[A case of craniofacial injury by javelin].

We report a case of 16-year-old woman with craniofacial impalement injury by javelin (used by athletic sports) thrown from a distance of 30 meters. When the patient was admitted to the emergency department, the javelin had been in the orbita, and the patient was assessed by trauma team including an anesthesiologist. The patient was alert and complained of severe pain. Head CT and X-ray indicated the javelin had entered though the orbita into the maxillary sinus. Sphenoidal sinus was also partly destroyed by the impact. Anesthesia was soon induced in the emergency room to release the patient fr

Warning from Nagano: increase of vulvar hematoma and/or lacerated injury caused by snowboarding.

BACKGROUND: On the basis of the number of women snowboarders whose vulvar injuries we have recently treated, we believe that the cases of vulvar hematoma and lacerated injuries caused by snowboarding have been increasing. To analyze the causal relation between snowboarding and vulvar injuries, we investigated how women were injured during snowboarding. METHODS: During the last three ski-ing seasons at the five hospitals near the ski resorts in Nagano Prefecture, Japan, the clinical charts of all patients with vulvar injuries during snowboarding were reviewed for the number of patients treated

Surgical treatment for recurrent dislocation of the extensor carpi ulnaris tendon.

Twelve patients with recurrent dislocation of the extensor carpi ulnaris tendon were treated with repair or reconstruction of its tendon sheath, and each had a satisfactory result. We found three types of disruption of the fibro-osseous sheath. Type A: the fibro-osseous sheath ruptured ulnarly and the torn sheath lay superficial to the tendon (n=5). These were treated by reconstruction of the sheath using a piece of the extensor retinaculum. Type B: the fibro-osseous sheath ruptured radially and the torn sheath lay in the ulnar groove beneath the tendon (n=3). These were treated by direct sutu

[Paget-Schroetter syndrome caused by wrestling].

Upper extremity thrombosis (Paget-Schroetter syndrome) hints already at physical strains with regard to the synonymous term "effort thrombosis". We report two cases of upper extremity thrombosis caused by wrestling. Wrestling as an example of martial arts activity leads to traumatic venous intima lesions with resulting local activation of coagulation as a main reason in pathogenesis of these thromboses. The possibility of a Paget-Schroetter syndrome should be considered in differential diagnosis if upper extremity complaints of unknown origin occur after wrestling matches. This kind of subclav

An aneurysm involving the axillary artery and its branch vessels in a major league baseball pitcher. A case report and review of the literature.

Baseball pitchers appear to be prone to aneurysms of the axillary artery and its branches. The cause is probably related to repetitive compression of or tension on the vessels at the level of the pectoralis minor muscle and the humeral head, which is exacerbated by the pitching motion. The incidence of aneurysms of the axillary artery and its branches among pitchers and other athletes is not known, nor is it clear whether pitchers who are at high risk of vascular injury can be identified before irreversible damage to the vessels has occurred. Perhaps patients who have documented compression or

Dynamics of human flight on skis: improvements in safety and fairness in ski jumping.

This study of ski jumping includes three areas of research: Wind tunnel measurements with world class athletes in various flight positions, field measurements during the World Championships in Ski Flying 1994 in Planica (Slovenia) and a highly reliable mapping of ski jumping to a computable simulation model. The results explain the effects of equipment, flight style changes, the reason for the enhanced tumbling risk and high gust sensitivity observed. Consequences can be drawn for changes to the FIS regulations, the design of jumping hills and training methods. The internationally induced anor

Peroneal nerve entrapment in athletes.

Peroneal nerve entrapment is one of the less common causes of exercise-induced leg pain in competitive athletes. This type of lower extremity peripheral nerve dysfunction is usually associated with activities that subject the nerve to constant compression or repetitive trauma. Herein, we present our experience with 12 competitive athletes treated for peroneal nerve entrapment. Diagnostic electromyography results, intraoperative findings, and the results of cadaveric dissections are discussed.

[Frontal sinus impression fracture as a collision sequelae in football].

4 soccer accidents, causing the rare case of impression fracture of the frontal sinus, are introduced as soccer-specific lesions, which are caused by head-to-head collisions, being due to heading-maneuvers. Mostly the right frontal sinus was hurt. The indication for an operation is the obvious dislocation of the osseous fragments. Primary intentions for an intervention are the precise reconstruction of the anterior wall of the frontal sinus because of esthetic reasons and a permanent drainage in order to avoid late complications as for instance mucopyoceles, if the infundibulum has been damage

Operative treatment of typical overuse injuries in sport.

In this report 670 operations, performed in 643 patients were analyzed. The indication for the surgery was a chronic symptomatic overuse injury caused by sports and physical activity. 552 (82.4%) of the operations were performed in men and 118 (17.6%) in women. The mean age of the patients was 24 years. Almost 90 per cent of the patients had presented following competitive sports and ten per cent recreational sports. More than 91 per cent of the operations were performed for lower extremity problems. The knee, foot and ankle regions needed surgical treatment most often in this series. Tendon a

Sports hematuria.

Strenuous exercise makes extraordinary demands. The transition from rest to intensive physical activity can cause pathological changes in various organs, particularly in the urinary tract. Hematuria (microscopic or macroscopic) is one of the abnormalities commonly found after sports activity. This phenomenon can occur in noncontact sports (such as rowing, running and swimming) as well as in contact sports (boxing, football and so forth). The pathophysiology can be either traumatic or nontraumatic. Renal trauma and/or bladder injury due to repeated impact of the posterior bladder wall against t

Management of orbital blow-out fractures. Case reports and discussion.

Blow-out fractures are fractures of the orbital floor or medial wall that occur as a consequence of blunt trauma. Impact increases the intraorbital pressure, forcing the nondistensible orbital contents through the orbital floor. The fracture is commonly caused by impact from a baseball or tennis ball. However, any blunt trauma to the orbit, as from a knee or elbow, can result in a blow-out fracture. The characteristic clinical findings include double vision, a sunken globe, and numbness in the distribution of the infraorbital nerve. Sometimes, the only sign of a blow-out fracture is the abrupt

Prepatellar bursitis in wrestlers.

As part of a larger retrospective study examining all knee injuries sustained by the University of Iowa wrestling team over 6 years, prepatellar bursitis was found to be the most frequent injury and, therefore, was examined in depth. Of the 136 wrestlers studied, 13 developed an initial case of prepatellar bursitis. This represented 21% of all initial episodes of knee injuries. Five of these wrestlers had no recurrences, but the other eight together had 20 recurrences. Median time lost for the initial injury was only 4 days, but recurrences and surgeries added significantly to the total time l

Surfer's rib: isolated first rib fracture secondary to indirect trauma.

A case of anterolateral first rib fracture produced by indirect trauma in a surfer is presented. A 17-year-old man was seen in the emergency department with the complaint of left shoulder pain that developed while he performed a so-called lay back maneuver on a surfboard. No history of direct trauma was elicited. After physical examination revealed point tenderness high in the left axilla, radiographic evaluation of the chest showed an isolated fracture of the anterolateral aspect of the left first rib. No morbidity was associated with this fracture which, when produced by other forces, can ha
