Characteristics of thiamin and its relevance to the management of heart failure.

Heart failure (HF) is a major public health problem in the United States that puts a significant burden on both patients and the healthcare system. The prevalence of malnutrition in HF patients is well-known and correlates with a dramatic decline in quality of life and disease progression, and is associated with high morbidity and mortality rates. The implication of HF on micronutrient status is underrecognized in the quest to offer "best practice" medical, device, and surgical interventions to this population. The micronutrient thiamin is of particular interest in the management of HF for sev

Population living in the Red Sea State of Sudan may need urgent intervention to correct the excess dietary iodine intake.

BACKGROUND: Both inadequate and high intakes of iodine are associated with thyroid disease and associated abnormalities. Consumption of foods deficient in iodine induces hypothyroidism. Conversely, excessive intake of the nutrient precipitates hyperthyroidism. Iodine deficiency causes impairment of thyroid hormonogenesis resulting in goiter (struma), cretinism which is associated with increased prenatal and infant mortality, deafness, motor disabilities and mental retardation due to damage during fetal and neonatal brain development. We have assessed the iodine status of school children from t

Critical review of strategies to prevent and control iron deficiency in children.

Iron deficiency is prevalent in infants and young children in developing countries and is associated with adverse developmental outcomes. The routine provision of additional iron by food fortification or the use of iron supplements is generally recommended. The wisdom of this approach in regions where the transmission of Plasmodium falciparum malaria is perennial and intense is now being questioned, because a large trial in Pemba, Tanzania, demonstrated an increased risk of serious morbidity among children under the age of 3 years who were given routine daily iron and folic acid supplements. Lipids in the nutritional support of the critically ill patients.

PURPOSE OF REVIEW: This review reports recent findings on lipid use in artificial nutrition in patients with acute respiratory failure or severe sepsis or undergoing major surgery. It examines current knowledge of fatty acid safety, biologic effects, and the impact on patients' morbidity and mortality. The newly emerging area of genotypic influence and timing of immunonutrition is also discussed. RECENT FINDINGS: In acute respiratory distress syndrome, the debate concerning the use of long-chain fatty acids as opposed to physical mixtures of medium- and long-chain fatty acids, specifically re

Maternal growth hormone treatment from day 35 to 80 of gestation alters nutrient partitioning in favor of uteroplacental growth in the overnourished adolescent sheep.

Overnourishing the pregnant adolescent ewe promotes maternal tissue synthesis at the expense of placental growth and leads to a major reduction in lamb birth weight at term. Growth hormone (GH) secretion is attenuated in these overnourished dams and the maternal somatotrophic axis may play a key role in coordinating nutrient usage in the pregnant adolescent. Thus we investigated whether increasing maternal GH during the period of rapid placental proliferation alters nutrient partitioning between the maternal, placental, and fetal tissues as assessed at Day 81 of gestation. Adolescent recipient

Acid-base balance and nutrition in peritoneal dialysis.

Acidosis has been implicated in increased protein catabolism and malnutrition of dialysis. The present study examines the effect of acid-base balance on the nutrition status of peritoneal dialysis (PD) patients. We followed 43 PD patients for one year. Blood chemistries were measured monthly. Patients were divided on the basis of subjective global assessment (SGA) into well-nourished (A), mildly-to-moderately malnourished (B), and severely malnourished (C) groups. Mean serum bicarbonate and albumin concentrations were, for group A (n = 16), 23.5 mmol/L and 3.96 g/dL respectively; for group B (

Can total parenteral nutrition reverse hypoalbuminemia in oncology patients?

To determine if hypoalbuminemia is a consequence of protein-energy malnutrition rather than an associated feature of cancer, we reviewed results when vigorous nutritional support was given to 13 men and 9 women who had solid tumors (head and neck [10], gastrointestinal [8], other [4]) and an initial serum albumin less than 3.5 g/dl. Patients had received a minimum of 21 days of nutritional support with total parenteral nutrition (TPN) supplying an average daily energy intake of at least 1.5 x basal energy expenditure (BEE). The TPN was started at a dosage commensurate with the patient's estima

Increased risk of respiratory disease and diarrhea in children with preexisting mild vitamin A deficiency.

Preschool-age rural Indonesian children were reexamined every 3 months for 18 months. An average of 3135 children were free of respiratory disease and or diarrhea at the examination initiating each of the six, 3-month follow-up intervals. Children with mild xerophthalmia (night blindness and/or Bitot's spots) at the start and end of an interval developed respiratory disease and diarrhea at twice (p less than 0.001) and three times (p less than 0.001) the rate, respectively, of children with normal eyes during the same interval, independent of age and anthropometric status (weight for length).

Iron deficiency is associated with food insecurity in pregnant females in the United States: National Health and Nutrition Examination Survey 1999-2010.

Food-insecure pregnant females may be at greater risk of iron deficiency (ID) because nutrition needs increase and more resources are needed to secure food during pregnancy. This may result in a higher risk of infant low birth weight and possibly cognitive impairment in the neonate. The relationships of food insecurity and poverty income ratio (PIR) with iron intake and ID among pregnant females in the United States were investigated using National Health and Nutrition Examination Survey 1999-2010 data (n=1,045). Food security status was classified using the US Food Security Survey Module. One

The polypharmacy-xerostomia-nutrition triad: the clinical framework for older adults.

BACKGROUND: Polypharmacy, the concurrent use of five or more medications, affects the majority of community-dwelling older adults aged 65 and older in the United States. A clinically underrecognized consequence is medication-induced xerostomia and salivary hypofunction, which critically impair oral function, food selection, and nutritional status. OBJECTIVE: This mini-review synthesizes mechanistic and clinical evidence linking polypharmacy-induced xerostomia to nutritional decline in older adults and proposes a clinical framework for integrated dental-medical screening. METHODS: A narrative

Rheological and Technological Design of Texture-Modified Food Systems for Dysphagia Management in Amyotrophic Lateral Sclerosis.

Amyotrophic lateral sclerosis (ALS) is frequently accompanied by progressive dysphagia, weight loss, and hypermetabolism, which complicate the maintenance of adequate nutritional status. For these patients, food systems must not only provide sufficient energy and protein within a limited volume but also exhibit textural and rheological properties that support safe swallowing. This review analyzes technological and rheological approaches to the design of texture-modified food systems for nutritional support in ALS. Particular attention is given to apparent viscosity, shear-thinning behavior, yi

Beyond mobility: A prospective study on diet and metabolism in hereditary spastic paraplegia.

Metabolism plays an important role in neurodegenerative diseases. Hereditary spastic paraplegias (HSP) are a heterogeneous group of rare genetic neurodegenerative disorders, commonly characterized by the clinical syndrome of progressive lower limb spasticity and mobility loss. Obesity has been linked to distinct genotypes, but the role of metabolism and nutrition in HSP remains unclear. We aimed to To evaluate metabolism and nutrition in specific HSP genotypes and to assess the impact of nutritional counseling on disease progression and body composition. In this prospective explorative pilot st

TNF-α is associated with low phase angle in male patients with chronic hepatitis C.

BACKGROUND: The potential role of Phase angle (PhA) as a biomarker of systemic inflammation in chronic hepatitis C (CHC) remains underexplored. OBJECTIVE: To assess the association between PhA values and the inflammatory marker-tumour necrosis factor alpha (TNF‑α) in patients with CHC. In addition, associations between PhA and a set of demographics, clinical, nutritional, and biochemical data were evaluated. METHODS: This cross-sectional study included 81 adults with confirmed CHC (72.8% male; mean age 50.0 ± 11.7 y; 30.9% with liver cirrhosis). Nutritional status and body composition were a

Treatment with CFTR Modulators for Cystic Fibrosis: What a Pediatric Gastroenterologist Needs to Know.

Background: Cystic fibrosis (CF) is a multisystemic disorder caused by CFTR gene mutations, leading to impaired protein function and affecting pulmonary, gastrointestinal, hepatobiliary, skeletal, and nutritional health. The advent of CFTR modulators-especially the triple therapy elexacaftor/tezacaftor/ivacaftor (ETI)-has revolutionized clinical management, offering genotype-specific benefits beyond pulmonary outcomes. Pediatric gastroenterologists must now recognize and address emerging gastrointestinal and nutritional challenges introduced by modulator therapy. Methods: A narrative review wa

Effects of nutritional intervention on total protein and albumin levels in patients with maxillofacial trauma receiving intermaxillary fixation: a randomized controlled trial.

BACKGROUND AND OBJECTIVE: Maxillomandibular fixation (MMF) serves as a conventional approach for immobilizing jaws in the treatment of maxillofacial fractures. However, intermaxillary fixation poses challenges to nutritional status in the immediate postoperative phase, resulting in the depletion of water, fat, and protein reserves. The objective is to evaluate the variations in total protein and albumin levels at distinct intervals throughout the treatment duration, aiming to elucidate the impact of intermaxillary fixation on nutritional markers. METHODOLOGY: A total of 144 patients with jaw
