[Autoantibodies in thymoma-associated myasthenia gravis and their clinical significance].

Myasthenia gravis (MG) is characterized by the development of antibodies that act against the acetylcholine receptor (AChR) present at the postsynaptic site of neuromuscular junctions. Some MG patients have antibodies that bind in a cross-striational pattern to skeletal and heart muscle tissue sections (striational antibodies). These antibodies react with the epitopes on the muscle protein titin, ryanodine receptor (RyR), and voltage-gated K channel α subunit, Kv1.4. Since these 3 molecules are expressed in the thymoma tissue of MG patients, striational antibodies are frequently detected in th…

[Current status and future prospects of therapy for myasthenia gravis: considering thymectomy].

The benefits of thymectomy in non-thymomatous patients with myasthenia gravis (MG) remain controversial. The first detailed case of thymectomy in a patient with MG was reported in 1939, following which many cases were published. In 2000, Gronseth and Barohn reported the first meta-analysis of the effectiveness of thymectomy in MG patients without thymoma. They reviewed 28 papers systematically and reached these conclusions: (1) The benefit of thymectomy in non-thymomatous autoimmune MG has not been conclusively established, and (2) a well-designed controlled trial is essential. Following this …

[Immunosuppressive / immunomodulating therapies in myasthenia gravis-at present and in the near future].

Myasthenia gravis (MG) is an autoimmune disorder of neuromuscular transmission that is primarily caused autoantibodies specific to the neuromuscular junction and is characterized by fatigability and weakness of the striated muscles. Immunosuppression is the mainstay of treatment for MG, and patients with MG are currently treated with immunomodulating or immunosuppressive agents. This review summarizes the current situation and perspective of medical therapy for MG, focusing on mechanisms of action and clinical application of corticosteroids and different classes of immunomodulating or immunosu…

Increased serum interleukin-17 levels in patients with myasthenia gravis.

It has been suggested that interleukin-17 (IL-17) plays a crucial role in the development of several autoimmune diseases. However, there are no data about the relationship between myasthenia gravis and IL-17. The aim of this study was to measure the concentration of IL-17 and determine whether levels depend on the severity of MG. Serum IL-17 concentrations were measured in 25 patients. IL-17 concentrations were higher in generalized MG compared with controls and correlated with anti-acetylcholinesterase receptor antibody titers.

Change in myasthenia gravis epidemiology in Trento, Italy, after twenty years.

BACKGROUND: The recent literature suggests that the incidence and prevalence of myasthenia gravis (MG) are changing. We performed a population-based study of MG in the province of Trentino (524,826 inhabitants) and compared the results with those collected 20 years ago. METHODS: Multiple sources of information (discharge diagnosis, antibody tests and anticholinesterase drugs) were analyzed. Incidence was calculated from 2005 to 2009. Prevalence was calculated on December 31, 2009. Comparison was made with descriptive statistics for 1981-1990 for the identical region. RESULTS: Incidence and p…

Evaluation of serum IgG subclass concentrations in myasthenia gravis patients.

Myasthenia gravis (MG) is an autoimmune disease characterized by preferential production of a variety of autoantibodies. We aimed to investigate serum IgG subclass levels in MG patients. Serum IgG subclass levels from 85 MG patients were determined by the immunonephelometric assay. The median levels of IgG1, IgG2, IgG3, and IgG4 in MG patients were 7,360 mg/L, 5,750 mg/L, 860 mg/L, and 340 mg/L, respectively. When compared with normal controls, a significant increase of IgG1 (p = .000), IgG2 (p = .000), IgG3 (p = .000), and IgG4 (p = .029) was observed in MG patients. When the MG patients were…

Autoimmunity to endoplasmic reticulum chaperone GRP94 in myasthenia gravis.

Immune responses to ER stress have been closely related to the pathogenesis of autoimmune diseases. Using an immunoprecipitation assay, 24 (7.1%) of 336 MG serum samples immunoprecipitated a 90-kDa protein from the muscle cellular extracts, but none of the disease or healthy control sera. The 90-kDa protein was affinity-purified and found to match to ER chaperon GRP94 by matrix-assisted laser desorption/ionization-time of flight mass spectroscopy analysis. The frequency of associated autoimmune diseases was much higher in the anti-GRP94-positive than the -negative patients (71% versus 11%, p<0

Clinical characteristics and predictive factors of myasthenic crisis after thymectomy.

The aims of this study were to assess the clinical characteristics of myasthenic crisis after thymectomy (MCAT) and to identify predictors affecting the occurrence of MCAT. Of 66 patients with myasthenia gravis (MG), MCAT occurred in 20 patients (30.3%). The median time interval from thymectomy to MCAT was 3.4 months. MCAT occurred in 65.0% of patients within the first 6 months of a thymectomy, and 35.0% after 6 months. A second MCAT occurred in nine (45.0%) patients who survived the first MCAT, and in seven (50.0%) of 14 patients with a history of a preoperative myasthenic crisis before thyme…

Bedside testing for myasthenia gravis: the ice-test.

A 67-year-old man presented to the acute medical take with a history of droopy eyelids and difficulty swallowing. A diagnosis of myasthenia gravis was suspected from the clinical history. The patient's symptoms were progressive, raising concerns of aspiration pneumonia or respiratory compromise if untreated. Definitive diagnosis of myasthenia gravis relies upon a combination of serological and electrophysiological investigations, the results of which are not immediately available in an acute situation. Bedside tests are therefore of practical clinical importance to enable prompt and effective …

[Juvenile myasthenia].

INTRODUCTION: Juvenile myasthenia is a chronic autoimmune neuromuscular disease characterized by varying degrees of fluctuating, painless muscle weakness and rapid fatigue of any muscles under voluntary control. Juvenile myasthenia is a form of myasthenia appearing in adolescent age, representing 10% to 15% of all cases of myasthenia gravis. Juvenile myasthenia is presented by a defect in the transmission of nerve impulses to muscles, resulting from a breakdown in the normal communication between nerves and muscles. In myasthenia, antibodies produced by the body's own immune system block, alte…

Anti-LRP4 autoantibodies in AChR- and MuSK-antibody-negative myasthenia gravis.

Myasthenia gravis (MG) is an autoimmune disorder characterized by a defect in synaptic transmission at the neuromuscular junction causing fluctuating muscle weakness with a decremental response to repetitive nerve stimulation or altered jitter in single-fiber electromyography (EMG). Approximately 80% of all myasthenia gravis patients have autoantibodies against the nicotinic acetylcholine receptor in their serum. Autoantibodies against the tyrosine kinase muscle-specific kinase (MuSK) are responsible for 5-10% of all myasthenia gravis cases. The autoimmune target in the remaining cases is unkn…

Treatment of myasthenia gravis: focus on pyridostigmine.

Acquired myasthenia gravis (MG) is a chronic autoimmune disorder of the neuromuscular junction, characterized clinically by muscle weakness and abnormal fatigability on exertion. Current guidelines and recommendations for MG treatment are based largely on clinical experience, retrospective analyses and expert consensus. Available therapies include oral acetylcholinesterase (AChE) inhibitors for symptomatic treatment, and short- and long-term disease-modifying treatments. This review focuses on treatment of MG, mainly on the use of the AChE inhibitor pyridostigmine. Despite a lack of data from …

A single-blinded trial of methotrexate versus azathioprine as steroid-sparing agents in generalized myasthenia gravis.

BACKGROUND: Long-term immunosuppression is often required in myasthenia gravis (MG). There are no published trials using methotrexate (MTX) in MG. The steroid-sparing efficacy of azathioprine (AZA) has been demonstrated after 18-months of starting therapy. However, AZA is considered expensive in Africa. We evaluated the steroid-sparing efficacy of MTX (17.5 mg weekly) compared with AZA (2.5 mg/kg daily) in subjects recently diagnosed with generalized MG by assessing their average monthly prednisone requirements. METHODS: The primary outcome was the average daily prednisone requirement by mont…
