Neuronal intermediate filament neurologic autoimmunity in a patient with Merkel cell carcinoma receiving pembrolizumab.

BACKGROUND: Neuronal intermediate filament (NIF) autoimmunity is an uncommon paraneoplastic neurologic syndrome that has increasingly been recognized in association with immune checkpoint inhibitor (ICI) therapy. Merkel cell carcinoma is among the malignancies most frequently associated with NIF antibodies, although reported cases remain limited. CASE PRESENTATION: A 55-year-old female with stage IIIA Merkel cell carcinoma developed vertigo, gait instability, dysarthria, and persistent right hemifacial and cervical spasms after receiving second dose of pembrolizumab for recurrent disease. Bra…

Innovations in conservative and surgical therapy for Ménière's disease-narrative review.

Ménière's disease (MD) is an inner ear disorder of poorly understood etiology, and its management remains symptomatic. This review presents novel therapeutic approaches to control vertigo, improve audiological function, and enhance patient outcomes in MD. The literature was reviewed to explore conservative and surgical methods under current investigation. Recent advances in pharmacological therapy target oxidative stress, immune-mediated pathways, and neuromodulatory mechanisms. Clinical trial reports evaluating the efficacy of new oral treatment-ebselen were included. Preliminary report descr…

The metabolic matrix hypothesis of recurrent BPPV: Otoconia-otolithic membrane-endolymph fragility.

Benign paroxysmal positional vertigo (BPPV) is treated as a purely mechanical disorder of displaced otoconia, yet this model alone does not readily account for spontaneous otoconia detachment, bilateral or multicanal disease, high recurrence rates, or the clustering of BPPV with systemic metabolic conditions in a subset of patients. This article proposes and structures a unifying mechanistic model-the Metabolic Matrix Hypothesis-linking systemic metabolic states to otoconial fragility, and derives testable predictions from it. It is a hypothesis-generating narrative and theoretical synthesis-n…

Age-Related Increase in Skull Vibration-Induced Nystagmus Slow-Phase Velocity in a Clinical Population with Normal vHIT and VEMP: A Retrospective Cross-Sectional Analysis.

Background/Objectives: Skull vibration-induced nystagmus (SVIN) is a reproducible marker of vestibular asymmetry, but its relationship with age in patients with normal conventional tests remains unclear. We evaluated whether age is associated with the presence of SVIN and with its slow-phase velocity (SVIN-SPV). Methods: In this retrospective cross-sectional analysis, 164 patients with dizziness or vertigo and normal video head impulse test (vHIT) and VEMPs were studied (107 SVIN-positive, 57 SVIN-negative). Logistic regression tested whether age predicted SVIN positivity; among the 107 positi…

Mutation in RNF170 Causes Unsteady Gait with Hypertrophic Olivary Degeneration.

Hypertrophic olivary degeneration (HOD) is a rare form of transsynaptic degeneration resulting from disruption of the Guillain-Mollaret triangle, typically presenting with palatal tremor, ataxia, and nystagmus. Mutations in the Ring Finger Protein 170 (RNF170) gene have been associated with autosomal dominant sensory ataxia. However, the coexistence of HOD and an RNF170 mutation has rarely been reported. A 60-year-old female presented with progressive gait instability, vertigo, oscillopsia, and a sensation of walking on cotton. Neurological examination revealed pendular nystagmus, slow rhythmi…

Longitudinal study of Emotional Intelligence Scale items associated with improvement in Meniere's disease.

BACKGROUND: Meniere's disease is a stress-related inner ear disorder characterized by recurrent vertigo and auditory symptoms. Evidence linking stress-coping abilities to clinical outcomes remains limited. Emotional intelligence (EI), particularly the situation-management domain of the Emotional Intelligence Scale (EQS), may influence symptom improvement through adaptability and behavioral flexibility. OBJECTIVE: To examine whether EI assessed by the EQS is associated with improvement in Meniere's disease. METHODS: This retrospective study included 93 patients aged 23-82 years who had baseli…

Periodic alternating nystagmus as a diagnostic clue in MRI-negative Wernicke encephalopathy mimicking multiple system atrophy: a case with complex heterogeneous dizziness.

Dizziness in elderly patients is heterogeneous and nonspecific, leading to frequent misdiagnosis. Wernicke encephalopathy (WE) is a severe but reversible nutritional disorder responsive to timely thiamine supplementation; it often lacks typical MRI changes and may mimic neurodegenerative diseases such as multiple system atrophy (MSA). A 62-year-old male with chronic heavy alcohol use presented with 1 year of recurrent heterogeneous dizziness, vertigo, unsteadiness, presyncope, falls, and nocturnal visual hallucinations. Severe orthostatic hypotension and pyramidal signs initially suggested MSA…

Dual Perspectives: A Nurse-Patient's Lived Experience Informing Holistic Perioperative Care and Rehabilitation for Acoustic Neuroma.

BACKGROUND: Acoustic neuroma (AN), or vestibular schwannoma, accounts for 8% to 10% of intracranial neoplasms and 80% of cerebellopontine angle tumors. Postoperative vestibular symptoms are common, yet research has largely failed to integrate the perspectives of both health care providers and patients. METHOD: This case report documents the perioperative experiences of a nurse diagnosed with AN, including psychological changes, symptom progression, and rehabilitation, from a dual nurse-patient perspective. RESULTS: The patient presented with vertigo and an unsteady gait and was diagnosed wit…

Idiopathic Hypertrophic Pachymeningitis with Elevated Anti-Thyroglobulin Antibodies-A Case Report.

Background and clinical significance: Idiopathic hypertrophic pachymeningitis (IHPM) is a rare inflammatory disorder characterized by diffuse or focal dural thickening and heterogeneous presentations. We report a corticosteroid-responsive IHPM with elevated anti-thyroglobulin (anti-Tg) antibodies despite oncologic control after thyroidectomy. This case suggests that systematic assessment for autoimmunity should be a standard component of the IHPM work-up. Case presentation: A 77-year-old woman presented with recurrent vertigo, imbalance, and headaches. Brain MRI showed diffuse pachymeningeal t…

Cerebrospinal fluid leakage following penetrating spinal trauma in a patient with congenital spinal dysraphism: A case report.

INTRODUCTION: Spinal dysraphism, a congenital condition characterized by vertebral malformations like spina bifida occulta, may lead to cerebrospinal fluid (CSF) leaks following a minor trauma. We report a case of a 19-year-old male with congenital spinal dysraphism who developed a CSF leak after a stab wound to the lumbosacral region. PRESENTATION OF CASE: The patient was admitted four days post-injury with CSF leakage, vertigo, and nausea but no neurological deficits. Imaging revealed a dural tear and spina bifida occulta. The combined surgical intervention, including primary closure, dural…

Hyperplastic growth, not hydrostatic distension, in endolymphatic hydrops in humans challenges the classic view of Meniere's disease.

Meniere's disease (MD), a degenerative inner ear disorder, is characterized by debilitating episodic vertigo and hearing fluctuations, progressing to permanent sensory impairment. The prevailing dogma attributes these symptoms to abnormal inner ear fluid buildup-endolymphatic hydrops (EH)-with pressure rise and repetitive microtrauma to sensory epithelia. However, this pressure-based mechanism lacks direct experimental evidence and fails to explain key clinical aspects. To revisit EH, we performed 3D reconstructive, machine-learning-enhanced histological analyses and immunohistochemistry on po…

Coexistence of anti-neurexin-3α-associated autoimmune encephalitis and systemic lupus erythematosus in an adult patient: a case report.

Autoimmune encephalitis (AE) associated with anti-neurexin-3α antibodies, an uncommon variant of AE, exhibits particularly low frequency when coexisting with systemic immune dysregulation. The heterogeneous clinical manifestations of this neuroinflammatory disorder hinder timely diagnosis. We describe a 55-year-old male diagnosed with anti-neurexin-3α-associated AE, complicated by systemic lupus erythematosus (SLE) and antiphospholipid antibodies. The case presented with pyrexia persisting for 3 days, followed by acute-onset vertigo and encephalopathy evolving over 7 h. The clinical course was…
