The public health foundation of health services for American Indians & Alaska Natives.

The integration of public health practices with federal health care for American Indians and Alaska Natives (AI/ANs) largely derives from three major factors: the sovereign nature of AI/AN tribes, the sociocultural characteristics exhibited by the tribes, and that AI/ANs are distinct populations residing in defined geographic areas. The earliest services consisted of smallpox vaccination to a few AI/AN groups, a purely public health endeavor. Later, emphasis on public health was codified in the Snyder Act of 1921, which provided for, among other things, conservation of the health of AI/AN pers

[Blastocystis spp. culturing as a method for diagnosing the parasite].

The technique for isolating blastocysts in the culture was assessed, by examining the fresh specimens from 196 patients with gastrointestinal tract (GIT) disorders and from 56 persons with an uncertain protozoological diagnosis. A set of techniques, such as triple examination of native smears, parallel inoculation of the Pavlova medium, and microscopy of obtained permanent specimens as may be required, contributes to the timely identification of protozoa, including blastocysts, in persons with GIT disorder. The efficiency of the technique for isolating blastocysts on Pavlova's medium and the n

Diagnosis of Cryptococcosis and Prevention of Cryptococcal Meningitis Using a Novel Point-of-Care Lateral Flow Assay.

Despite access to antiretroviral therapy, mortality from cryptococcal meningitis (CM) is high among persons with advanced HIV infection in sub-Saharan Africa. Cryptococcal antigen (CrAg) is present several weeks to months before the onset of symptoms of meningitis and can be screened to prevent life threatening meningitis. Recently, the World Health Organisation recommended that a new rapid CrAg lateral flow ''dipstick" assay (LFA) is to be used to screen HIV-infected persons with CD4 counts of less than 100 cells/µL. In this paper, we describe two cases of cryptococcosis with differing outcom

Parental psychopathology and offspring suicidality in Mexico.

The objective of this study was to estimate the extent to which parental psychopathology may confer increased risk of suicide ideation and attempts among their offspring in Mexico. Data from a representative sample of 5,782 respondents participating in the Mexican National Comorbidity Survey (2001-2002) to examine the unique associations between parental psychopathology and offspring suicidality were used. Parental disorders (major depression, panic disorder, generalized anxiety disorder, substance dependence, and antisocial personality disorder) were comorbid and after controlling for comorbi

[Behavioural problems and personality change related to cerebral amyloid angiopathy].

OBJECTIVE: Cerebral amyloid angiopathy (CAA) belongs to the group of amyloidoses that are characterized by the deposition of insoluble and tissue-damaging amyloid proteins. Spontaneous intracerebral hemorrhage is the common clinical presentation of CAA resulting from the degenerative effect of beta amyloid on the cerebral vascular system. Though CAA is rather a neurological disease psychiatric symptoms can occur and even dominate the clinical picture. METHODS: A case report is presented in order to illustrate the association between CAA and psychiatric symptoms. RESULTS: We report the case o

Two persons with multiple disabilities use camera-based microswitch technology to control stimulation with small mouth and eyelid responses.

BACKGROUND: A camera-based microswitch technology was recently developed to monitor small facial responses of persons with multiple disabilities and allow those responses to control environmental stimulation. This study assessed such a technology with 2 new participants using slight variations of previous responses. METHOD: The technology involved a computer with a CPU using a 2GHz clock, a USB video camera with 16-mm lens, and special software. Small colour spots were used under the lower lip of one participant and on the eyelid of the other participant to aid the camera and computer to dete

Personalized therapies in pediatric inflammatory and autoimmune diseases.

Pediatric inflammatory and autoimmune diseases are a wide array of systemic or organ-specific conditions, characterized by an exaggerated immune reactivity, which generally occurs in immunogenetically predisposed children. Among the most important ones, in terms of their diffusion and morbidity in the population worldwide, pediatric inflammatory bowel disease (IBD) and juvenile rheumatoid arthritis (JRA) have to be considered. The aim of personalized therapy is to give to each patient the most appropriate drug and dose regimen, in order to maximize treatment response and reduce the risk of adv

Travel medicine and vaccines for HIV-infected travelers.

For the purposes of vaccination, persons with asymptomatic HIV infection and CD4+ cell counts of 200/μL to 500/μL are considered to have limited immune deficits and are generally candidates for immunization. HIVinfected persons with CD4+ cell counts less than 200/μL or history of an AIDS-defining illness should not receive live-attenuated viral or bacterial vaccines because of the risk of serious systemic disease and suboptimal response to vaccination. Available data indicate that immunization during antiretroviral therapy restores vaccine immunogenicity, improves the rate and persistence of i

[Noroviruses: leading cause of gastroenteritis].

Although noroviruses were the first viral agents to be linked to gastrointestinal disease, they were long considered a secondary cause far behind rotaviruses. Development of molecular-based diagnostic techniques has provided clearer insight into the epidemiological impact of noroviruses that are now recognized not only as the leading cause of non-bacterial gastroenteritis outbreaks but also as an important cause of sporadic gastroenteritis in both children and adults. Norovirus infection is generally characterized by mild acute vomiting and diarrhea usually lasting for only a few days, but it

[Compressive-spectral analysis of EEG in patients with panic attacks in the context of different psychiatric diseases].

Panic disorders (PD) which develop in the context of different psychiatric diseases (neurotic, personality disorder and schizotypal disorders) have their own clinical and neurophysiological features. The results of compressive-spectral analysis of EEG (CSA EEG) in patients with panic attack were different depending on the specifics of initial psychiatric status. EEG parameters in patients differed from those in controls. The common feature for all PD patients was the lower spectral density of theta-, alpha- and beta-bands as well as total spectral density without any alterations of region dist

[Transsexuality: living in a wrong body?].

Transsexuality is a permanent feeling of uneasiness and non-affiliation to the gender in which a person was born, accompanied with a permanent striving to live and be accepted as a person of the opposite gender. The dysfunction appears in childhood and adult age. There is at least one transsexual male in 30,000 adult men and one transsexual female in 100,000 adult women. The aetiology of the dysfunction is still not elucidated, although there are hints connecting transsexuality with a non-standard hormonal activity in prenatal and short perinatal phase of development, which determines an erron

Self-stigma, empowerment, and perceived legitimacy of discrimination among women with mental illness.

OBJECTIVE: The study sought to better understand why some people with mental illness self-stigmatize and develop low self-esteem while others remain indifferent to stigma or respond with a sense of empowerment. The authors hypothesized that a high level of perceived discrimination, little sense of identification with the group of people with mental illness, and a high level of perceived legitimacy of discrimination lead to self-stigma. METHODS: Sixty women with borderline personality disorder and 30 women with social phobia, who were recruited at three centers in Germany and Switzerland, comp

Plague: from natural disease to bioterrorism.

Yersinia pestis is the causative agent of plague, an enzootic vectorborne disease usually infecting rodents (rats) and fleas. Humans can become infected after being bitten by fleas that have fed on infected rodents. In humans, the disease usually occurs in the form of bubonic plague. In rare cases, the infection spreads to the lungs via the bloodstream and causes secondary pneumonic plague. Person-to-person transmission has been described for pneumonic plague but is rare in primary bubonic plague. Bubonic plague can usually be treated successfully with antibiotics; however, pneumonic plague de

PCR-ELISA: a new simplified tool for tracing the source of cryptosporidiosis in HIV-positive patients.

Cryptosporidium parvum is a major parasitic cause of death in end-stage AIDS patients that results from both zoonotic and person-to-person transmission. Recent studies have provided evidence that parasites causing zoonotic disease and those causing anthroponotic infection are genetically distinct. Isolates carrying "animal"-type genetic markers were presumed to be the result of zoonotic spread, either directly or through contaminated food and water. The need for a genotype-specific diagnostic tool that can provide clues as to the origin and possible modes of spread of C. parvum strains has bee

Steatorrhea and weight loss in a 72-year-old man: primary biliary cirrhosis? Celiac disease? Bacterial overgrowth? What else?

Unintentional weight loss is an ominous sign, particularly when it occurs in the elderly; concern for malignancy is especially worrisome. In this report, we describe a 72-yr-old man who presented with weight loss and was found to have massive steatorrhea. An extensive evaluation revealed evidence of primary biliary cirrhosis (PBC), celiac disease, and small intestinal bacterial overgrowth. No malignancy was identified. The weight loss was attributed to severe steatorrhea due, in part, to intraluminal bile salt deficiency, small bowel mucosal disease, and bacterial overgrowth. Several points ar

A hostility scale for the California Psychological Inventory: MMPI, observer Q-sort, and big-five correlates.

Using two samples, we developed and validated a hostility scale that can be scored from the California Psychological Inventory (CPI) and serves as an alternate for the Cook-Medley Hostility Scale (Ho; Cook & Medley, 1954). The CPI Hostility (H) scale consists of 33 items that are either duplicates or close equivalents of specific Ho items, and the two scales correlate at least .90 in samples differing in sex. The H and Ho scales show a similar pattern of correlations with conceptually relevant MMPI scales and with observer-rated personality attributes tapping Barefoot, Peterson, et al.'s (1991

Tentative findings of a study of the technology needs and use patterns of persons with mental retardation.

An examination of the technology use patterns and needs of 680 persons with mental retardation was conducted as part of a grant application process in response to P. L. 100-407, the Technology-Related Assistance for Individuals with Disabilities Act of 1988. Important trends which were deemed valuable for state technology planning purposes were revealed which have implications for other states desiring to develop comprehensive technology systems. Responses obtained from participants indicated that needs for technology were evident in all areas of life functioning, with computer technology bein

Rheumatoid factors antedating clinical rheumatoid arthritis.

The relationship between rheumatoid factors (RF) and rheumatoid arthritis (RA) was studied in the Mini-Finland Health Survey. This covered a representative sample of the Finnish population over 30 years of age, initially comprising 8,000 persons, of whom 7,217 participated in the field survey carried out in 1978-80. The participants were followed using record linkage with the Social Insurance Institution's population register to identify patients entitled to free medication. Until the end of 1988, 21 persons had developed new seropositive RA. In 15 cases, preillness specimens contained elevate

Changes in ocular onchocerciasis four and twelve months after community-based treatment with ivermectin in a holoendemic onchocerciasis focus.

The impact of ivermectin mass treatment on ocular onchocerciasis was studied in a holoendemic focus of blinding onchocerciasis in Ghana. A cohort of 417 persons, 369 of whom were treated, was followed up at 4 and 12 months after treatment. The mean ocular microfilarial load in the anterior chamber of the eye and in the cornea of treated persons was reduced to less than 20% and 10% of the pretreatment levels respectively at the 4 months follow-up but had increased significantly by 12 months. Lesions of the eye at the advanced stage of development remained stable. There was significant regressio
