Incidence of subclinical trigeminal and facial nerve involvement in diabetes mellitus.

We investigated the frequency of subclinical trigeminal and facial nerve involvement in 40 patients with diabetes mellitus and without clinical signs of cranial nerve lesions. 60% of the patients had distal symmetric sensory polyneuropathy which was confirmed by nerve conduction studies. Trigeminal and facial nerve functions were evaluated electrophysiologically using the blink-reflex R1 component (BlinkR-R1), masseter reflex (MassR), the first exteroceptive suppression of the masseter muscle (Mass-ES1), and distal motor latency of the facial nerve (DML VII). Latencies were significantly prolo

Quantitative measures of sympathetic skin response in diabetes: relation to sudomotor and neurological function.

The sympathetic skin response (SSR) at the foot to a deep inspiration was measured in 68 randomly selected diabetic patients and 46 age matched normal subjects and compared with other quantitative measures of neurological and sudomotor function. SSR was obtained in all but three diabetic patients. The upper limit of normal for the onset latency was 2202 ms and the lower limit for the amplitude of the first wave 92 microV. Ten diabetic patients had measurable but prolonged latencies, and 11 had measurable but low amplitudes. There were no significant associations between latency, height, and ag

The ultrasonographic evaluation of hemodynamic changes in patients with diabetic polyneuropathy after tarsal tunnel decompression.

BACKGROUND: Nerve decompression procedures have shown to have promising roles in patients with diabetic polyneuropathy. It is known that not only nerves but also arteries pass through the same compressed anatomical tunnels. The aim of the study is to reveal whether the surgical decompression procedures have a positive effect on hemodynamic and morphological parameters of the arterial structures passing through these anatomic tunnels. METHODS: Twenty-seven patients who underwent posterior and anterior tarsal tunnel release procedures were retrospectively scanned for preoperative and 3 months p

Potential misdiagnoses of Bell's palsy in the emergency department.

STUDY OBJECTIVE: We evaluate the incidence of potentially incorrect emergency department (ED) diagnoses of Bell's palsy and identify factors associated with identification of a serious alternative diagnosis on follow-up. METHODS: We performed a retrospective cohort study from California's Office of Statewide Health Planning and Development for 2005 to 2011. Subjects were adult patients discharged from the ED with a diagnosis of Bell's palsy. Information related to demographics, imaging use, and comorbidities was collected. Our outcome was one of the following diagnoses made within 90 days of

Shoulder dystocia: the unpreventable obstetric emergency with empiric management guidelines.

OBJECTIVE: Much of our understanding and knowledge of shoulder dystocia has been blurred by inconsistent and scientific studies that are of limited scientific quality. In an evidence-based format, we sought to answer the following questions: (1) Is shoulder dystocia predictable? (2) Can shoulder dystocia be prevented? (3) When shoulder dystocia does occur, what maneuvers should be performed? and (4) What are the sequelae of shoulder dystocia? STUDY DESIGN: Electronic databases, including PUBMED and the Cochrane Database, were searched using the key word "shoulder dystocia." We also performed

Chronic pain. Clinical management of common causes of geriatric pain.

Effective management of chronic pain in older persons is attainable, although unnecessarily elusive. A host of factors can impede assessment and drug management, including impaired cognitive function, multiple potential causes of pain, pharmacokinetics and pharmacodynamics unique to the geriatric population, and clinician anxiety regarding analgesic addiction. In the geriatric population, some of the most prevalent chronic pain management cases involve osteoarthritis, low back pain, and neuropathy. Effective pain management is achieved with analgesics (acetaminophen and nonsteroidal anti-infla

Intercostal somatosensory-evoked potentials. A new technique.

Presently, there are few electrodiagnostic medicine techniques to evaluate lesions affecting the thoracic nerve roots or spinal cord. A new electrophysiologic technique to assess these structures, intercostal somatosensory-evoked potentials (SEPs), is described. Thirty neurologically normal subjects were used in this investigation to generate intercostal SEPs. Bilateral intercostal SEPs were easily elicited after stimulation of the third intercostal nerves just lateral to the sternum anteriorly. Intercostal SEPs were also easily elicited from the fifth, seventh and ninth intercostal nerves alo

Proposal of a new method for the use of the Rydel-Seiffer tuning fork in the screening of diabetic polyneuropathy. A pilot study.

We investigated the usefulness of a new examiner-independent method based on the duration of vibration sensation following the placement of the Rydel-Seiffer tuning fork over the dorsum of the interphalangeal hallux joint. This method demonstrated the same diagnostic efficacy as the Rydel-Seiffer method coupled with greater ease of use.

[Diagnostic validity of ultrasonography in carpal tunnel syndrome].

INTRODUCTION: Ultrasonography has emerged as an alternative tool for diagnosing peripheral neuropathies, such as carpal tunnel syndrome (CTS). Yet, data are still lacking as regards the diagnostic validity of the different ultrasonography measurements for detecting CTS in clinical settings. AIMS: To determine the diagnostic validity of ultrasound measurements of the cross-sectional area of the median nerve in the wrist (CSA-M) and of the ratio of the area of the median nerve between the wrist and the forearm (R-WF) in the diagnosis of CTS, using electroneuromyography (ENG) as the reference te

Deformity following fracture in diabetic neuropathic osteoarthropathy. Operative management of adults who have type-I diabetes.

We studied fifteen lower extremities that had a deformity following a fracture in the region of the ankle or the tarsal bones, in fourteen adults who had type-I diabetes and neuropathic osteoarthropathy. The skeletal deformities associated with these fractures were either diagnosed late (two limbs) or occurred in lower extremities that had been treated by means other than non-weight-bearing and immobilization (thirteen limbs). All deformities were severe and secondary to a non-union or malunion of the fracture; all were ultimately treated with operative reconstruction. Before the reconstructio

Electrophysiological findings including single fibre EMG in a family with mitochondrial myopathy.

Nerve conduction studies, conventional and quantitative concentric needle EMG and single fibre ENG were performed on 5 clinically affected and 7 clinically asymptomatic members of a family with a mitochondrial myopathy manifesting as a facioscapulohumeral syndrome. Abnormalities of nerve conduction present in 3 clinically affected cases were attributed to co-existent diabetes mellitus. Quantitative CNEMG showed a reduction of the mean motor unit potential duration and increased incidence of polyphasic potentials in all 5 clinically manifest cases consistent with a primary myopathic disorder. Glucagon-like peptide-1 receptor agonists and the risk of nonarteritic anterior ischemic optic neuropathy: Evidence from a global real-world cohort.

AIMS: To evaluate whether GLP-1 RA therapy is associated with an increased risk of NAION in a large, diverse real-world population. METHODS: This retrospective cohort study utilized the TriNetX U.S. Collaborative Network from 2015 to 2024. Adults (≥18 years) with type 2 diabetes mellitus (T2DM) were identified and grouped as GLP-1 RA users versus other antidiabetic users and GLP-1 RA users versus sodium-glucose cotransporter-2 inhibitors (SGLT-2 i) users. Patients with preexisting optic neuropathy or severe ocular disease were excluded. Propensity score matching (1:1) was applied to balance b

Cerebral venous sinus thrombosis secondary to nasopharyngeal abscess with Klebsiella pneumoniae infection in an elderly patient with multiple comorbidities: a case report and literature review.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is a rare but severe cerebrovascular disorder. While often associated with hypercoagulable states, infections, particularly those involving the head and neck region, can be a significant predisposing factor. Klebsiella pneumoniae is a known pathogen causing invasive syndromes, including liver abscesses and metastatic infections, but its association with nasopharyngeal abscess leading to CVST and cavernous sinus syndrome is exceptionally rare, especially in elderly patients with complex medical backgrounds. CASE PRESENTATION: We report the ca

Mechanism of alkaloid-based inhibition of aldose reductase: Computational perspectives and experimental validations.

Excessive aldose reductase activity drives the polyol-pathway damage that underlies diabetic cataract, neuropathy and nephropathy, yet few safe, potent AR inhibitors have reached the clinic. Here we integrated virtual screening, atomistic simulation and enzymology to evaluate six natural alkaloids-calycanthine, rutaecarpine, glaucine, sparteine, berbamine and tetrandrine-as prospective AR antagonists. A 2500-compound AutoDock Vina screen singled out these scaffolds for high predicted affinity (≤ - 7.0 kcal mol-1), chemotype diversity and favorable in silico developability. Docking located all
