Pyoderma gangrenosum: a new complication after groin exposure for attempted endovascular aortic stent-graft placement.

Pyoderma gangrenosum (PG) is an uncommon and frequently misdiagnosed inflammatory dermatosis. This neutrophilic inflammation of the dermis has been well described in relation to other inflammatory illnesses (i.e. inflammatory bowel disease). Although rare, PG is increasingly described in relation to trauma or surgical interventions. It is important for vascular surgeons to be aware of the potential development of PG at vascular access sites. Although endovascular surgery is a minimally invasive form of vascular surgery, it does require cutaneous manipulation, which can incite PG. The purpose o

De novo basilar artery aneurysm formation after placement of a Wingspan stent.

Report the de novo formation of an aneurysm after wingspan placement and describe the treatment modality. We report a patient with symptomatic basilar stenosis who underwent placement of a wingspan stent. On 9-month follow-up, the patient was found to have a new 5-mm aneurysm adjacent to the proximal tines of the stent. The patient subsequently underwent placement of an enterprise stent and coils for aneurysm occlusion. We reviewed the literature and discussed possible etiologies for the development of this unique de novo aneurysm.

Endovascular treatment for a thoracic-abdominal aortic aneurysm without fenestrations or branches.

We describe a case of a patient with a thoraco-abdominal aortic aneurysm, affecting the origin of the celiac trunk, with the particularity of the normal aortic diameter in the segment between superior mesenteric artery and both renal arteries. Endovascular treatment was performed with no fenestrated or branch endoprosthesis. The procedure was divided into two steps. In the first attempt, an aortic prosthesis was deployed at the infrarenal aorta. Then, a thoracic endoprosthesis was deployed in a second procedure. In this case, the celiac trunk was intentionally occluded in order to increase the

[Management of arterial access for thoracic endovascular aortic repair].

Problems associated with arterial access can sometimes occur in elder patients with arteriosclerosis. This study examined the management of patients with restricted arterial access using thoracic endovascular aortic repair (TEVAR). Thirty-four consecutive patients underwent TEVAR at our institution. TEVAR was performed using a Gore TAG device (n = 20), Talent thoracic stent graft (n = 10), and a homemade endoprosthesis device (n = 3). The sizes of the applied sheath introducer or delivery catheter were 20 Fr (n = 1), 22 Fr (n = 12), 24 Fr (n = 17), and 25 Fr (n = 4). Although the procedure was

Is EVAR the treatment of choice for aortoenteric fistula?

Aortoenteric fistula formation is a devastating condition regardless of whether it is primary or secondary (i.e. after previous aneurysm repair) in nature. Patients present with signs and symptoms of gastrointestinal bleeding with or without signs of systemic infection and are often in a very poor clinical condition. Conventional treatment consists of extensive open surgery (extra-anatomical bypass or aortic ligation), closure of fistula tract and complete removal of any prosthetic material. This treatment is associated with high morbidity and mortality and therefore more minimally invasive op

Iatrogenic type A aortic dissection after catheter intervention for the left subclavian artery.

Cardiac surgical procedure and catheter intervention of the aorta or its major branches have a potential risk for iatrogenic aortic dissection. This case demonstrates an iatrogenic type A aortic dissection after the elective balloon angioplasty for severe stenosis of the left subclavian artery orifice. The dissection retrospectively extended to the ascending aorta, and intramural hematoma was observed in the false lumen of the aorta. The ascending aorta was successfully replaced 14 days after the occurrence of dissection, using hypothermic circulatory arrest and antegrade selective cerebral pe

Evaluation of thrombogenic potential of electrospun bioresorbable vascular graft materials: acute monocyte tissue factor expression.

The purpose of this study was to quantify the acute expression of tissue factor (TF) by monocytes on interaction with electrospun bioresorbable constructs. A minimal expression of TF will demonstrate the potential for scaffolds to be used as a vascular graft without enhanced risk of failure from acute thrombotic occlusion. Polydioxanone (PDO) (60, 80, 120, and 160 mg/mL) and polycaprolactone (PCL) (80, 10, and 160 mg/mL) dissolved in 1,1,1,3,3,3 hexafluoro-2-propanol (HFP) were electrospun to form fibrous scaffolds. Circular discs (10 mm diameter) of each scaffold were disinfected and seeded w

New model of reperfused stroke by occlusion of the anterior cerebral artery in baboons.

BACKGROUND: The understanding of stroke has been greatly enhanced by studies employing nonhuman primate models of focal ischemia. However, devastating neurological disability in previously described stroke models has led to ethical concerns and difficulty achieving prolonged survival for the evaluation of long-term outcome. We determined if reversible occlusion of the anterior cerebral artery in baboons would produce a small infarct with minimal neurological impairment. METHODS: In six baboons, anesthetized with isoflurane, Guglielmi coils were placed by endovascular technique in the anterior

Repair of traumatic tricuspid insufficiency via minimally invasive port access.

We report on a successful tricuspid valve plasty using port-access minimally invasive cardiac surgery (MICS) for severe traumatic tricuspid insufficiency caused by blunt chest trauma suffered 15 years previously. A combination repair procedure, consisting of cleft closures, plication of the anteroseptal commissure, and ring annuloplasty, was necessary to achieve valve competence and proved possible via port access without difficulty. Port-access MICS is an alternative approach for tricuspid valve surgery.

[Aortic valve repair and patch closure for unruptured aneurysm of non-coronary sinus of valsalva with aortic regurgitation].

The patient was a 53-year-old woman who complained of chest pain. Echocardiography and angiography revealed mild aortic regurgitation (AR) with an eccentric jet and an unruptured aneurysm of the non-coronary sinus of Valsalva which protruded into the left atrium. Operative findings showed that tethering due to elongation of the circumference of the aortic wall at the level of the non-coronary sinus commissures caused AR. Then patch closure and partial sino-tubular (ST) junction plication were carried out. Postoperative echocardiography showed decrease of AR and complete repair of the aneurysm

Time does not heal every wound: coronary stent thrombosis of a bare-metal stent more than one decade after its implantation.

In the present report, we describe a unique case of very late stent thrombosis with a bare-metal stent that occurred more than a decade after stent implantation. Potential explanations for the late stent thrombosis are non-compliance to aspirin, late acquired malapposition of the stent, progression of atherosclerosis or in-stent restenosis. In our patient, none of these explanations seems to have played a role. Although the occurrence of (very) late stent thrombosis is not uncommon with drug-eluting stent (DES), it is rather unusual with bare-metal stent (BMS). Nevertheless, cardiologists shou

Clinical and angiographic follow-up of stent-only therapy for acute intracranial vertebrobasilar dissecting aneurysms.

BACKGROUND AND PURPOSE: Little has been known about the clinical and angiographic follow-up results of stent-only therapy for intracranial vertebrobasilar dissecting aneurysms (VBDA). The purpose of this study was to evaluate the feasibility, safety, clinical, and angiographic follow-up of stent-only therapy for VBDA. MATERIALS AND METHODS: Twenty-seven patients with 29 VBDAs (11 ruptured, 18 unruptured), not suitable for deconstructive treatment, underwent stent-only therapy. Feasibility, safety, clinical, and angiographic follow-up were retrospectively evaluated. Angiographic outcomes were

Control of troublesome bleeding during repair of acute type A dissection with use of modified rapid deployment hemostat (MRDH).

Troublesome bleeding occurred in the posterior portion of the proximal anastomosis in an ascending aorta graft, being performed on a patient with acute type A dissection. The suture line was close to the left coronary ostium, and attempts at suturing and other maneuvers failed to control hemorrhage. Modified rapid deployment hemostat (MRDH) was successful in achieving hemostasis. MRDH is a novel hemostatic agent with invaluable use in troublesome bleeding occurring during heart surgery.

Improved pulmonary artery geometry after a Norwood procedure by using a Venaflo II graft as an RV-MPA conduit.

Sano's modification of the right ventricle to main pulmonary artery shunt improves the surgical results of the Norwood procedure for hypoplastic left heart syndrome. Usually the transected distal stump of the main pulmonary artery is reconstructed by direct suturing of a polytetrafluoroethylene tube or suturing on a pericardial bifurcation patch. However, the resulting pulmonary artery geometry significantly affects the outcome of the staged procedure. Herein, we report our surgical strategy to improve the pulmonary artery geometry by using a commercially available Venaflo II graft (Bard, Temp

Transpulmonary stenting of both pulmonary arteries with a surgical access through redo sternotomy.

We report the case of a four-year-old boy with pulmonary atresia (PA) and ventricular septal defect (VSD) associated with MAPCAs who had a complete repair two times. He developed 21 months later stenosis at the origin of both pulmonary arteries. The conventional interventional catheterization approaches were not available due to femoral vein thrombosis and severe transient complete atrioventricular block due to the manipulations to catheterize the left pulmonary artery. The stenoses were treated by implant of stents using a surgical access to the proximal portion of a Contegra valved conduit (

Aortic thrombus due to essential thrombocytosis: strategies for medical and surgical management.

Essential thrombocytosis (ET) remains a rare cause of aortic thromboembolic disease. Management options vary based on the state of clinical presentation and range from observation and medical management to immediate operative intervention. We report a case of ET causing "trash foot" from an aortic thrombus. The patient was administered appropriate cytoreductive therapy and eventually underwent an aortic interposition graft. The various management strategies are discussed.

[Surgical treatment of abdominal tumours closely related to major blood vessels].

INTRODUCTION: Radical operative treatment of abdominal tumours closely related to major blood vessels often demands complex vascular procedures. OBJECTIVE: The aim of this paper was to present elementary principles and results of the complex procedures, based on 46 patients operated on at the Institute for Cardiovascular Diseases, Clinical Centre of Serbia, Belgrade, from January 1999 to July 2006. METHOD: Primary localisation of the tumour was the kidney in 14 patients, the suprarenal gland in 2, the retroperitoneum in 23 and the testis in 7 patients. Histologically, the most frequent were

Drug-eluting stent thrombosis during perioperative period.

Recently, the number of patients in whom a drug-eluting stent (DES) has recently been implanted and who need to undergo surgery or the most invasive procedure is increasing. However, there are limited data about the risk of perioperative thrombosis of DES. We evaluated the incidence and the risk factors of DES thrombosis during the perioperative period. Between January 2002 and December 2006, 141 patients who underwent surgery requiring discontinuation of a dual antiplatelet agent within 12 months of DES implantation were enrolled in one of the 3 study hospitals. We reviewed the clinical and p

Inflow stenosis obscures recognition of outflow stenosis by dialysis venous pressure: analysis by a mathematical model.

BACKGROUND: Recent studies have shown that inflow stenosis of haemodialysis grafts is more common than previously realized. The influence of inflow stenosis on graft haemodynamics and venous pressure (VP) surveillance has not been previously systematically studied. METHODS: We used a well-established mathematical model to determine the relation between inflow stenosis and static VP (adjusted for mean arterial pressure, VP/MAP), outflow stenosis and artery and vein luminal diameters. We applied low, median and high ratios of artery/vein diameters from 94 patients with grafts. The median ratio
