Sleep-disordered breathing and atrial fibrillation: review of the evidence.

Sleep-disordered breathing and atrial fibrillation are two common medical conditions. A strong association between them has been described and has prompted significant research in understanding the mechanism connecting the two conditions while explaining the synergic negative effects in terms of morbidity and mortality in affected patients. This review focuses first on the pathophysiologic mechanisms favoring the development of atrial fibrillation in patients with sleep-disordered breathing, considering specifically and separately obstructive sleep apnea and central sleep apnea. Then, the effe…

Developmental changes in sleep and breathing across infancy and childhood.

Sleep and breathing are physiological processes that begin in utero and undergo progressive change. While the major period of change for both sleep and breathing occurs during the months after birth, considered a period of vulnerability, more subtle changes continue to occur throughout childhood. The systems that control sleep and breathing develop separately, but sleep represents an activity state during which breathing and breathing control is significantly altered. Infants and young children may spend up to 12 hours a day sleeping; therefore, the effects of sleep on breathing are fundamenta…

The efficacy of a chinstrap in treating sleep disordered breathing and snoring.

STUDY OBJECTIVES: A previously published case report suggested that a chinstrap alone might improve obstructive sleep apnea (OSA). We conducted this study to determine whether a chinstrap was a feasible alternative to continuous positive airway pressure (CPAP) in patients with OSA. METHODS: 26 adult patients with OSA (apnea-hypopnea index [AHI] > 5/h on diagnostic polysomnogram [PSG]) underwent a modified split-night PSG, using only a chinstrap for the first 2 hours of sleep, followed by CPAP titration for the remainder of the night. Improvements in AHI, arterial oxygen saturation (SpO2), and…

Tonsillar hypertrophy implications in sleep disorders in adults and children.

INTRODUCTION: The obstructive sleep apnea syndrome is the most common sleep disorder. It covers a multitude of symptoms associated with apnea. Next to polysomnography, otorhinolaryngological clinical examination plays an important role in establishing the diagnosis, in evaluating any oropharyngeal and hypopharyngeal modification. PATIENTS AND METHODS: In this research, we intend to demonstrate the histopathological examination importance in establishing the tonsillar hypertrophy degree. This is made in direct correlation with its volume, and the sleep apnea degree. In this context, we have co…

SCOPRISM: a new algorithm for automatic sleep scoring in mice.

BACKGROUND: Scoring of wake-sleep states by trained investigators is a time-consuming task in many sleep experiments. We aimed to validate SCOPRISM, a new open-source algorithm for sleep scoring based on automatic graphical clustering of epoch distribution. METHODS: We recorded sleep and blood pressure signals of 36 orexin-deficient, 7 leptin knock-out, and 43 wild-type control mice in the PRISM laboratory. Additional groups of mice (n=14) and rats (n=6) recorded in independent labs were used to validate the algorithm across laboratories. RESULTS: The overall accuracy, specificity and sensit…

Reflection of tolerance to alcohol in the structure of the sleep wakefulness cycle.

The purpose of the present work was to study the effects of tolerance to ethanol on the sleep-wakefulness cycle (SWC) structure. Experiments were carried out in chronic conditions on the adult cats (n=5). The following methods were used: the stereotaxic - for implanting electrodes; polysomnographic - for EEG registration of the SWC structure. Alcoholisation (0.2-2.5 g/kg 25% ethanol solution) was conducted by intraperitoneal injections that lasted for two weeks. The obtained results were processed statistically and significance of the changes was determined by the Student t-test. Low single do…

Aripiprazole is effective for treatment of delayed sleep phase syndrome.

Delayed sleep phase syndrome (DSPS) comprises a persistent or recurrent pattern of sleep disturbances, sleep disruption that leads to insomnia and/or excessive daytime sleepiness, and impaired functioning in social, occupational, or other spheres. Aripiprazole (APZ), a second-generation antipsychotic, manifests a novel mechanism of action by serving as a partial agonist of both D2 and serotonergic 5-HT1A receptors and antagonist of 5-HT2A receptors. We have used APZ to treat DSPS. One reason it was effective may be that the insomnia induced by daytime APZ was effective in treating the patient'…

Inducing task-relevant responses to speech in the sleeping brain.

Falling asleep leads to a loss of sensory awareness and to the inability to interact with the environment [1]. While this was traditionally thought as a consequence of the brain shutting down to external inputs, it is now acknowledged that incoming stimuli can still be processed, at least to some extent, during sleep [2]. For instance, sleeping participants can create novel sensory associations between tones and odors [3] or reactivate existing semantic associations, as evidenced by event-related potentials [4-7]. Yet, the extent to which the brain continues to process external stimuli remains…

Daytime sleep in Parkinson's disease measured by episodes of immobility.

Excessive daytime sleepiness (EDS) is common in Parkinson's Disease (PD). Actigraphy uses periods of immobility as surrogate markers of nighttime sleep but there are no examples of its use in assessing EDS of PD. A commercial wrist worn system for measuring bradykinesia and dyskinesia also detects 2 min periods of immobility, which have a 85.2% concordance with the detection of sleep by ambulatory daytime polysomnography, (p < 0.0001 Chi Squared). High Epworth Sleepiness Scores (ESS) were associated with a proportion of time immobile (PTI) (p = 0.01 Mann-Whitney U). The median PTI between 0900…

The effect of obstructive sleep apnea on QRS complex morphology.

BACKGROUND: Obstructive sleep apnea (OSA) has been reported to be associated with an increased risk of ventricular arrhythmias and conduction disturbances. The aim of this study was to analyze the QRS complex morphology potentially indicative of intraventricular conduction impairment in patients with mild to severe OSA. MATERIAL AND METHODS: One hundred ninety-three consecutive patients, who underwent complete overnight polysomnography, were divided into four groups based on the OSA severity: (1) no OSA, (2) mild OSA, (3) moderate OSA and (4) severe OSA (apnea-hypopnea index <5, 5-15, 15-30, …

The upper airway in sleep-disordered breathing: UA in SDB.

Sleep disordered breathing (SDB) is a common condition and could be a risk factor for cardiovascular morbidity and mortality. However, the pathogenesis of SDB remains to be elucidated. In general, SDB is divided into two forms, obstructive and central sleep apnea (OSA and CSA, respectively). OSA results from the sleep-related collapse of the upper airway (UA) in association with multiple factors like race, gender, obesity and UA dimensions. CSA primarily results from a fall in PaCO2 to a level below the apnea threshold during sleep through the reflex inhibition of central respiratory drive. It…

Neurobehavioral and autonomic alterations in adults with obstructive sleep apnea.

OBJECTIVE: Obstructive sleep apnea (OSA) is associated with sympathetic hyperactivity, excessive nocturnal sweating, sleepiness, and neurobehavioral cognitive alterations. However, it is not well known if cognitive consequences of OSA are independent from autonomic alterations. Thus, we assessed the association between polysomnographic, autonomic, and cognitive tests performance in OSA patients. METHODS: Fifty eight OSA patients (53 male) were administered with questionnaires assessing demographic, Epworth, Beck Depression Inventory, Syndrom Kurz test (SKT), Trail Making part B (TMT-B), and F…
