Treatment outcomes and contributing factors in pediatric radial neck fracture.

BACKGROUND: Few studies have investigated the contributing factors for pediatric radial neck fractures. This study aimed to analyze the radiographic and clinical factors that influence the treatment outcomes in pediatric radial neck fracture. METHODS: This retrospective study included the 112 pediatric patients with radial neck fracture. On elbow radiographs, angulation, displacement, and carrying angle were measured, and fractures were classified using the modified Judet classification. Clinical outcomes were assessed using the Flynn criteria based on the elbow ROM and carrying angle. Univar

Exploring the Feasibility of Ultrasound-Guided Supraclavicular Nerve Blocks for Distal Forearm Fracture Management in Pediatric Patients.

BACKGROUND: Forearm fractures are a common presentation to the emergency department (ED), frequently requiring reduction and immobilization. In the pediatric population, procedural sedation (PS) is the gold standard to allow for appropriate analgesia, patient compliance, and ultimately successful reduction. When clinical or patient-specific factors limit PS, a possible alternative for analgesia to facilitate reduction of forearm fractures in the ED is ultrasound-guided nerve blocks. This is especially significant for the pediatric population, given that nonsurgical treatment is frequently the

"After Surgery, I Filed for Bankruptcy."The Financial Ramifications of Unexpected Surgery for Upper Extremity Trauma.

BACKGROUND: Traumatic injuries of the upper extremity and hand (UE) can result in notable financial ramifications particularly if they involve surgical intervention. Although most studies in orthopaedics address direct costs, little is known about the personal experience with financial distress following UE trauma surgery. METHODS: Fourteen patients, identified as having markedly high financial distress using self-reported validated surveys, who received UE trauma surgery were interviewed. Semistructured interviews were conducted until thematic saturation was achieved after sequential themati

Early range of motion results in good elbow function following conservative treatment of non-displaced radial head fractures.

INTRODUCTION: The aim of this study was to evaluate the range of motion (ROM), elbow function and predictors for good elbow function after conservative treatment of non-displaced radial head fractures. MATERIAL AND METHODS: All patients with non-displaced radial head fractures (displacement < 2 mm), that were diagnosed between January 1st 2017 and December 31st 2021 in a level I trauma center, were included in this retrospective case series and the charts were evaluated for ROM and elbow function. Elbow function was categorized as "good" or "bad" depending on the ROM measured defined by Morre

Creation of a replicable anatomic model of terrible triad of the elbow.

BACKGROUND: Terrible triad of the elbow (TTE) is a complex dislocation associating radial head (RH) and coronoid process (CP) fractures. There is at present no reproducible anatomic model for TTE, and pathophysiology is unclear. The main aim of the present study was to create and validate an anatomic model of TTE. Secondary objectives were to assess breaking forces and relative forearm rotation with respect to the humerus before dislocation. METHODS: An experimental comparative study was conducted on 5 fresh human specimens aged 87.4 ± 8.6 years, testing 10 upper limbs. After dissection conse

Fracture of the Anteromedial Facet of the Coronoid is More Common Than Previously Thought in Combined Fractures of the Coronoid and Radial Head.

BACKGROUND: The terrible triad injury involves an ulnohumeral dislocation, radial head fracture, and coronoid process fracture. According to traditional teaching, these injuries are strongly associated with anterolateral coronoid tip fractures and can be addressed via a lateral approach to the elbow. However, recent small clinical series suggest that some terrible triad injuries have larger coronoid fractures involving the anteromedial facet. It is important to understand how often these larger coronoid fractures occur because anteromedial facet fractures may need a different approach and diff

Uncementing the status quo: systematic review of a loose-fit, polished stem radial head prosthesis shows stable clinical results in complex elbow injuries with a concomitant radial head fracture.

INTRODUCTION: Selecting the optimal radial head prosthesis to treat radial head fractures, especially in the context of complex elbow injuries like terrible triad, Monteggia, and Essex Lopresti, can be challenging, as there is currently no consensus in the field that favors a particular design. This study investigated the safety and performance of a Polished Stem Radial Head Prosthesis (PS RHP) compared to other modern RHP designs. MATERIALS AND METHODS: A systematic review was conducted according to PRISMA guidelines to capture data on a Polished Stem Radial Head Prosthesis (PS RHP) and othe

Acute atraumatic carpal tunnel syndrome due to flexor tendon rupture following palmar plate osteosynthesis in a patient taking rivaroxaban.

INTRODUCTION: Few case reports describe the development of a hematoma under oral anticoagulation as the cause of an atraumatic carpal tunnel syndrome. CASE REPORT: A 76 years old woman presented an acute atraumatic carpal tunnel syndrome of her left hand under oral anticoagulation with rivaroxaban due to atrial fibrillation. 12 years ago, palmar plate osteosynthesis of a distal radius fracture had been performed on the affected wrist. Open decompression of the carpal canal was performed due to persistent severe pain under intense pain therapy and progressive neurological symptoms. The cause o

Natural history and factors associated with ulnar-sided wrist pain in distal radial fractures treated by plate fixation.

UNLABELLED: We documented longitudinal changes in the incidence of ulnar-sided wrist pain after distal radial fractures treated by plate fixation and identified factors associated with ulnar-sided wrist pain. A total of 140 patients were enrolled in this study. Radiographs were taken 3 months after operation, and were used to measure radial inclination, anterior angulation and ulnar variance, and to identify the presence of an ulnar styloid fracture. Clinical assessments at the same time included grip strengths, ranges of wrist motion and the patient-rated wrist evaluation questionnaire. The p

Radial head fractures: indications and outcomes for radial head arthroplasty.

Radial head fractures without associated bony or ligamentous injury can be safely treated with internal fixation, if possible, or arthroplasty if nonreconstructable. However, nonreconstructable radial head fractures in association with elbow dislocation and/or ligamentous injury in the elbow or forearm represent a specific subset of injuries that requires restoration of the radiocapitellar articulation for optimal function. The purpose of this article was to summarize the indications for radial head arthroplasty and discuss the reported outcomes.

An evaluation of operative management of displaced volar Barton's fractures using volar locking plate.

Volar Barton's fractures are common but more convincing treatment methods are still controversial. Malunion can cause serious disability. Twenty-six consecutive patients were treated with open reduction Internal fixation using volar locking plate. Records of 21 cases were available for follow-up for a mean period of 18 months (range 12-34 months). All 21 fractures healed within 8 to 14 weeks (mean 9.3 weeks). The wrist function was satisfactory in 90.4% cases (19 out of 21 patients). This surgical technique can provide high success rate. Locking plate has better efficacy in terms of immediate

Operative management of distal radial fractures with 2.4-millimeter locking plates. A multicenter prospective case series.

BACKGROUND: In the past decade, there has been a trend toward open reduction and internal fixation of unstable distal radial fractures. There are now more than thirty different implant designs specific for the fixation of distal radial fractures. A multicenter prospective study of a case series was conducted to determine the efficacy of the operative management of distal radial fractures stabilized with 2.4-mm locking plates. METHODS: One hundred and fifty patients were entered into the study over a two-year period. The mean age of these patients was fifty-one years, and the cohort included e

Treatment of external fixation pins about the wrist: a prospective, randomized trial.

BACKGROUND: Pin-track infection remains one of the most troublesome complications of external fixation, in some cases compromising otherwise successful fracture treatment. METHODS: One hundred and eighteen patients (120 wrists) who had been managed with the placement of an external fixation device for the treatment of a displaced, unstable, distal radial fracture were randomized into one of three treatment groups: (1) weekly dry dressing changes without pin-site care; (2) daily pin-site care with a solution of one-half normal saline solution and one-half hydrogen peroxide; and (3) treatment w

Chronic radial head dislocation in children, Part 2: results of open treatment and factors affecting final outcome.

Fifteen elbows (3 congenital, 12 traumatic) in 14 children (mean age 9.5 years) with chronic radial head dislocation for at least 3 months (range 3-180 months) underwent radial head-sparing reconstructive surgery and were followed up at an average of 43.5 months postoperatively (range 12-105 months). At follow-up, the patients were evaluated radiographically and clinically using range-of-motion measurements and an elbow performance score based on four parameters (deformity, pain, motion, function). Ten cases had excellent results, two had good results, two had fair results, and one case had po
