We compared simultaneous, same-arm blood pressure (BPs) obtained with the Accutracker, an ambulatory blood pressure (BP) monitor, which uses R-wave gating for Korotkoff sounds to those of both a blinded, skilled clinician using a mercury column and a three-channel graphic display of cuff pressure tracings, Korotkoff sounds, and ECG tracing. Eighteen subjects, with a wide variety of BPs, heart rates, and ages, participated in the study. Abnormal cardiac rhythms appear more normal frequently in the elderly population. Studies confirm that the incidence of serious or life-threatening arrhythmias, including Mobitz type II atrioventricular block and ventricular tachycardia, increases the incidence of cardiac disease in this population group. As age progresses, the hemodynamic manifestations are more symptomatic and require specific therapy, including cardiac pacemakers and antiarrhythmic agents.

During human hepatic transplantation, ionized calcium (ICa) measured in whole blood with an ion-selective electrode varied greatly from ICa predicted from total calcium (TCa), protein, albumin, and pH, by means of recently published nomograms. Twelve patients between 14 and 26 years of age, ten with infectious mononucleosis and two with Guillain-Barré syndrome, all of whom were suspected of having had Epstein-Barr viral infection, developed daytime sleepiness. The daytime somnolence was confirmed objectively by polygraphic monitoring seven weeks to 4 1/2 months after the onset of clinical symptoms. The patients have been followed up for three to 12 years. We prospectively studied 33 patients with micturition syncope. Of these, eight were young (mean age, 25 years), healthy individuals whose diagnostic evaluation gave normal findings. The remaining 25 patients were older (mean age, 60 years), and 16 were women. This group had an average of 3.8 acute and chronic illnesses per patient, and was taking 3.5 medications per patient. Twenty-four children (aged 3 months to 14 years) with severe head injuries were treated by means of invasive neurointensive care for normalizing intracranial pressure (ICP) involving hyperventilation, control of body temperature, dexamethasone, barbiturates and continuous intracranial and arterial pressure monitoring. Holter monitoring was used to detect the underlying mechanism among 53 patients referred for dizziness, fainting and/or syncope. The complaints were unexplained on clinical grounds in 38, suggestive of SSS in 11, and of pacemaker dysfunction in 4 patients who underwent pacemaker implantation for symptomatic A-V block. Advances in neonatal medicine have markedly reduced mortality in preterm infants. Current emphasis should be placed on early detection and treatment of neonatal hypotension and shock. Ideal clinical management includes perinatal recognition of low birth weight infants at risk for hypotension. This review has focused on several aspects of monitoring post-injury patients.
