A 3 months mild functional test regime does not affect disease parameters in young mdx mice.

To assess the effect of potential therapeutic agents in dystrophic mice it is useful to have a functional test regime that does not affect the natural disease progression of mdx mice with dystrophinopathy. We determined the effect of a 12 week test regime consisting of fore limb grip strength, rotarod analysis and two and four limb hanging wire tests on the disease progression of 4-week-old mdx mice. Mice performed the different functional tests on consecutive days on a weekly basis. No difference was found in serum creatine kinase levels between functionally active and sedentary mice. The per

The benefits of intravenous thrombolysis relate to the site of baseline arterial occlusion in the Echoplanar Imaging Thrombolytic Evaluation Trial (EPITHET).

BACKGROUND AND PURPOSE: In ischemic stroke, the site of arterial obstruction has been shown to influence recanalization and clinical outcomes. However, this has not been studied in randomized controlled trials, nor has the impact of arterial obstruction site on reperfusion and infarct growth been assessed. We studied the influence of site and degree of arterial obstruction patients enrolled in the Echoplanar Imaging Thrombolytic Evaluation Trial (EPITHET). METHODS: EPITHET was a prospective, randomized, placebo-controlled trial of intravenous tissue plasminogen activator (tPA) in the 3- to 6-

[Simultaneous carotid endarterectomy and cardiac surgery-additional risk factor or safe procedure?].

Источник пометил публикацию как отозванную

BACKGROUND: The occurrence of severe carotid artery disease in more than 12% of patients requiring ACB results in a discrepancy concerning the best treatment for both diseases. We reviewed the early outcome of patients with ACB and/or valve replacement and simultaneous carotid endarterectomy (TEA). METHODS: We evaluated retrospectively 244 patients operated simultaneously between 7/94 and 10/2001: 209 patients received ACB, 35 patients ACB and/or valve replacement. Mean age was 68 years. 188 patients were male. We analyzed risk factors, morbidity, incidence of neurological complications and 3

Adjuvant radiotherapy with and without concurrent chemotherapy for locally advanced transitional cell carcinoma of the renal pelvis and ureter.

PURPOSE: Transitional cell carcinoma of the upper urinary tract is a relatively uncommon malignancy. The role of adjuvant radiation therapy and chemotherapy is not well defined. We retrospectively reviewed the records of 31 patients who underwent surgery followed by adjuvant radiotherapy with or without concurrent chemotherapy to determine overall outcome as well as impact of concurrent chemotherapy administration. MATERIALS AND METHODS: Between 1970 and 1997, 31 patients with nonmetastatic transitional cell carcinoma of the upper urinary tract (renal pelvis in 13, ureter in 15, and renal pel

Optimizing hepatitis B diagnosis for mothers in a low-resource setting: A field pilot of Xpert point-of-care viral load testing in Ugandan antenatal clinics.

Mother-to-child transmission accounts for most new hepatitis B (HBV) infections in high-burden regions globally. The World Health Organization recommends antiviral prophylaxis for pregnant women with high hepatitis B viral load levels, but access to viral load testing remains limited and untimely in many countries, due to centralized testing. We assessed the operational feasibility, accessibility, and cost of implementing the Xpert HBV DNA assay in antenatal care settings. This was a two-phase study at 10 high-volume hospitals across nine regions of Uganda. Phase I verified the diagnostic accu

How has the impact of 'care pathway technologies' on service integration in stroke care been measured and what is the strength of the evidence to support their effectiveness in this respect?

UNLABELLED: EXECUTIVE SUMMARY: BACKGROUND: Across the developed world, we are witnessing an increasing emphasis on the need for more closely coordinated forms of health and social care provision. Integrated care pathways (ICPs) have emerged as a response to this aspiration and are believed by many to address the factors which contribute to service integration. ICPs map out a patient's journey, providing coordination of services for users. They aim to have: 'the right people, doing the right things, in the right order, at the right time, in the right place, with the right outcome'. The value f

Diagnostic management strategies for adults and children with minor head injury: a systematic review and an economic evaluation.

BACKGROUND: Patients with minor head injury [Glasgow Coma Scale (GCS) score 13-15] have a small but important risk of intracranial injury (ICI) that requires early identification and neurosurgical treatment. Diagnostic assessment can use either a clinical decision rule or unstructured assessment of individual clinical features to identify those who are at risk of ICI and in need of computerised tomography (CT) scanning and/or hospital admission. Selective use of CT investigations helps minimise unnecessary radiation exposure and resource use, but can lead to missed opportunities to provide ear

Root Cause Analysis, Action, and Audit (RCA3): A Novel Approach to Sustainably Reduce Medication Errors.

BACKGROUND: Medical errors threaten patient safety. Although root cause analysis (RCA) is commonly used to investigate medication errors, such errors often recur, posing ongoing risks to patient safety. The RCA2 framework, introduced by the National Patient Safety Foundation, aimed to strengthen traditional RCA by emphasizing action-oriented outcomes. To further enhance sustainability, we implemented RCA3-an iterative process adding follow-up audits to evaluate long-term effectiveness of corrective actions. We aim to highlight RCA3's ability to maintain the effectiveness of interventions in pr

Clinical effectiveness of interventions for treatment-resistant anxiety in older people: a systematic review.

BACKGROUND: Anxiety and related disorders include generalised anxiety disorder, obsessive-compulsive disorder, panic disorder, post-traumatic stress disorder and phobic disorders (intense fear of an object or situation). These disorders share the psychological and physical symptoms of anxiety, but each disorder has its own set of characteristic symptoms. Anxiety disorders can be difficult to recognise, particularly in older people (those aged over 65 years). Older people tend to be more reluctant to discuss mental health issues and there is the perception that older people are generally more w

A cluster-randomized trial of interventions for adolescent mental disorders in Zimbabwe.

PURPOSE: Young people have low uptake of mental health. We compared two task-shifted mental health care models, i.e., adult Friendship Bench (FB) delivered by community health workers and Youth Friendship Bench (YouFB) delivered by trained university students in Harare, Zimbabwe. We hypothesised that the peer-delivered YouFB would have greater uptake and effectiveness in managing common mental disorders (CMDs) in 16-19-year-olds compared to the standard FB model. We also aimed to evaluate the reach, fidelity, acceptability and cost of the YouFB compared to standard FB. METHODS: We conducted a

Community and health system intervention to reduce disrespect and abuse during childbirth in Tanga Region, Tanzania: A comparative before-and-after study.

BACKGROUND: Abusive treatment of women during childbirth has been documented in low-resource countries and is a deterrent to facility utilization for delivery. Evidence for interventions to address women's poor experience is scant. We assessed a participatory community and health system intervention to reduce the prevalence of disrespect and abuse during childbirth in Tanzania. METHODS AND FINDINGS: We used a comparative before-and-after evaluation design to test the combined intervention to reduce disrespect and abuse. Two hospitals in Tanga Region, Tanzania were included in the study, 1 ran

Learning from the community: early medical students' territory-based immersion with community health agents in Brazil.

BACKGROUND: Early immersion of medical students in Primary Health Care (PHC) settings is vital for fostering socially accountable medical education. In Brazil, PHC is territorially organised, and Community Health Agents (CHAs) play a central role in mediating healthcare delivery. However, pedagogical descriptions of supervised, CHA-led student immersion experiences remain limited. This study addresses this gap by presenting an interpretive qualitative analysis of a structured, territory-based immersion designed for first-year medical students. METHODS: Between March and October 2025, seventy-

A Sarcoptes scabiei specific isothermal amplification assay for detection of this important ectoparasite of wombats and other animals.

BACKGROUND: The globally distributed epidermal ectoparasite, Sarcoptes scabiei, is a serious health and welfare burden to at-risk human and animal populations. Rapid and sensitive detection of S. scabiei infestation is critical for intervention strategies. While direct microscopy of skin scrapings is a widely utilised diagnostic method, it has low sensitivity. PCR, alternatively, has been shown to readily detect mite DNA even in microscopy-negative skin scrapings. However, a limitation to the latter method is the requirements for specialised equipment and reagents. Such resources may not be re

Using interpretable decision trees to explore TB-HIV integration under data constraints: a proof-of-concept analysis.

BACKGROUND: Strengthening tuberculosis (TB) and HIV service integration remains a major priority in resource-limited settings, where fragmented care contributes to delayed diagnosis, poor treatment continuity, and suboptimal patient outcomes. Evaluating implementation conditions associated with integrated care remains challenging because health system data are often simplified, inconsistently recorded, and insufficiently structured to capture the complexity of real-world service-delivery processes. This study aimed to demonstrate the application of an interpretable decision-tree framework to e
