A Novel Snore Recording Application Demonstrating High Correlation With Two-channel Portable Monitoring for Diagnosing Sleep Apnea Syndrome.

BACKGROUND/AIM: Obstructive sleep apnea syndrome is a common disorder that is diagnosed via polysomnography. However, polysomnography is complex and expensive. Therefore, the aim of this study was to develop a novel snore recording application to evaluate sleep apnea status. PATIENTS AND METHODS: First, we monitored 67 participants using a two-channel portable monitor and a smartphone recording application. Acoustic data and SpO2 or airflow data from 25 participants whose apnea-hypopnea index (AHI) was ≥5 were used to develop an algorithm for evaluating sleep apnea. RESULTS: The coefficient

Body composition profiles and metabolic vulnerability in comorbid insomnia and sleep apnea (COMISA): A large-scale BIA analysis.

BACKGROUND: Comorbid insomnia and sleep apnea (COMISA) presents a greater clinical burden than obstructive sleep apnea (OSA) alone. We investigated the association between body composition and insomnia to identify the body composition phenotype of COMISA. METHODS: This cross-sectional study included 2915 patients with moderate-to-severe OSA (apnea-hypopnea index ≥15) who underwent polysomnography and body composition analysis. Participants were categorized into OSA (Insomnia Severity Index [ISI] < 15) and COMISA (ISI ≥15). Skeletal muscle mass (SMM), visceral fat area (VFA), and the SMM/VFA r

Development and Explainable Machine Learning Validation of a Novel Sleep Disturbance Ratio for Obstructive Sleep Apnea Severity Assessment.

Background/Objectives: Obstructive sleep apnea (OSA) is traditionally classified according to the apnea-hypopnea index (AHI), although AHI alone does not fully capture the heterogeneity of disease severity. This study introduces a novel polysomnography-derived biomarker, the Sleep Disturbance Ratio (SDR), and evaluates its contribution to OSA severity classification using explainable machine learning approaches. Methods: A retrospective study was conducted using polysomnographic data from 767 adults who underwent overnight sleep studies. SDR was calculated as the logarithmic ratio between ligh

Respiratory-hypoxic burden and multimorbidity-related vulnerability are distinct dimensions of obstructive sleep apnea: A retrospective cohort analysis.

BACKGROUND: Obstructive sleep apnea (OSA) is a heterogeneous disorder in which conventional apnea-hypopnea index (AHI)-based severity classification incompletely captures respiratory burden, nocturnal hypoxemia, obesity, and multimorbidity. Whether OSA severity is organized along distinct respiratory and multimorbidity-related dimensions remains unclear. OBJECTIVES: To determine whether OSA severity is organized along distinct respiratory-hypoxic and multimorbidity-related dimensions and whether these dimensions define clinically meaningful patient phenotypes. METHODS: We retrospectively ana

Dose-dependent vitamin D3 supplementation attenuates endothelial dysfunction and renal injury in an obstructive sleep apnea rat model.

BACKGROUND: Endothelial dysfunction driven by intermittent hypoxia (IH)-induced oxidative stress, nitric oxide depletion, and adhesion molecule upregulation underlies obstructive sleep apnea (OSA)-related cardiovascular complications. Vitamin D deficiency, which frequently coexists with OSA, may further amplify this endothelial injury through impaired endothelial nitric oxide synthase (eNOS) regulation and heightened renin-angiotensin-aldosterone system (RAAS) activity; however, the dose-dependent therapeutic potential of vitamin D3 on combined endothelial and renal outcomes in an IH model rem

Development and validation of machine learning models for obstructive sleep apnea risk stratification in high-risk pregnant women.

BACKGROUND: Obstructive sleep apnea (OSA) affects approximately 15% of pregnancies and is associated with adverse maternal and fetal outcomes. Although polysomnography (PSG) is the diagnostic gold standard, increasing clinical demand creates substantial bottlenecks in manual PSG scoring and specialist review. Conventional screening tools often show limited discriminative ability in high-risk referred populations. Therefore, optimized risk stratification models are needed to streamline clinical workflows, prioritize diagnostic resource allocation, and facilitate timely intervention for high-ris

Mechanistic insights into di(2-ethylhexyl) phthalate exposure in obstructive sleep apnea: network toxicology and in vitro validation.

BACKGROUND: Di(2-ethylhexyl) phthalate (DEHP), a widely used plasticizer and endocrine-disrupting chemical, has been associated with multiple adverse health outcomes. However, whether DEHP exposure contributes to poor outcomes in individuals with obstructive sleep apnea (OSA), particularly through interaction with chronic intermittent hypoxia (CIH), remains unclear. METHODS: We used NHANES data to assess the relationship between DEHP exposure and all-cause mortality in OSA patients. Environmental health-related targets of DEHP were identified through ChEMBL, SEA, and SwissTargetPrediction dat

Biventricular dysfunction and blood oxygenation deficits in obstructive sleep apnea: a prospective study with non-contrast cardiac MRI.

BACKGROUND: Conventional cardiac magnetic resonance imaging(CMR) metrics may remain normal in Obstructive Sleep Apnea (OSA) despite subclinical myocardial injury, limiting early risk stratification. This study aimed to evaluate whether biventricular function, mechanics, and tissue characteristics assessed by non-contrast CMR differ between patients with severe and non-severe OSA, and to explore their association with markers of nocturnal hypoxemia. METHODS: Seventy-five newly diagnosed OSA patients(62 male; age 43.7 ± 9.3 years) were prospectively included and underwent polysomnography follow

Olfml3 Regulates Microglial Inflammation and Neuronal Injury in Obstructive Sleep Apnea via Cybb-Mediated TLR4/NF-κB Pathway.

BACKGROUND: Neurocognitive impairment in obstructive sleep apnea (OSA) is primarily driven by intermittent hypoxia (IH)-triggered neuroinflammation, where microglia play a pivotal role. The involvement of Olfml3 in IH-induced neuroinflammation remains unclear. METHODS: Single-cell RNA sequencing (scRNA-seq) data from the hippocampi of IH-induced OSA mice were analyzed to identify cell subpopulations, with further focus on Olfml3's differential expression, enriched pathways, and differentiation trajectories in microglia. An in vitro OSA model was established using IH-treated microglia. qRT-PCR

Metabolic dysfunction-associated steatotic liver disease and obstructive sleep apnea: A cohort analysis of prevalence and hepatic outcomes.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most common chronic liver disease worldwide and often coexists with obstructive sleep apnea (OSA). Emerging evidence suggests that OSA may independently accelerate liver injury in MASLD. Despite this the epidemiology and clinical significance of OSA within the population with MASLD remain incompletely understood. AIM: To estimate the prevalence of OSA among MASLD and determine the impact of OSA on hepatic complications. METHODS: This was a large, multicenter, population-based retrospective cohort study conduc

Reducing referral-to-consultation delays in pediatric obstructive sleep apnea through standardized triage: A regional quality improvement initiative.

BACKGROUND: Timely management of pediatric obstructive sleep apnea (OSA) is essential to prevent sequelae. In publicly funded health systems, delays remain common, particularly in non-urban regions with limited specialty capacity. At a regional hospital serving Québec's Bas-Saint-Laurent region, prolonged wait times for adenotonsillectomy were observed. Preliminary review identified referral-to-consultation delay (Wait-1) as the principal access bottleneck, exceeding Pediatric Canadian Access Targets for Surgery. METHODS: Using SQUIRE 2.0 guidelines, we conducted a quality improvement initiat

Mortality impact of long-term home non-invasive ventilation in COPD patients with versus without obstructive sleep apnea.

RATIONALE: Coexisting chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea (OSA) is known as overlap syndrome. This may represent a distinct clinical phenotype that shows different responses after being initiated on noninvasive ventilation (NIV) for hypercapnic chronic respiratory failure. However, current data remain scarce. OBJECTIVE(S): This study estimated the impact of overlap syndrome versus COPD without OSA on transitions between 3 states (without/recovery from severe exacerbation, severe exacerbation and death) in patients initiated on domiciliary NIV therapy. Differential correlation patterns of serum prolactin and total antioxidant status in obstructive sleep apnea: A cross-sectional study.

Obstructive sleep apnea (OSA) severity classification relies primarily on the Apnea-Hypopnea Index (AHI), which quantifies event frequency but does not capture event duration or hypoxemic burden. We examined whether serum prolactin and total antioxidant status (TAS) show differential correlation patterns with frequency- versus duration-based polysomnographic parameters, potentially reflecting distinct pathophysiological mechanisms. This cross-sectional study enrolled 55 participants (controls: n = 12, AHI < 5; mild/moderate OSA: n = 22, 5 ≤ AHI < 30; severe OSA: n = 21, AHI ≥ 30) undergoing ov
