisone (Neo_HC) is a known risk for SIP. Mothers in premature labor often receive indomethacin (Mat_IN) for tocolysis and steroids (Mat_S) for fetal maturation. Coincidentally, ELGANs may receive Neo_IN or Neo_HC within the first week of life. There are limited data on the effect of combined exposures to maternal and neonatal medications. We hypothesized that proximity exposure to these medicat

Reductions in Corpus Callosum Volume Partially Mediate Effects of Prenatal Alcohol Exposure on IQ.

Disproportionate volume reductions in the basal ganglia, corpus callosum (CC) and hippocampus have been reported in children with prenatal alcohol exposure (PAE). However, few studies have investigated these reductions in high prevalence communities, such as the Western Cape Province of South Africa, and only one study made use of manual tracing, the gold standard of volumetric analysis. The present study examined the effects of PAE on subcortical neuroanatomy using manual tracing and the relation of volumetric reductions in these regions to IQ and performance on the California Verbal Learning

Gestational diabetes and offspring birth size at elevated environmental pollutant exposures.

BACKGROUND: Gestational diabetes mellitus (GDM) is associated with increased availability of glucose and macronutrients in fetal circulation and macrosomia. Therefore, the role of GDM in the association between metabolism-disrupting chemicals and birth size deserves attention. OBJECTIVE: We examined whether GDM may mediate or modify the associations between maternal environmental pollutant exposures and offspring birth size measures. METHODS: We analyzed 604 Faroese pregnant women and their offsprings born in 1997-2000. Maternal pregnancy serum concentrations of organochlorine compounds (OCs

Exploratory assessment of preconception phthalate exposure on pregnancy and offspring health outcomes in mice.

Understanding how endocrine-disrupting chemicals influence reproductive success requires attention to sensitive windows beyond gestation, including the understudied preconception period. In this exploratory pilot study, female CD-1 mice were exposed to a human-relevant phthalate mixture (200 µg/kg/day) for 30 days prior to mating. Although implantation and litter size were unaffected, exposed dams exhibited nonsignificant shifts in estrus cyclicity, spending more time in proestrus and less in metestrus. Maternal liver transcriptomics revealed persistent changes more than a month after exposure

Socioeconomic inequalities in inflammation in childhood and adolescence: an international study across five cohorts.

Chronic inflammation may be a key mechanism driving social to biological transitions across the lifecourse. We investigated the effect of early-life socioeconomic position (SEP) on inflammatory biomarkers in childhood and adolescence and assessed mediation by body mass index (BMI). We conducted cohort-specific analysis in five birth cohorts: the Barwon Infant Study (BIS, Australia, n = 708, 4-5 years); Born in Bradford (BiB, United Kingdom, n = 4576, 7-11 years); the Longitudinal Study of Australian Children's Child Health CheckPoint (LSAC-CP, Australia, n = 1874, 11-12 years); the Northern Fi

Heavy Metal Levels in Maternal Blood and Breast Milk and Associated Risk Factors.

OBJECTIVES: This study aimed to determine the concentrations of selected toxic metals in maternal blood and colostrum and to investigate their associations with environmental, lifestyle, and obstetric factors in postpartum women. METHODS: In this prospective observational study, 95 postpartum women who had delivered at term without any chronic or pregnancy-related comorbidities were included. Venous blood samples were collected from the antecubital vein, and approximately five to ten milliliters of colostrum were obtained from both breasts following breastfeeding within forty-eight hours after

The association of fractional cover, foliage projective cover and biodiversity with birthweight.

INTRODUCTION: Environmental exposures can contribute both benefits and risks to human health. Maternal exposure to green space has been associated with improvements in birthweight, among other birth outcomes. Newer measures of green space have been developed, which allows for an exploration of the effect of different ground covers (green, dry and bare earth), as well as measures of biodiversity. This study explores the association of these novel green space measures with birthweight in a large birth cohort in Queensland, Australia. METHODS: Birthweight was acquired from the routine health rec…

Prospective associations between household secondhand smoke exposure across critical developmental windows and early childhood caries: A birth cohort study.

BACKGROUND: Early childhood caries (ECC) is preventable, and household secondhand smoke (HSHS) is a modifiable risk factor. However, evidence regarding critical early-life exposure windows and potential sex differences in the association remains limited. OBJECTIVE: To examine the associations between HSHS exposure from early fetal life to 24 months of age and ECC in preschool children, and to explore potential sex differences. METHODS: Data from 2431 mother-child pairs in the Shanghai Maternal-Child Pairs Cohort were analyzed. HSHS exposure was self-reported during early and late pregnancy a

Association between passive smoking and dental caries status in children: A cross-sectional analytical study.

BACKGROUND: Several risk factors contribute to the development of dental caries in children, including sociodemographic, dietary, oral hygiene-related and other miscellaneous factors. Maternal smoking was highly associated with dental caries when compared to smoking by fathers or other household members. OBJECTIVES: The aim of the study was to determine the prevalence of dental caries and their association with exposure to environmental tobacco smoke (ETS) among 5- to 10-year-old students attending private and government schools. MATERIAL AND METHODS: A cross-sectional analytical study was c

MCT1-mediated bidirectional propionate transport across human placental syncytiotrophoblast layer: insights from a trophoblast stem cell-derived barrier model.

Propionate, a gut microbiota-derived short-chain fatty acid, influences fetal development and postnatal metabolic programming. Although the fetus lacks microbiota and endogenous propionate production, human pregnancies show a fetal-to-maternal propionate concentration ratio greater than unity, suggesting concentrative transport across the placenta. However, its underlying mechanism remains undefined. The present study aimed to identify the transporter responsible for transplacental transport of propionate across the syncytiotrophoblast (SynT) layer. Transporter knockdown in human choriocarcino

Does Antenatal Progesterone Administration Modify the Risk of Neonatal Intraventricular Hemorrhage?

OBJECTIVE: Progesterone administration has been associated with improved neurological outcomes following traumatic brain injury in adults. However, studies examining the effect of progesterone on the risk of neonatal intraventricular hemorrhage (IVH) are inconsistent. We sought to determine if maternal administration of intramuscular 17-α-hydroxyprogesterone caproate (17-OHPC) is associated with decreased rates of IVH in infants born before 32-weeks gestation. STUDY DESIGN: This is a retrospective study of liveborn singleton deliveries between 20- and 32-weeks gestation at two large academic
