We evaluated the age-related stroke risk factors in 164 Italian male patients with a diagnosis of first-ever ischemic stroke. Nocardial cerebral abscess, an uncommon condition, is usually secondary to a septic focus elsewhere in the body. We report a 44-year-old Chinese male patient who had primary nocardial cerebral abscesses due to Nocardia asteroides which presented with stroke-like episodes. He improved spontaneously between these episodes. The diagnosis was reached only after a contrast CT brain study and microbiological examination of specimens obtained during craniotomy. An early contrast CT study is important in avoiding any delay in diagnosis and treatment of this serious condition.

Atrial fibrillation (AF) and other cardiac arrhythmias may occur after stroke in absence of cardiac disease. We report a case of pontine ischemic stroke with paroxysmal atrial fibrillation of spontaneous resolution. Cardiac investigations were normal. We propose that the AF was the consequence of the stroke.

AIMS: To assess the safety and efficacy of the intra-arterial administration of streptokinase within 24 hours of acute ischaemic stroke.
METHODS: Patients who presented to the Austin Hospital casualty department between 3 and 22 hours after an acute stroke were considered for the study. Eligible patients had pretreatment non-contrast computed tomographic scans of the brain to exclude haemorrhage. Aphasia is common in elderly patients with stroke. This paper presents some results of aphasia therapy within the activation phase (acute aphasia) and the syndrome-specific learning phase (until the 24th week of treatment). A phase-specific therapeutic concept was developed to connect stimulation and cirumvention method and compensatory techniques. The results attest to the success early after stroke regarding development of speech pathology logopedic diagnosis and therapy.

Serial electrocardiograms as well as echocardiographic studies of 51 pilgrims suffering from acute heat stroke (mean rectal temperature 41.6 degrees C) were performed. A single unilateral lesion, near the midline, may interrupt the pathways involved in vertical gaze just before and after they decussate, inducing an anatomically unilateral, but functionally bilateral, lesion. We report 11 patients with supranuclear vertical gaze palsies (two with conjugate upgaze palsy, four with combined up- and downgaze palsies, two with monocular elevation palsy, and three with vertical "one-and-a-half" syndrome) due to unilateral midbrain strokes.

The risk of embolic stroke during sick sinus syndrome before cardiac pacemaker insertion is substantial, but stroke after pacemaker insertion has not been well studied. We observed 10 sick sinus syndrome patients who developed an ischemic stroke 4 days to 112 months after pacemaker insertion. Although evidence indicates that the incidence of stroke has declined over the past 30 years, stroke remains one of the leading causes of disability and death in the United States. A comparative study of two patients, one affected by haemorrhagic shock and encephalopathy (HSE) and the other by heatstroke is reported. Both presented shock, disseminated intravascular coagulation, neurological damage and hepatopathy.
