[Serodiagnosis of tuberculosis by detection of antituberculous glycolipid antigen (TBGL antigen) antibodies in serum using enzyme-linked immunosorbent assay: clinical evaluation of anti-TBGL antibodies assay kit].

Kyowa Medex Co., Ltd. developed the kit for the sero-diagnosis of tuberculosis, which detects IgG antibodies against tuberculous glycolipids antigen containing cord factor (TBGL antigen) prepared from M. tuberculosis using the enzyme-linked immunosorbent assay technique. We evaluated the kit using clinical specimens and the results are as follows: 1) In total, 34 out of 39 cases (87.2%) with active pulmonary tuberculosis showed positive anti-TBGL antibody. 2) Patients with cavity, patients with extensive lesions and patients excreting large amount of acid fast bacilli tended to show high posit

[Preventable tuberculosis cases in Japan--a new approach to the assessment of tuberculosis control problems].

I defined cases of tuberculosis which could not be prevented from infection or development of disease among infected, or could not be detected in the early stages as "preventable cases" in order to evaluate tuberculosis control efforts in the community, Japan. Among 241 bacteriologically confirmed cases with pulmonary tuberculosis newly registered from 1988 through 1989 in Yamagata Prefecture, 80 (33%) were defined as preventable cases by observing their course and the process of diagnosis. That is to say, one-third of bacillary cases could have been prevented in Yamagata where the incidence o

Prevention of tuberculosis in household members: estimates of children eligible for treatment.

OBJECTIVE: To estimate of the number of children younger than 5 years who were household contacts of people with tuberculosis and were eligible for tuberculosis preventive treatment in 2017. METHODS: To estimate the number of eligible children, we obtained national values for the number of notified cases of bacteriologically confirmed pulmonary tuberculosis in 2017, the proportion of the population younger than 5 years in 2017 and average household size from published sources. We obtained global values for the number of active tuberculosis cases per household with an index case and for the pr

Transmission of Mycobacterium tuberculosis in four prisons in Colombia.

Our study aimed to describe the transmission dynamics and genotypic diversity of Mycobacterium tuberculosis in people deprived of liberty (PDL) in four Colombian prisons. Our cohort study included 64 PDL with bacteriologically confirmed pulmonary tuberculosis diagnosed in four Colombian prisons. The 132 isolates were genotyped using 24-mycobacterial interspersed repeated units-variable number tandem repeats (MIRUs-VNTR). A cluster was defined when ≥2 isolates from different PDL had the same genotype. Tuberculosis acquired in prison was considered when ≥2 persons were within the same cluster an

Primary nasopharyngeal tuberculosis: a case report.

BACKGROUND: The occurrence of nasopharyngeal tuberculosis is rare even in areas where tuberculosis is endemic. Here, we report a case of rare primary nasopharyngeal tuberculosis, promptly evaluated by nasolaryngoscopy. CASE PRESENTATION: A 78-year-old woman presented with postnasal drip and a cough of 1-month duration. Endoscopic examination of the nasopharynx revealed irregular mucosal thickening of the right lateral and posterior wall of the naso (epi)-pharynx, which was covered with yellow discharge presenting as postnasal drip. Computed tomography (CT) demonstrated enhanced soft tissue ar

Engagement of Toll-like receptor 2 on CD4(+) T cells facilitates local immune responses in patients with tuberculous pleurisy.

BACKGROUND: Although it has been recognized that Mycobacterium tuberculosis contains large amounts of Toll-like receptor 2 (TLR2) ligands, their direct effects on CD4(+) T cells and the clinical implications have not been determined. METHODS: With the recent finding that activated CD4(+) T cells express TLR2 as a costimulatory receptor, we hypothesized that M. tuberculosis and its components may directly affect CD4(+) T cells by engaging TLR2, thus facilitating the expansion and function of these lymphocytes in tuberculous pleura. RESULTS: Our results indicate that CD4(+) T cells from the pl

Miliary lung disease revisited.

This article reviews the high-resolution computed tomography imaging features of miliary pattern, a characteristic radiologic manifestation of diffuse micronodular lung disease. The most common entities with this pattern are miliary tuberculosis, pneumoconiosis, sarcoidosis, metastases, and hypersensitivity pneumonia. According to the distribution of the nodules in relation to the secondary lobule, high-resolution computed tomography findings divide miliary patterns into 3 groups: centrilobular, perilymphatic, and random presentation. The radiologic features that help in the differential diagn

Comparison of 6 months isoniazid versus 3 months isoniazid - rifampicin therapy for prevention of tuberculosis in children suffering from tuberculosis infection - A randomized controlled trial.

BACKGROUND: Tubercular infection is highly prevalent in our country and conversion to tuberculosis disease is a major concern. Under the National Strategic Plan, India is heading towards ending tuberculosis by 2025. The benefits of tuberculosis preventive therapy are being extended to the children aged ≥5 years, adolescents and adults who are household contacts of people with bacteriologically confirmed pulmonary tuberculosis who are found not to have active tuberculosis. Many types of tuberculosis preventive therapy are studied, the choice depends on availability and tolerability profile. The

PURPOSE: QuantiFERON® TB-Gold In-Tube (3G) testing was performed on tuberculosis-positive index cases and their contacts. The purpose of this study was to evaluate the relationship between 3G test results and the subsequent development of tuberculosis, and to identify effective strategies to prevent the onset of tuberculosis. METHODS: Index cases and their contacts were subjected to 3G testing in a contact investigation in Osaka City in 2011-2012. For index cases, sputum smears were tested, and the infecting organism was identified. For the contacts, the following information was collected: a

A rare case of constrictive pericarditis with Budd-Chiari syndrome due to right atrial thrombosis.

Patients living with HIV (PLWH) with previous pulmonary tuberculosis, presenting with disproportionate ascites to peripheral congestion, should alert the clinician to consider constrictive pericarditis and Budd-Chiari syndrome (BCS). Constrictive pericarditis is the scarring and loss of the pericardial sac elasticity. The aetiology of constrictive pericarditis varies between developed and developing countries, with infective causes like tuberculosis being significant in South Africa. Budd-Chiari syndrome is a group of disorders characterised by hepatic venous outflow obstruction. The level of

[Surgical Removal of Left Atrial Thrombus Originating from the Left Upper Pulmonary Vein Stump after Left Upper Lobectomy;Report of a Case].

Left upper lobectomy may be a risk factor for thrombosis in the pulmonary vein stump. We report a case of surgical removal of a left upper pulmonary vein stump thrombus after left upper lobectomy. A 73-year-old man with transit ischemic attack had a history of left upper lobectomy for pulmonary tuberculosis 40 years before. Echocardiography and chest computed tomography (CT) scan revealed a mobile thrombus in the left atrium originating from the left upper pulmonary vein stump. Surgical removal of the thrombus as well as the left upper pulmonary vein stump was performed under extracorporeal ci

Prospects of Inhalable Formulations of Conventionally Administered Repurposed Drugs for Adjunctive Treatment of Drug-Resistant Tuberculosis: Supporting Evidence from Clinical Trials and Cohort Studies.

Background: Drug resistant tuberculosis is a major public health concern, since the causative agent Mycobacterium tuberculosis is resistant to the most effective drugs against tuberculosis treatment ie., rifampicin and isoniazid. Globally, it accounts 4.6 percent of the patients with tuberculosis, but in some low socioeconomic areas this proportion exceeds to 25 percent. The treatment of drug resistant tuberculosis is prolonged (9-12 months) and often have less favorable outcome with novel as well as recently repurposed drugs administered by conventional routes.
