Dynamic coupling between fMRI local connectivity and interictal EEG in focal epilepsy: A wavelet analysis approach.

Simultaneous scalp EEG-fMRI recording is a noninvasive neuroimaging technique for combining electrophysiological and hemodynamic aspects of brain function. Despite the time-varying nature of both measurements, their relationship is usually considered as time-invariant. The aim of this study was to detect direct associations between scalp-recorded EEG and regional changes of hemodynamic brain connectivity in focal epilepsy through a time-frequency paradigm. To do so, we developed a voxel-wise framework that analyses wavelet coherence between dynamic regional phase synchrony (DRePS, calculated f…

Demographic Characteristics and Association of Serum Vitamin B12, Ferritin and Thyroid Function with Premature Canities in Indian Patients from an Urban Skin Clinic of North India: A Retrospective Analysis of 71 Cases.

BACKGROUND: The incidence of self-reported premature hair graying (PHG) seems to be on the rise. PHG has a profound impact on the patient's … vided details of the onset, duration and pattern of involvement, history, and examination findings (scalp, cutis, and general physical). Since all these patients had been screened for anemia, thyroid disorder, fasting blood glucose, and Vitamin B12 levels at the time of presentation, these parameters were also available for analysis. RESULTS: The mean …

Automatic brain extraction methods for T1 magnetic resonance images using region labeling and morphological operations.

In this work we propose two brain extraction methods (BEM) that solely depend on the brain anatomy and its intensity characteristics. Our methods are simple, unsupervised and knowledge based. Using an adaptive intensity thresholding method on the magnetic resonance images of head scans, a binary image is obtained. The binary image is labeled using the anatomical facts that the scalp is the boundary between head and background, and the skull is the boundary separating brain and scalp. A run length scheme is applied on the labeled image to get a rough brain mask. Morphological operations are the…

Tumour and tumour-like conditions of peripheral nerve origin: ten years' experience.

BACKGROUND: There are four major lesions which may arise in the peripheral nerve, namely neuroma, schwannoma, neurofibroma and malignant per … ls with hyperchromatic nuclei. The commonest site involved for both schwannoma and neurofibroma was scalp-face-neck followed by back. The age range for schwannoma was 16 to 75 years whereas the same for the neurofibroma cases was 2 to 82 years. MPNST cases were seen in the age range of 40 to 72 with 3 cases in upper extremity, 3 in lower extremity and 1 …

Brain-sparing holo-cranial radiotherapy: a unique application of helical tomotherapy.

AIMS: Diffuse and extensive involvement of the scalp/skull by malignancy mandates holo-cranial radiotherapy with the aim to deliver homogeneous doses to the planning target volume (PTV) while minimising the dose to surrounding organs at risk (OARs). Previously described techniques result in significant heterogeneity, suboptimal coverage or poor conformity and need complicated beam matching. Here we report our preliminary experience of planning and delivery of brain-sparing holo-cranial radiotherapy with helical tomotherapy. MATERIALS AND METHODS: Three patients with extensive involvement of t…

Serum concentrations of interferon-gamma (IFN-g) in patients with alopecia areata: correlation with clinical type and duration of the disease.

Alopecia areata (AA) is a heterogeneous disease characterized by nonscarring hair loss on the scalp or other parts of the body. A wide range of clinical presentations can occur-from a single patch of hair loss (alopecia unilocularis, AUl), multiple patches (alopecia multilocularis, AM) to complete loss of hair on the scalp (alopecia totalis, AT) or the entire body (alopecia universalis, AU). The cause ofAA is unknown although most evidence supports the hypothesis that AA is a T-cell mediated autoimmune disease of the hair follicle and that cytokines play an important role. The aim of the study…

Development and pilot-testing of a psoriasis screening tool.

BACKGROUND: There is a need to validate psoriasis self-reports in epidemiological studies, where individuals may not be seeing dermatologists or other health care providers. OBJECTIVES: To develop and pilot test the Psoriasis Screening Tool (PST) in an ambulatory setting. PATIENTS AND METHODS: The PST was designed with eight closed-ended questions requiring a 'yes' or 'no' response. Typical images of skin, nail and scalp changes in psoriasis were included with respective questions. We administered the PST to 222 consecutive individuals being seen at a dermatology clinic. All English-speaking…

Multiple huge epidermal inclusion cysts mistaken as neurofibromatosis.

Epidermal inclusion cyst is one of the common benign soft tissue tumors, and it can be easily confirmed and treated by surgical excision. We experienced a patient who had multiple masses on the face and scalp region, and the masses had been misdiagnosed as neurofibromatosis because of accompanying mental retardation. We would like to introduce a case of clinical diagnosis error caused by the lack of radiologic evaluation and pathologic confirmation. A 27-year-old male patient visited with multiple masses, with a length of approximately 1 to 10 cm on the face and scalp region. These mass have d…

The Jarvik 2000 Oxford system: increasing the scope of mechanical circulatory support.

METHODS: We developed a system for mechanical circulatory support based on the Jarvik 2000 intraventricular axial flow impeller pump (Jarvik Research, Inc., New York, N.Y.) and percutaneous electric power. The adult pump provides flow at a rate up to 10 L/min with an energy requirement of 7 to 10 watts. The device was implanted into the apex of the left ventricle through a left thoracotomy without cardiopulmonary bypass. A Dacron graft conveyed blood to the descending thoracic aorta. In patients, we will use a skull-mounted carbon pedestal to transmit fine electric wires through the scalp skin…

[Embolization and detached balloon occlusion by the femoral route in craniofacial lesions].

Embolization and detached balloon occlusion by the femoral route were performed in 45 cases: 19 cases of meningiomas, 5 of scalp arteriovenous malformations (AVMs), 4 of dural AVMs, 6 of cerebral AVMs, 8 of facial angiomas and 3 of carotid cavernous sinus fistulas (CCFs); and favorable results were attained. In meningiomas, there is only a short interval between the embolization and the removal of tumor (we usually perform the embolization a couple of days before the removal), and we use Gelfoam as embolus material. We aim at central tumor embolization with small emboli. The embolization reduc…

Successful Use of Adalimumab for Dissecting Cellulitis in a Pediatric Patient With Keratitis-Ichthyosis-Deafness (KID) Syndrome.

Dissecting cellulitis of the scalp (DCS) is a chronic inflammatory condition characterized by painful, draining nodules and progressive, scarring alopecia. It is often refractory to treatment, and there are currently no FDA-approved therapies. We present a case of a 15-year-old boy with keratitis-ichthyosis-deafness (KID) syndrome and DCS who experienced significant clinical response to treatment with adalimumab after failing multiple courses of oral antimicrobials, surgical interventions, intralesional corticosteroids, and topical therapies. Treatment with adalimumab led to reduction in drain…

Spontaneous subgaleal haematoma with severe headache in two children during acute painful sickle cell crisis.

Sickle cell disease is an inherited disorder of haemoglobin formation, predominantly affecting individuals of African, Indian, or Mediterranean descent. Acute painful episodes frequently occur in these patients due to the blockage of small blood vessels by sickled cells, leading to tissue infarction. Two children of Nigerian descent, presented with severe headaches during a painful crisis episode with associated swelling on their scalps. CT imaging in both cases showed subgaleal haematoma which was managed conservatively in addition to managing the painful crisis with a good outcome. This repo…

Congenital Triangular Alopecia: A Case of Effective Response with 5% Topical Minoxidil in a Male Adolescent.

Congenital triangular alopecia (CTA) is a rare entity of non-cicatricial alopecia characterized by triangular or oval-shaped alopecia patches on the frontotemporal region of the scalp. Few therapeutic options exist, and there is currently no effective treatment except for hair transplantation. We report the case of an adolescent boy with CTA who was treated with 5% topical minoxidil solution. During therapy, the patient showed improvement in the form of the appearance of terminal hairs, which started to show after two months of treatment. After eight months of treatment, the affected area was …

A Unique Role of the Oncoplastic Breast Surgeon's Approach to an Inframammary Proliferating Trichilemmal Cyst: A Case Report.

Trichilemmal cysts, also known as pilar cysts, are commonly found on hair-bearing surfaces, such as the scalp or hairline. These are cysts that form from hair follicles and are benign. We describe an unusual case of a middle-aged lady presenting with a longstanding left-sided inframammary mass that had started growing more in the last year prior to the presentation. She was referred to the breast surgery clinic for further evaluation. A magnetic resonance imaging (MRI) scan was done for further delineation, which was not distinguished, and histopathological correlation was suggested. A decisio…
