BK channel activity in skin fibroblasts from patients with neurological disorder.

Seventy-five unique variants in the KCNMA1 gene have been identified from individuals with neurological disorders. However, variant pathogenicity and evidence for disease causality are lacking in most cases. In this study, the KCNMA1 variants N999S and E656A (rs886039469 and rs149000684, respectively) were investigated from two individuals presenting with neurological disorders. N999S was previously shown to produce strong gain-of-function (GOF) changes in homomeric BK channel properties in vitro and is found as a heterozygous allele associated with epilepsy and paroxysmal dyskinesia in humans

A concise practical clinical guide to identifying spasticity in neurological shoulder dysfunction.

BACKGROUND: Spasticity is a known complication to the hemiplegic shoulder following acquired brain injury. However, there is a current discrepancy between the diagnosis of arm spasticity and the subsequent assessment and treatment of spasticity in people with shoulder involvement. OBJECTIVE: An expert panel aimed to establish a consensus and provide a simple structured approach to identifying and assessing people with spasticity of the hemiplegic shoulder. METHODS: A consensus group of six spasticity experts were interviewed individually via 1-h video calls to ascertain how they assess peopl

Neurological complications associated with chimeric antigen receptor T cell therapy.

Chimeric antigen receptor T (CAR-T) cells have made brilliant achievements in the treatment of many kinds of malignant tumors, and six kinds of CAR-T products have been approved by the Food and Drug Administration (FDA), bringing new hope for the treatment of diseases. However, the complications associated with CAR-T cell therapy should not be ignored. Neurological complications often jeopardize patients' lives, including immune effector cell-associated neurotoxicity syndrome, cerebrovascular accidents, movement and neurocognitive treatment-emergent adverse events. The current knowledge of the

[Epileptic seizures as a key neurological symptom: diagnosis, differential diagnosis and management].

Epileptic seizures rank among the most striking neurological key symptoms and pose a particular diagnostic challenge, especially when occurring for the first time. Differentiating between provoked, unprovoked seizures and epilepsy is essential for making appropriate therapeutic decisions. A thorough medical history, supported by EEG, MRI, and laboratory tests, usually allows for a well-founded classification and helps exclude important differential diagnoses such as syncope or psychogenic non-epileptic seizures. The International League Against Epilepsy (ILAE) classifies seizures based on thei

Left-right confusion in psychiatric, neurodevelopmental and neurological disorders: A systematic review.

Left-right confusion is common in healthy individuals, but increased prevalence has been reported in several psychiatric, neurodevelopmental, and neurological disorders. The present systematic review aimed to identify disorders in which the prevalence of left-right confusion is higher than in the general population. For this purpose, a systematic review following PRISMA guidelines was conducted using the databases Google Scholar, PubMed, and Mendeley. A total of 20 quantitative case-control studies were included. Lesions in parietal brain areas and damage to temporal-parietal-occipital brain a

Neurological complications following cerebral angiography: a case of contrast-induced encephalopathy.

BACKGROUND: Contrast-induced encephalopathy (CIE) is a rare but increasingly recognized iatrogenic complication associated with the use of intravascular iodinated contrast agents, particularly during neurovascular procedures. It often presents with acute neurological deficits that mimic ischemic strokes or seizure disorders. Despite its often reversible course, radiological and electroclinical mimicry of other acute conditions poses a diagnostic dilemma. CASE PRESENTATION: We report the case of an 82-year-old woman with multiple vascular comorbidities who developed acute left hemiparesis and c

Increase of nitrous oxide-induced neurological disorders - a German multicenter experience.

BACKGROUND: Nitrous oxide (N₂O), commonly known as laughing gas, is widely recognized for its anesthetic and analgesic effects, and is frequently used in medical contexts. However, its misuse can lead to significant neurological complications, which are often under-recognized in clinical practice. Recent data on such cases in Germany are rare. We here report the spectrum of neurological complications associated with the recreational use of N₂O, as encountered in German neurology centers. METHODS: We retrospectively analyzed of 23 cases presenting with neurological symptoms following N₂O abuse…

Historical Perspectives of Parkinson's Disease: Early Clinical Descriptions and Neurological Therapies.

Although components of possible Parkinson's disease can be found in earlier documents, the first clear medical description was written in 1817 by James Parkinson. In the mid-1800s, Jean-Martin Charcot was particularly influential in refining and expanding this early description and in disseminating information internationally about Parkinson's disease. He separated the clinical spectrum of Parkinson's disease from multiple sclerosis and other disorders characterized by tremor, and he recognized cases that later would likely be classified among the parkinsonism-plus syndromes. Early treatments

Association Between Rewarming Rate and Survival and Neurologic Outcome of Accidental Hypothermia.

OBJECTIVES: Accidental hypothermia has high mortality. Rewarming is the initial primary management strategy. However, detailed evidence on rewarming management is limited, that is, rewarming rate is unclear, particularly with noncardiac arrest. Here, we evaluated the association between rewarming rate in the early phase of rewarming and survival and neurologic outcomes in patients with accidental hypothermia. DESIGN: A secondary analysis of a nationwide, multicenter, prospective, observational study-the Intensive Care with ExtraCorporeal membrane oxygenation Rewarming in Accidentally Severe H

Long-term neurological consequences following benzodiazepine exposure: A scoping review.

Benzodiazepine acute withdrawal syndrome is well known, but the long-term neurological consequences of benzodiazepine exposure are much less familiar. A scoping review was conducted of electronic databases for studies that reported on patient outcomes four or more weeks after complete cessation of benzodiazepine use. Forty-six results were retrieved in total, some of which provided signals for protracted symptoms, often reported as incidental findings, and others that showed benzodiazepine discontinuation was beneficial. Some overlap occurred in the outcomes, but these two groups of studies su

Impact of Marine Microplastics on Neurologic and Functional Disabilities: A Population-Level Study.

BACKGROUND: Microplastics are emerging as environmental pollutants with potential neurotoxic effects, yet their association with neurological disabilities remains largely unexplored. METHODS: In this cross-sectional study comprising 218 coastal counties in the United States, we compared the self-reported prevalence of cognitive disability, mobility disability, self-care disability, and independent living disability in counties with very high and low marine microplastic levels (MMLs). Unadjusted and adjusted prevalence ratios (PRs) were computed using population-weighted quasi-Poisson regressi

[Efficacy of Recognan in the therapy of neurological disorders after traumatic brain injury].

Traumatic brain injury (TBI) is a brain injury caused by external mechanical stress, which leads to a deterioration in the patient's health (temporary or irreversible), and is the main cause of death and disability. Along with neurodegenerative and cerebrovascular diseases, TBI is one of the main causes of cognitive impairment, as a result of injury. Among the neuroprotectors prescribed for the correction of neurological disorders after a TBI, the medicine drug (MD) Recognan (citicoline), which has neuroprotective and reparative effects, is of particular interest. The effectiveness of citicoli

Characterisation of Challenging Behaviours and Associated Genetic and Neurological Features in Cardiofaciocutaneous Syndrome.

BACKGROUND: Challenging behaviours such as self-injury and aggression are prevalent among individuals with intellectual disability (ID), significantly impacting quality of life. Cardiofaciocutaneous syndrome (CFCS), a rare multisystem genetic disorder caused by variants in the BRAF, MAP2K1, MAP2K2, or KRAS genes, commonly presents with ID and other neurobehavioural features. To inform effective clinical management, we aimed to characterise and quantify challenging and repetitive behaviours in CFCS, identify functions that may maintain the behaviours, and examine associations with genotype and

Stress hyperglycemia ratio is associated with neurological outcome after cardiac arrest.

This study examined the association between glycemic parameters, including absolute glucose and stress hyperglycemia ratio (SHR), and neurological outcomes in out-of-hospital cardiac arrest (OHCA) survivors. We conducted a retrospective analysis using data from a prospective, multicenter registry comprising 743 adult comatose OHCA survivors. Glycemic measurements (glucose and SHR) were collected following return of spontaneous circulation (ROSC) and during the subsequent 24-hour period. The primary outcome was poor neurological outcome at 6 months. Subgroup analyses were stratified by glycemic

Severe Myxedema Coma With Neurological, Cardiac, and Renal Involvement: A Case Report.

Myxedema coma is the most severe manifestation of hypothyroidism, with high mortality despite treatment. It typically affects elderly women and is often triggered by infection, drug exposure, or environmental stressors. We report the case of a 66-year-old woman with no prior history of thyroid disease who presented with progressive neurological deterioration, hypothermia, bradycardia, hypotension, and multiorgan dysfunction. Laboratory evaluation confirmed severe hypothyroidism with elevated thyroid-stimulating hormone and undetectable free T4, alongside markedly elevated cardiac enzymes, glob

Determinants of retinopathy and short-term neurological outcomes after cerebral malaria.

Neurological abnormalities are frequent after cerebral malaria (CM) resolves. The identification of survivors that should be prioritized during follow-up after CM is necessary for post-hospitalization care. We analysed social, clinical, and immune determinants of malarial retinopathy (MR) and short-term neurological outcomes after CM. Children aged 24 to 71 months with CM were prospectively followed-up until 28 days after admission at two hospitals in Benin. Direct ophthalmoscopy was performed shortly after admission. Plasma biomarkers were measured at admission. A neurocognitive deficit scree

Neurologic symptoms as a hallmark of glymphatic alteration in recovered patients with COVID-19.

BACKGROUND: The glymphatic system is a glial-based perivascular network that facilitates the clearance of metabolic waste from the brain. Dysfunction of the glymphatic system, along with neurological symptoms such as cognitive deficits and olfactory dysfunction, has been reported in patients with coronavirus disease (COVID-19). However, the link between these neurological symptoms and alterations in the glymphatic system remains unclear. In this study, we aimed to evaluate magnetic resonance imaging (MRI)-based measures of the glymphatic system in patients recovered from COVID-19 with and with

Persisting Symptoms After Concussion and Functional Neurological Disorder: Points of Intersection.

Persistent symptoms after concussion (PSaC) and functional neurological disorder (FND) are frequently encountered in clinical practice and are often challenging to manage due to heterogeneous and polysymptomatic presentations, as well as fragmented care pathways. This review outlines key points of intersection between PSaC and FND across pathophysiology, illness beliefs, rehabilitation models, and emerging treatments. We describe when FND should be considered in the differential diagnosis of patients with PSaC, and provide guidance on history-taking, examination, diagnostic communication, and

A Newly Defined, Common Ophthalmologic Condition Requires Special Neurological Attention.

A host of acquired abnormalities in visual function are known to occur in persons who suffer stroke, traumatic brain injury, and neurodegenerative disorders. Cerebral visual impairment occurs in children with early neurological injury or disorders, especially neonatal hypoxic-ischemic injury. With the improved survival of pre-term infants through meticulous neonatal intensive care unit care, cerebral visual impairment in children has become much more common in developed countries. In recent months a number of National Institutes of Health institutes and the American Academy of Pediatrics have
