Cofactors of alcohol-related trauma at a rural trauma center.

To analyze the cofactors of alcohol-related trauma at a rural, Level II trauma center, 127 admitted trauma patients were interviewed about psychological problems, social factors, and medical histories. Patients were divided into two groups, blood alcohol content (BAC) positive and BAC negative, for analysis. Seventy-one patients (56%) were BAC negative; 56 (44%) were BAC positive. Forty-three of the 82 males (52%) had positive BAC tests, compared with 13 of the 45 females (29%) (P = .01). Fifty-one of the 56 BAC-positive patients (91%) were aged 21 to 50 years, compared with 29 of the 71 BAC-n

[Treatment of alcoholic liver diseases and psychiatric and psychosocial problems].

Only about 15% of the subjects abusing ethanol will eventually develop cirrhosis of the liver, suggesting that other factors in addition to the consumption of large quantities of ethanol play a role in the pathogenesis of alcoholic cirrhosis. Important contributors may be infection with hepatitis viruses, in particular HCV, protein-calorie malnutrition and immunologic factors. Abstinence improves the prognosis of patients with alcoholic cirrhosis, provided that the liver disease is not too far advanced. No pharmacotherapeutic intervention has shown a convincing improvement of the prognosis of

Treating acute withdrawal from alcohol and other drugs.

The withdrawal syndrome is the predictable constellation of signs and symptoms that follow abrupt discontinuation of, or rapid decrease in, the use of a substance that has been used consistently over a period of time. To manage the withdrawal syndrome in a way that is safe and humane for the patient, the nurse must understand the pharmacological principles underlying the drug use. Whereas supportive nursing care is often effective, many patients require some pharmacotherapy. Rating scales for assessing signs and symptoms of withdrawal and for placing patients in the most appropriate setting ca

Renal angiomyolipoma: embolotherapy with a mixture of alcohol and iodized oil.

PURPOSE: To evaluate the efficacy of superselective embolotherapy of renal angiomyolipomas with a 1:3 mixture of iodized oil and absolute ethanol as embolic material. MATERIALS AND METHODS: Fifteen patients with 21 symptomatic renal angiomyolipomas were treated with embolization. The sizes of tumors ranged from 6 cm to 15 cm (mean, 8.6 cm). Six of 15 patients were diagnosed with tuberous sclerosis. The diagnoses of renal angiomyolipoma were made from characteristic computed tomographic findings. All angiomyolipomas were successfully embolized with a 1:3 mixture of iodized oil and absolute eth

Efficacy of an alcohol-free chlorhexidine mouthrinse as an antimicrobial agent.

STATEMENT OF PROBLEM: Antimicrobial mouthrinses have become an important part of comprehensive dental care. However, mouthrinses that contain alcohol have been shown to be detrimental to patients undergoing radiation therapy for head and neck cancer, to some immunocompromised patients, to families with small children, and to persons sensitive to alcohol. PURPOSE: This study evaluated the effectiveness of alcohol-free, 0.12% chlorhexidine mouthrinse in reducing microbial levels when compared with commercially available 0.12% chlorhexidine (CHX) and essential oils mouthrinses. Hospital utilization of Saskatchewan people with fetal alcohol syndrome.

We describe the hospital utilization of 194 Saskatchewan persons with Fetal Alcohol Syndrome (88% Aboriginal), born between 1973-92. Complete provincial hospitalization data were obtained for 128 patients; partial data for 29 patients. Proportionately more persons missing data were adopted, not living with biological family members or were deceased. The hospital separation rates for the children with FAS, pooled from 1987-91, compared to the 1989-90 Saskatchewan rates were significantly higher (95% level of confidence) for males and females < 1 year, 1-4 years and 5-14 years of age. Effects of moderate alcohol consumption on the central nervous system.

The concept of moderate consumption of ethanol (beverage alcohol) has evolved over time from considering this level of intake to be nonintoxicating and noninjurious, to encompassing levels defined as "statistically" normal in particular populations, and the public health-driven concepts that define moderate drinking as the level corresponding to the lowest overall rate of morbidity or mortality in a population. The various approaches to defining moderate consumption of ethanol provide for a range of intakes that can result in blood ethanol concentrations ranging from 5 to 6 mg/dl, to levels of

Natural history of alcoholic myopathy: a 5-year study.

Chronic myopathy is a common complication of alcoholism, but its natural history has not been well described. We, therefore, studied muscle structure and function in a 5-year study of 30 chronic alcoholics who became abstinent and 20 who relapsed, and 40 control subjects. The mean strength of the abstaining alcoholics increased from 18.6 to 23.2 kg; but, after 5 years, they were still substantially weaker than controls. In a subset who showed histological myopathy, the strength of half of the patients remained two standard deviations below that of controls. Alcoholics who relapsed tended to be

A geographic relation between alcohol availability and gonorrhea rates.

BACKGROUND AND OBJECTIVES: The availability of alcohol measured as alcohol outlet density is associated with numerous alcohol-related outcomes in small area analysis. A number of studies suggest that high-risk sexual behavior should also be considered an alcohol-related outcome. GOAL OF THIS STUDY: To assess the geographic relationship between alcohol availability and high-risk sexual behavior at the neighborhood level. STUDY DESIGN: Ecological analysis of the geographic relation between off-premise, on-premise, and total alcohol outlet density and reported gonorrhea rates among 155 urban re

Management of portal hypertension and esophageal varices in alcoholic cirrhosis.

Portal hypertension is a frequent consequence of liver disease, especially alcoholic cirrhosis. Unabated elevations in portal pressure may presage an esophageal variceal hemorrhage. Propranolol or isosorbide therapy is effective in the prophylaxis of variceal bleeding. In patients with acute variceal hemorrhage, endoscopic sclerotherapy or ligation is the best initial intervention. Stapled esophageal transection is often effective when endoscopic management fails. Surgical shunts have a lower associated mortality rate when they are performed electively and involve partial shunting of hepatic b

Estimating blood alcohol level from observable signs.

Attempts to induce hosts and friends to prevent drinkers from reaching dangerous levels of alcohol impairment depend upon the ability to judge impairment from observable signs of physical appearance and behavior. In a study of this ability, researchers first observed and recorded signs of change in behavior and physical appearance among dosed drinkers in small social groups (n = 149). Signs were grouped into impairment levels corresponding to three broad categories of blood alcohol concentration (BAC): < 0.04% (no signs), 0.04%-0.08%, and > 0.08%. Next, drinkers were then classified into judge

The formation and measurement of DNA neuroadduction in alcoholism. Case report.

We present a case report of an intoxicated alcoholic driver who sustained fatal motor vehicle injuries. We subsequently quantified ethanol-derived acetaldehyde (ACE) DNA products in his brain, which may represent a major contributor to clinical alcoholic use and complications. Further, ACE DNA neuroadducts may indicate chronic exposure to alcohol, as demonstrated by 32P-prelabeled DNA and two-dimensional thin-layer chromatography. ACE and other unknown neuroadducts were evident in the histologically normal frontal, parietal, and caudate lobes. DNA neuroadduct formation was extensive and simila

Excess weight risk factor for alcoholic liver disease.

Though the hepatotoxicity of ethanol has been established, only 8% to 20% of chronic alcoholics develop cirrhosis. The aim of this study was to assess whether being overweight is a risk factor for alcoholic liver disease. One thousand six hundred four alcoholic patients were studied. According to the liver biopsies, 194 patients had a normal liver; 402 had steatosis without fibrosis; 281 presented with fibrosis, with or without steatosis; 119 presented with acute alcoholic hepatitis (AAH) without cirrhosis; 232 indicated cirrhosis without AAH; and 179 presented with cirrhosis with AAH. Alcoholism in a geriatric outpatient clinic of São Paulo-Brazil.

Alcohol abuse and dependence are an increasing health problem among the elderly, but there is only scanty information about their prevalence and associated risk factors in developing countries. The authors set out to evaluate the prevalence and associated clinical/demographic features of alcoholism in a sample of male elderly subjects attending a Geriatric Primary Health Outpatient Clinic in a State University Hospital in the City of São Paulo-Brazil. Three hundred four patients were assessed with the Brazilian version of the Michigan Alcoholism Screening Test and a semistructured questionnair

Hostility, cigarette smoking and alcohol consumption in the general population.

Hostility has been associated with coronary heart disease, and hostility may affect coronary risk through its influence on risk factors such as cigarette smoking and alcohol consumption. The objective of this study was to determine relationships between hostile personality, cigarette smoking and alcohol consumption in the general population. The Edinburgh Artery Study comprises a cross-sectional survey of 1592 men and women aged 55-74 years sampled from age-sex registers of 10 general practices throughout the city. The Bedford-Foulds Personality Deviance Questionnaire was used to elicit extrap

Alcohol consumption and pregnancy in the Mexican national addiction survey.

In 1988, the General Directorate of Epidemiology and the Mexican Institute of Psychiatry conducted the first National Addiction Survey (ENA), providing regional and national data on alcohol, tobacco, and drug use. The ENA providing a subsample of women who have been pregnant at some time in their lives. There were 5,234 affirmative responses. Women were asked if they had suffered any of three adverse outcomes during their last pregnancy: spontaneous abortion, stillbirth, and congenital abnormalities. Prevalence of spontaneous abortion was 3.8%, stillbirth 1.2%, and congenital abnormalities 1.1

Moderate maternal alcohol consumption and risk of spontaneous abortion.

Maternal alcoholism is known to have adverse effects on reproduction and fetal development, but the effects of moderate consumption remain controversial. In a previous case-control study, we found a doubled risk of spontaneous abortion with an average consumption of seven or more drinks per week during the first trimester. To confirm this finding while avoiding potential biases from the case-control design, we examined moderate alcohol consumption in a prospective cohort study of over 5,000 pregnant women. An interview in the first trimester asked about alcohol consumption during the week befo

Effects of alcohol on sympathetic activity, hemodynamics, and chemoreflex sensitivity.

Alcohol intake has been shown to worsen obstructive sleep apnea and increase nocturnal hypoxemia. The mechanisms of this action are unclear. Animal studies suggest that a reduction in chemoreflex sensitivity may be implicated. Using a double-blind, randomized, vehicle-controlled design, we tested the hypothesis that oral alcohol intake depresses chemoreflex sensitivity in humans. We examined the effects of oral alcohol intake (1.0 g/kg body wt) on blood pressure, heart rate, heart rate variability, muscle sympathetic nerve activity, forearm vascular resistance, and minute ventilation in 16 nor
