Lyon Consensus 2.0 vs Conventional Surgical Criteria for Predicting Favorable Outcomes After Antireflux Surgery.

BACKGROUND: Lyon Consensus 2.0 revised the criteria for conclusive GERD by including grade B esophagitis and requiring abnormal acid exposure on at least 2 monitoring days for physiologic confirmation. Whether these criteria improve selection of patients for antireflux surgery (ARS) remains uncertain. We compared Lyon 2.0 with conventional surgical criteria for their association with favorable postoperative outcomes. STUDY DESIGN: Patients undergoing ARS after preoperative endoscopy and 48-hour wireless pH monitoring were classified according to Lyon 2.0 criteria and conventional surgical cri

LOw-Dose RAbeprazole Therapy for Reducing Gastrointestinal Events in Patients with High Bleeding Risk (LORA-HBR): A Prospective, Multicenter, Interventional Study.

Background: The widespread use of antithrombotic therapies increases bleeding risk, particularly in patients with a high bleeding risk (HBR). Although proton pump inhibitors are recommended for lowering the risk of upper gastrointestinal (UGI) bleeding, the optimal agent and dosage remain uncertain. This study evaluated the efficacy and safety of low-dose rabeprazole (LORA, 5 mg) in reducing the incidence of GI-related adverse events in HBR patients receiving chronic antithrombotic therapy. Methods: This was a prospective, multicenter, interventional study that enrolled 909 South Korean patien

Dispensing of actionable pharmacogenetic drugs among adults receiving cardiovascular disease medications in the Northern Netherlands: A serial cross-sectional study.

The extent of pharmacogenetic (PGx) drug dispensing among Dutch adults receiving medications for cardiovascular disease (CVD) is unknown. Using the University of Groningen IADB.nl pharmacy database, we performed a serial cross-sectional study (2019-2023) to estimate the annual prevalence of PGx drug dispensing and annual rates of initiation. We also identified the most dispensed PGx drug classes, most frequently associated genes, and the proportion of individuals with potential opportunities for reuse of PGx testing results. Adults on CVD medication treatment were defined as those with ≥2 pres

A single small molecule-based human embryo model reveals V-ATPase requirement in mammalian blastocyst cavitation.

Human naïve pluripotent stem cells (nPSCs) can be induced by various combinations of signaling factors to generate blastocyst-like structures, termed blastoids. Despite rapid progress in human blastoid models, their potential to uncover fundamental mechanisms of early human development remains limited, leaving key morphogenetic processes poorly understood. Here, we describe a simple and robust system in which dimethyl sulfoxide (DMSO) alone induces blastoid formation from human nPSCs. This model recapitulates key pre- and post-implantation features and exhibits enhanced polar trophectoderm (TE

Osteoporosis After Menopause and After Drug Therapy: The Molecular Mechanism of Bone Loss and Its Treatment.

Osteoporosis is a prevalent skeletal disorder characterized by reduced bone mass and microarchitectural deterioration, leading to increased fracture risk, particularly in aging populations. Postmenopausal osteoporosis (PMOP) remains the most common primary form and results from abrupt estrogen deficiency after menopause, which disrupts bone remodeling by accelerating the receptor activator of nuclear factor-κB ligand (RANKL)-mediated osteoclastogenesis, suppressing Wnt/β-catenin signaling, and promoting inflammatory cytokine production. In contrast, drug-induced osteoporosis (DIOP) encompasses

Frequency of Helicobacter pylori screening in patients with end-stage liver disease prior to liver transplant.

BACKGROUND: Post-transplant immunosuppression increases malignancy risk. Gastric cancer is one of the most common post-transplant malignancies among high-risk groups including Asian patients. Helicobacter pylori is a known risk factor for gastric cancer; however, pre-transplant screening is not standard. This retrospective study assesses the frequency with which patients with end-stage liver disease, prior to liver transplant, are screened for H. pylori using endoscopic or non-endoscopic methods. METHODS: We conducted a retrospective chart review of 127 patients who underwent upper endoscopy
