The haemodynamic effects of a simultaneous infusion of salbutamol and nitroprusside were measured in 20 patients with acute myocardial infarction and severe left ventricular failure. Six patients also had clinical manifestations of cardiogenic shock. Ten patients received salbutamol first with the subsequent addition of nitroprusside; in the other 10 patients nitroprusside was infused first. Salbutamol was infused at a constant rate of 20 micrograms/min in all patients, while the dose of nitroprusside, which averaged 51.25 micrograms/min, was adjusted to reduce left ventricular filling pressur

The present study was designed to explore whether the depletion of serotonin (5-HT) in the central nervous system (CNS5-HT) leads to systolic and diastolic dysfunction and whether this dysfunction is exacerbated by sex, age and spinal cord injury. Echocardiographic assessment of systolic and diastolic function was completed in young and old male and female tryptophan hydroxylase 2 knockout (TPH2-/-) and wild-type (TPH2+/+) mice with intact spinal cords, as well as in C2 spinal cord hemisected young TPH2-/- and TPH2+/+ mice. In addition, lumbar sympathetic nervous system activity was recorded

BACKGROUND: Regional strain by speckle-tracking echocardiography can be used to detect occult right ventricular (RV) myocardial dysfunction. However, in patients with coronary artery disease, the impact of RV strain is unknown. The aim of this study was to validate the prognostic value of RV strain in patients with non-acute coronary syndrome angina.
METHODS: In total, 208 patients with coronary artery disease proved by coronary angiography were retrospectively identified (mean age, 63.81 ± 10.12 years; 161 men). In addition to clinical and traditional echocardiographic parameters, RV free wa

ALCAPA (anomalous left coronary artery from pulmonary artery) syndrome is a rare congenital abnormality that involves an anomalous insertion of the left coronary artery into the pulmonary artery. Ninety percent of patients present in the first year of life with signs and symptoms of heart failure or sudden cardiac death secondary to chronic myocardial ischemia. There have been an increasing number of reports of ALCAPA patients surviving to adulthood. There seems, however, to be a tendency to die suddenly in the third decade of life. Adult survivors are either asymptomatic or present with mitra

Cardiac fibroma is a rare, benign tumor that occurs chiefly in children and rarely in adults. Most fibromas occur in the ventricles and may reach a very large size that complicates surgical removal. Herein, we report the case of a 38-year-old woman who presented with shortness of breath, fatigue, and lightheadedness and was found to have a 6 × 8-cm fibroma of the left ventricle. Surgical resection was successful, but 7 days later she developed sudden-onset severe mitral regurgitation due to partial disruption of the posterolateral papillary muscle. Mitral valve replacement with a 27-mm mechani

Left bundle brunch block (LBBB) is met in approximately 20% of hospitalized patients and in 1/3 of outpatients with chronic heart failure (CHF). Abnormalites of impulse conduction in LBBB promote dyssynchronization, which represent a pathophysiological process which not only worsens ventricular function but also facilitates remodeling of the myocardium and development of CHF. As a result of dyssynchrony systolic and diastolic functions of the heart are impaired. This manifests as shortening of diastole of the left ventricle and dyssynchronous movement of interventricular septum (IVS) with a te

Epidemiologic studies have shown an important increase in the high mortality of patients with congestive heart failure (CHF) despite optimal medical management. Ventricular arrhythmia was recognized as the most common cause of death in this population. Electrolyte imbalance, myocardial fibrosis, left ventricular dysfunction, and inappropriate neurohumoral activation are presumed responsible for sudden cardiac death. In this study, we focused on the deleterious effects of the overproduction of aldosterone that occurs in patients with CHF. Secondary hyperaldersteronism can be part of several fac

Advanced heart failure due to dilated cardiomyopathy (DCM) has limited treatment options. Although intracoronary autologous cell therapy may improve cardiac function, its use in patients with severe left ventricular dysfunction is limited by procedural risk. This exploratory study investigated whether temporary mechanical circulatory support with an Impella device could enable safer therapy delivery. Twenty patients with DCM and a left ventricular ejection fraction (LVEF) of ≤35% received granulocyte colony-stimulating factor pretreatment followed by Impella-supported intracoronary infusion of

Cancer prognosis has steadily improved over recent decades. In 2025, approximately 85% of patients with cancer were in remission. The growing complexity of oncologic treatments has led to substantial gains in progression-free survival, albeit at the cost of increased cardiotoxic risk. Cardiotoxicity most commonly manifests as heart failure or left ventricular dysfunction. Consequently, the cardiovascular mortality of individuals treated for childhood cancers, as well as of older women treated for breast cancer, now equals or even exceeds oncologic mortality. In response, the European Society of

A healthy man in his 40s presented with acute anterolateral ST-segment elevation myocardial infarction after sudden-onset chest pain during a coughing episode. Coronary angiography and intravascular ultrasound revealed extensive spontaneous coronary artery dissection (SCAD) of the left anterior descending artery with Thrombolysis in Myocardial Infarction II distal flow. He was managed conservatively, but his course was complicated by severe left ventricular systolic dysfunction, apical thrombus and a ventricular tachycardia storm leading to cardiogenic shock. He required repeated emergency dir

BACKGROUND: The timing of permanent pacemaker implantation in myocarditis-related complete heart block remains challenging because no established criteria exist for reversibility of the conduction system.
CASE SUMMARY: A previously healthy 50-year-old Asian woman presented with lymphocytic myocarditis complicated by cardiogenic shock and complete atrioventricular block (CAVB). Despite successful hemodynamic stabilization with Impella CP support and complete recovery from left ventricular dysfunction, CAVB persisted. A permanent pacemaker was implanted on day 28. Afterward, cardiac enzymes and

BACKGROUND: Wolff-Parkinson-White (WPW) syndrome is associated with an abnormal accessory conduction pathway leading to ventricular pre-excitation and potential re-entrant tachyarrhythmias.
CASE SUMMARY: A 13-year-old female presents with shortness of breath, cough, and wheezing for 1 day. She was tachycardic with a left bundle branch block. Echocardiographic findings were notable for dyskinetic septal wall motion with a repeat electrocardiogram uncovering pre-excitation pattern consistent with WPW syndrome and an expected right septal or posteroseptal accessory pathway. Tachycardia had self-

We report three patients with bilateral lower limb weakness as the initial symptom. Case 1 presented at 37 years old with bilateral lower limb weakness. The condition gradually worsened, eventually leading to wheelchair dependence. He later sought medical attention for heart failure. Echocardiography showed diffuse left ventricular dysfunction, and the electrocardiogram revealed third-degree atrioventricular block. Case 2 developed bilateral lower limb weakness at 38 years old, with milder symptoms. The main cardiac manifestation was paroxysmal atrial fibrillation. Case 3 presented at 33 years

BACKGROUND: Stress cardiomyopathy (SCM) is an acute heart failure syndrome characterized by transient, usually reversible left ventricular systolic dysfunction with normal or enhanced basal compensatory wall motion abnormalities involving the left ventricular anterior septum and apex, resulting in a "ballooning" appearance. However, it has rarely been reported in patients undergoing spinal surgery.
CASE PRESENTATION: We report a case of severe stress cardiomyopathy in a scoliosis patient with pectus excavatum who underwent spinal corrective surgery. During the wake-up period, circulatory coll

Transthyretin (ATTR) cardiac amyloidosis has recently received increased attention; however, the diagnosis is often delayed. We present a case of ATTR cardiac amyloidosis in which a comprehensive history-taking and focused physical examination played an important role in establishing the diagnosis. A 75-year-old man was referred to the cardiology department for left ventricular hypertrophy on electrocardiography. No fourth sound was audible despite concentric biventricular hypertrophy and diastolic dysfunction on echocardiography. Additional history-taking revealed that he had undergone bilate

A 66-year-old woman with a history of hypertension, ischemic stroke, and rheumatoid arthritis presented to the hospital with severe angina pectoris and dyspnea and was diagnosed with myocardial infarction (MI). Coronary angiography revealed multisystem coronary artery occlusive disease. Due to refractory myocardial ischemia/evolving MI, emergency coronary artery bypass grafting (CABG) was undertaken. Intraoperative transesophageal echocardiography additionally revealed an apical muscular ventricular septal defect (VSD). Concomitant VSD repair was deferred due to the absence of surface evidence

Secondary mitral regurgitation (MR) is present in up to half of patients with dilated cardiomyopathy, and is associated with a poor prognosis. It primarily results from progressive left ventricular remodelling, papillary muscle displacement and tethering of the mitral valve leaflets. Mitral valve repair with an undersized ring annuloplasty is the reparative procedure of choice in the treatment of secondary MR. However, this technique is associated with a 30-60% incidence of recurrent moderate or greater MR at mid-term follow-up, which results in progressive deterioration of left ventricular fu
