Common peroneal nerve block for sciatica.

The effectiveness of common peroneal nerve block for lumbar disc herniation was evaluated in a double blind study. Common peroneal nerve block was performed near the fibular head in nine patients using 2% lidocaine and in 10 patients using saline. The average pain scale score decreased from 3.1 to 0.6 in the lidocaine group, whereas it decreased from 3.0 to 2.6 in the placebo group. The average result in the straight leg raising test increased from 61 degrees to 84 degrees in the lidocaine group, but from 44 degrees to 50 degrees in the placebo group. Lower leg pain lessened more in the lidoca

Continuous brachial plexus infusion of butorphanol-mepivacaine mixtures for analgesia after upper extremity surgery.

We have recently reported that continuous administration of butorphanol into the brachial plexus neurovascular sheath provided superior analgesia compared with that obtained with continuous i.v. administration. Furthermore, we found that analgesia was most pronounced when a mixture of mepivacaine and butorphanol was given and that butorphanol alone ranked next. In this study, we increased the dose of butorphanol, compared with that used in our previous reports, and an initial bolus dose of butorphanol was administered into the brachial plexus neurovascular sheath just after surgery had ended.

[The spread of subarachnoid hyperbaric dibucaine in the term parturient].

To study the effect of patient's age, height, body mass index (BMI), site of injection, and volume injected on determining the spread of hyperbaric spinal anesthesia in the term parturient, we performed a retrospective analysis on 86 parturients who had received 0.3% hyperbaric dibucaine for cesarean section. All patients received subarachnoid injection of the drug in the left lateral decubitus position on a horizontal operating table. After the drug was injected, the final cephalad level of analgesia to pinprick was measured. Multiple regression analysis revealed that BMI and volume injected

[Subdural, extra-arachnoid block as a complication of stellate ganglion block: documentation with ultrasound].

We present a patient who developed a high central neural block after stellate-ganglion-blockade. The underlying pathophysiology was assessed via sonographic imaging. Stellate ganglion block was performed in a 19-year old patient according to the standard technique. Multiple aspiration tests were negative and a test dose of 3 ml bupivacaine 0.25% was injected. After a 3 min interval another 5 ml were injected. Two minutes after the local anaesthetic administration the patient reported nausea and sensations in the upper extremity. Spontaneous respiration efforts stopped, and the patient became u

Effects of lidocaine on salicylate-induced discharge of neurons in the inferior colliculus of the guinea pig.

Using the extracellular recording method, the effects of lidocaine (a local anesthetic known to relieve tinnitus) on discharge of inferior colliculus (IC) neurons of the guinea pig were studied before and after salicylate (200 mg/kg) administration. The salicylate-induced discharge was inhibited by intravenous injection of lidocaine at a concentration (1 mg/kg) clinically used for treating tinnitus. IC neurons could be classified into two groups according to the difference in sensitivity to lidocaine: (1) weakly-sensitive neurons and (2) highly-sensitive neurons. In weakly-sensitive neurons, t

Unusual complications of intermittent self-catheterisation in spinal cord injury patients.

Three unusual complications of intermittent self-catheterisation in spinal cord injury patients observed over a period of 30 months are described. (1) Two patients performing intermittent catheterisation with a PVC catheter requiring the use of lignocaine gel to numb the urethra, developed allergic reaction in the form of swelling and erythematous lesions around the external urethral meatus. This rare complication was overcome by switching over to the use of a Lofric catheter instead of a PVC catheter for intermittent catheterisation. (2) Urethral bleeding was observed on three occasions in ma

Determination of highest no effect dose (HNED) for local anaesthetic responses to procaine, cocaine, bupivacaine and benzocaine.

The highest no effect doses (HNEDs) for the local anaesthetic (LA) effects of procaine, cocaine, bupivacaine and benzocaine were determined using the heat lamp/hoof withdrawal model of Kamerling et al. (1985b) and the abaxial sesamoid block model of local anaesthesia. The heat lamp rapidly (4 or 5 s) increased the temperature of the superficial skin layers of the pastern to about 90 degrees C, at which point the animal sharply withdrew its hoof. Effective LA blockade precluded this response and superficial skin temperatures exceeded 120 degrees C. Thermal stimulus experiments were routinely te

Transfer of lidocaine and bupivacaine across the isolated perfused human placenta.

Drug permeability and pharmacokinetics through the placenta are important factors determining foetal drug exposure. The purpose of the present study was to establish a perfused human placental cotyledon system to assess the placental transfer of lidocaine and bupivacaine, widely used local anaesthetics in obstetric anaesthesia. Term placentas were obtained immediately after delivery with maternal consent and a two-hour recycling perfusion of a single placental cotyledon was performed. Bupivacaine or lidocaine with antipyrine as a reference compound were added to the maternal reservoir and thei

On the relative potency of amino-amide local anaesthetics in vivo.

With the aim of comparing the analgesic effectiveness of lidocaine, prilocaine, bupivacaine and etidocaine in vivo, a study of the relationships between dose and duration of infraorbital nerve block (IONB) of various intensities (IONB degrees 3-10) was performed in the rat. With increasing doses longer durations of action were obtained. Further analyses were performed using multiple regression analysis. The log (dose)-duration lines for bupivacaine and etidocaine were found to be linear, whereas those for lidocaine in degree 3 and degree 10 IONB and for prilocaine in degree 10 IONB were so onl

Chloroprocaine and lidocaine decrease hospital stay and admission rate after outpatient epidural anesthesia.

Lumbar epidural anesthesia with 20 ml of either 3% 2-chloroprocaine (C), 1.5% lidocaine (L), or 1.5% mepivacaine (M) with epinephrine was studied in 84 outpatients undergoing surgery (extracorporeal shock wave lithotripsy (ESWL]. The average duration of the procedure was 31.9 minutes. The total duration of sensory anesthesia was 133 +/- 28 minutes (C), 182 +/- 38 (L), and 247 +/- 42 (M) (p less than 0.05). Times to discharge were 269 +/- 62 minutes (C), 284 +/- 62 (L), and 357 +/- 71 (M). The time to discharge with M, almost six hours, was significantly longer than with C or L. There was a tre

The use of lignocaine-prilocaine local anaesthetic cream for pain-free venepuncture in laboratory animals.

An assessment was made of the effects of topical application of a eutectic mixture of local anaesthetics (EMLA cream) in a number of species of laboratory animals. Application of EMLA cream enabled percutaneous insertion of catheters into the cephalic vein in dogs and cats and the marginal ear vein in rabbits without causing any detectable pain or discomfort. Application to the tail in rats prior to percutaneous cannulation of the lateral tail vein did not produce a significant reduction in the behavioural responses to venepuncture. EMLA cream represents a useful refinement of current techniqu

Local anaesthetic block protects against electrically-induced damage in peripheral nerve.

This study is one of a series addressing the mechanisms involved in the production of neural damage caused by continuous, prolonged electrical stimulation of peripheral nerve. It has been previously shown that sustained, high frequency electrical stimulation of the cat's peroneal nerve may cause irreversible neural damage in the form of axonal degeneration of the large myelinated fibres. In this study we demonstrate that blocking the action potentials on most of the nerve fibres with local anaesthetics (10% procaine or 2% lidocaine) almost completely prevents the axonal degeneration. The aboli

Vasoactive actions of local anaesthetics on human isolated umbilical veins and arteries.

1. An in vitro study, using helical preparations of human umbilical arteries and veins obtained from healthy women at term pregnancy, was designed to determine: (a) whether three local anaesthetics commonly utilized in obstetric anaesthesia (bupivacaine, 2-chloroprocaine, and lignocaine) can induce contraction or relaxation of resting umbilical vessels; (b) whether these agents can induce contraction or relaxation of umbilical vessels which have been previously induced to contract by a known activator, potassium chloride (KCl); and (c) the relative potency of these agents as compared to KCl. 2

Local anaesthetics inhibit influx of calcium, sodium and potassium into rat ileum, diaphragm and human isolated saphenous vein.

The effects of 5 different local anaesthetics, lignocaine, prilocaine, etidocaine, mepivacaine, bupivacaine, on uptake of Ca2+, Na+ and K+ were studied, using a Kone Microlyte Analyzer, into 3 different types of muscle fibres, the rat ileum, rat diaphragm and human isolated saphenous vein. The aim of the present experiments was to see if local anaesthetics had a calcium antagonistic activity, like that produced by calcium antagonists, e.g. verapamil. The results showed that local anaesthetics in moderate and high concentrations (more than 100 microM) depressed uptake of Ca2+, as well as the Na

A pilot study of the clinical problem of regionally anesthetizing the pulp of an acutely inflamed mandibular molar.

The diplomates of the American Association of Endodontists were surveyed regarding difficulties in regionally anesthetizing acutely painful mandibular molar teeth. Frequently patients developed evidence of adequate regional block anesthesia but experienced pain when the dentist attempted access. A pilot projection was undertaken to determine if a scientific basis existed for this problem. With the animal under general anesthesia, the mandibular nerve of a cat was isolated and then regionally blocked. A stimulating electrode was implanted in a cuspid tooth and a receiving electrode in the corte

Experimental studies of new topical anaesthetics on the oral mucosa.

In order to reduce pain by injection, a frequently used topical anaesthetic is Xylocaine 5% ointment. This preparation, however, has certain limits in effectiveness. A new topical anaesthetic formulation, EMLA (Eutectic Mixture Local Anaesthetics), has been developed. Promising local anaesthetic properties on intact skin made it interesting to investigate the clinical usefulness of EMLA on mucous membranes and to find a suitable time of application. 30 volunteers, 20-40 yr, took part in the study. In two experimental double blind series, the topical anaesthetic effect was compared for EMLA 5%

Blockade of cardiac nerves by intrapericardial local anaesthetics in the conscious rabbit.

Local anaesthetic drugs were instilled into the pericardial sac of conscious rabbits through a chronically implanted catheter. Twenty mg of procaine HCl always caused complete blockade of cardiac vagal and sympathetic efferent nerves, tested by eliciting the baroreceptor-heart rate reflex, and abolished the reflex depression of renal sympathetic nerve activity elicited by impeding left ventricular outflow. It also slowed heart rate by a direct effect on the sinoatrial pacemaker. When the same dose of procaine was given intravenously there were only transient changes in blood pressure, heart ra

Cholecystokinin and satiety in pigs.

Twenty-three pigs, 1-3 mo of age, were fitted variously with intraperitoneal, intrajugular, intraportal, and intraduodenal catheters. After a 4-h fast, porcine cholecystokinin (CCK), 5-40 Ivy dog units/kg body wt (IDU/kg); caerulein, 0.25-2 micrograms/kg; or the octapeptide of cholecystokinin (CCK-OP), 5-40 IDU/kg, was given parenterally; or 2-5% sodium oleate or 5% protein hydrolysate (5 ml/kg) was injected intraduodenally. Pelleted feed intake was then measured for 10 min. Food intake was depressed in a dose-related fashion in all instances as compared to after 0.9% NaCl control injections.
