Characterization of postoperative edema following laser-assisted uvulopalatoplasty using MRI and polysomnography: implications for the outpatient treatment of obstructive sleep apnea syndrome.

Laser-assisted uvulopalatoplasty (LAUP) has been introduced as an alternative to uvuloplatopharyngoplasty for the treatment of snoring. Despite limited study, the use of this procedure has been expanded to include patients with obstructive sleep apnea syndrome. Although the potential cost-savings of performing sleep apnea surgery on an outpatient basis are self-evident, concern exists regarding the safety of this practice. In an effort to characterize the risk of performing LAUP on an outpatient basis, eight patients with mild or no sleep apnea were identified and intensively studied before an

Reasons for encounter and recommended interventions in an interprofessional integrative oncology counseling program in outpatient care: a mixed-methods analysis of 1,693 consultation letters.

INTRODUCTION: Complementary and integrative healthcare (CIH) approaches play an increasingly important role in oncology care, involving a wide range of healthcare professionals and therapeutic methods. While patient demand for CIH and the evidence base for individual interventions have grown considerably, the implementation of structured counseling services for the effective and safe use of CIH remains limited. To address this gap, a controlled, multicenter implementation study was conducted at four Comprehensive Cancer Centers (CCC) in Germany to establish an outpatient, interprofessional, ev

Statin Prescribing and Guideline Concordance Among Adults With HIV Aged 40-75 Years in a Single Urban Outpatient HIV Clinic, 2024.

BACKGROUND: People with HIV (PWH) have elevated atherosclerotic cardiovascular disease (ASCVD) risk. Following the REPRIEVE trial, 2024 HHS Adult and Adolescent Antiretroviral Guidelines recommend at least moderate-intensity statins for adults with HIV aged 40-75 years with 10-year ASCVD risk 5% to <20%. The 2026 ACC/AHA Dyslipidemia Guideline expands eligibility, supporting statins for all adults aged 40-75 years with HIV for primary prevention, regardless of LDL-C. We evaluated statin prescribing and HHS guideline concordance for primary prevention in this population. SETTING: Single urban

Caring, pragmatic and discriminatory: how private paediatric outpatient providers normalise unequal care for Roma-ethnographic findings from affluent Slovakia.

BACKGROUND: Healthcare providers may enact discriminatory practices while perceiving themselves as caring and pragmatic. Whilst anti-Roma discrimination in Central and Eastern European healthcare has been documented in hospital and emergency settings in poorer regions, less is known about routine outpatient care in affluent areas. This ethnographic study examines how private paediatric outpatient providers in Slovakia’s most affluent region normalise unequal care for Roma families through everyday practices rationalised as contextually appropriate responses. METHODS: The study was conducted be

A novel meropenem dosing strategy for Outpatient Parenteral Antimicrobial Therapy (OPAT): a pilot pharmacokinetic/pharmacodynamic study in non-critically ill patients.

Meropenem is a valuable option for outpatient parenteral antimicrobial therapy (OPAT) due to its broad-spectrum activity, including multidrug-resistant Gram-negative bacteria, particularly when no oral alternatives exist. However, its conventional thrice-daily dosing poses logistical challenges for home administration, as the third dose typically falls during nighttime. This prospective pharmacokinetic study evaluated the adequacy of a novel daytime-only meropenem regimen adapted for OPAT in non-critically ill hospitalized patients. Twenty-two patients receiving meropenem 2 g every 8 h were sw

Beyond early termination: Predicting quality-related vs. quality-neutral dropout during the active phase of outpatient cognitive behavioral therapy.

AbstractObjective: This study examined predictors of dropout during the active phase of outpatient CBT, distinguishing between quality-related and quality-neutral dropouts. The focus was on patient, process, and therapist variables, including the therapeutic alliance and early treatment courses.Method: Routine data from 590 outpatient psychotherapy patients were analyzed. Dropouts were classified as quality-related (dissatisfaction or lack of progress) or quality-neutral (external reasons). Logistic regression models identified predictors of quality-related dropout. Symptom improvement differe
