Naloxone fails to antagonize initial hypoalgesic effect of a manual therapy treatment for lateral epicondylalgia.

BACKGROUND: Recent research has shown that Mulligan's Mobilization With Movement treatment technique for the elbow (MWM), a peripheral joint mobilization technique, produces a substantial and immediate pain relief in chronic lateral epicondylalgia (48% increase in pain-free grip strength).([1]) This hypoalgesic effect is far greater than that previously reported with spinal manual therapy treatments, prompting speculation that peripheral manual therapy treatments may differ in mechanism of action to spinal manual therapy techniques. Naloxone antagonism and tolerance studies, which employ widel

Repetitive strain injury. 2. Diagnostic and treatment tips on six common problems. The Goff Group.

Repetitive strain injury is caused by recurrent overuse, resulting in microtrauma to tissues. Local pain and tenderness, weakness, inflammation, and limited function are common findings. Some of the strain injuries seen most often are carpal tunnel syndrome, trigger finger, shoulder impingement syndrome, tennis elbow, thoracic outlet syndrome, and myofascial pain disorders. Often, treatment can be started at the initial visit, after systemic disorders have been ruled out. A vital step is elimination of aggravating factors, such as improper posture, inadequate attention to ergonomic factors at

Prevalence and imaging characteristics of the transverse trochlear ridge on elbow MRI and radiographs in youths.

OBJECTIVES: The transverse trochlear ridge (TTR) is an anatomic variant of the ulnar trochlear groove. This study evaluated the prevalence and imaging characteristics of the TTR on MRI and radiographs in children, adolescents, and young adults. MATERIALS AND METHODS: This retrospective analysis included 178 MRI examinations and 95 corresponding radiographs of patients aged 4-25 years. The presence or absence of the TTR was evaluated. The width, height, and depth of the TTR were measured. The sensitivity and specificity of radiography to detect TTRs were evaluated compared with MRI as the refe

Preoperative Proximal Migration of the Radial Head as an Independent Predictor of Suboptimal Outcomes After Osteochondral Autograft Transplantation for Capitellar Osteochondritis Dissecans: A Retrospective Cohort Study.

BACKGROUND: Osteochondral autograft transplantation (OAT) is widely performed for capitellar osteochondritis dissecans (OCD). However, preoperative predictors of suboptimal postoperative outcomes remain unclear. PURPOSE/HYPOTHESIS: The authors aimed to evaluate clinical outcomes after OAT for capitellar OCD and identify preoperative risk factors associated with suboptimal outcomes. They hypothesized that radiographic indicators of disease severity, including preoperative proximal migration of the radial head, lesion size, and lateral wall disruption, would be associated with suboptimal postop

Open elbow arthrolysis with hinged external fixation for postburn elbow stiffness: a case series with an averaged 4-year follow-up.

OBJECTIVE: Patients with severe burns may develop heterotopic ossification (HO), articular cartilage loss, and soft tissue contracture in the elbow, significantly limiting elbow function and diminishing quality of life. Open elbow arthrolysis (OEA) combined with hinged external fixation (HEF) has demonstrated efficacy in managing severe elbow stiffness. However, no recent studies have specifically investigated its application in postburn elbow stiffness. This study evaluates the clinical outcomes of OEA with HEF in patients with postburn elbow stiffness, with an average follow-up period of fou

A rare complication of tension band fixation of olecranon osteotomy: Distal migration of K-wire.

Tension band wiring (TBW) is one of the most commonly used fixation techniques to fix olecranon osteotomies. Hardware prominence has been the most commonly reported complication of TBW. However, distal migration of Kirschner (K)-wire after TBW fixation for olecranon osteotomy has not been reported. In this case report, we presented distal migration of K-wire detected nine months after initial surgery in a 46-year-old male patient. The patient was operated on for an intraarticular distal humerus fracture using an olecranon osteotomy. The osteotomy was fixed with TBW fixation. The patient missed

Wrist extension strength changes after extensor carpi radialis brevis release in the lateral epicondylitis.

PURPOSE: Lateral epicondylitis (LE) is often attributed to degenerative changes and microscopic ruptures in the extensor carpi radialis brevis (ECRB). Surgical procedures, such as arthroscopic or open ECRB release, are commonly used to address this condition. However, there is limited research on changes in wrist muscle strength and their clinical impact after ECRB release. This study aims to investigate the changes in wrist extension strength following ECRB release in patients with lateral epicondylitis. METHODS: A retrospective study was conducted involving thirty-six right-handed patients

Characterizing focal muscle signal on MRI in flexor-pronator muscles' status post ulnar collateral ligament reconstruction.

OBJECTIVE: Focal high signal is commonly seen about the flexor pronator mass on MRI in some patients after ulnar collateral ligament (UCL) reconstruction of the elbow. The etiology of this high signal is unclear and not described in literature. The hypothesis is that the edema we see on post-operative MRI is related to palmaris longus graft harvest rather than secondary to other causes of muscle edema such as denervation or muscle strain. METHODS AND MATERIALS: An IRB waiver-approved, retrospective search of our radiology database was conducted using the keywords "ucl," "elbow," and "reconstr

Management of nonunion of humeral medial condyle fracture: A case series and review of the literature.

Pediatric humeral medial condyle fracture (HMCF) is a rare condition and is difficult to detect, especially in young children. The management of late presentation of HMCF is challenging and lacks consensus. Herein, we reported four cases of HMCF nonunion received open reduction and internal fixation (ORIF) or supracondyle osteotomy from our institution. In addition, 12 cases of ORIF and 4 cases of osteotomy reported in the previous studies were also reviewed. The HMCF nonunion can heal after ORIF, but the indication and the optimal techniques need to be clarified. Supracondylar osteotomy alone

Correlation between modified trochleocapitellar index and post-traumatic elbow stiffness in type C2-3 distal humeral fractures among adults.

BACKGROUND: The purpose of this study was to propose the modified trochleocapitellar index (mTCI), assess its reliability, and evaluate its correlation with post-traumatic elbow stiffness in type C2-3 distal humeral fractures among adults. METHODS: From January 2013 to June 2017, a total of 141 patients with type C2-3 distal humeral fractures were included. The mTCI was calculated as the ratio between the modified trochlear and capitellar angles relative to the humeral axis (mTCI-HA), lateral humeral line (mTCI-LHL), and medial humeral line (mTCI-MHL) from anteroposterior radiographs taken im

Effect of enhanced recovery after surgery (ERAS) pathway on the postoperative outcomes of elbow arthrolysis: A randomized controlled trial.

BACKGROUND: Enhanced recovery after surgery (ERAS) protocols aim to improve perioperative outcomes and facilitate recovery for the patient through multimodal pathways. While implementation of ERAS has improved outcomes in numerous surgical specialties, benefits specific to elbow arthrolysis have not been investigated. The purpose of this study was to determine the effects of an evidence-based ERAS pathway on: (1) reducing pain intensity and postoperative complications compared to conventional care, and (2) improving range of motion (ROM) and function of the elbow after open arthrolysis. Long-term results of closed-wedge osteotomy of the lateral humeral condyle for osteochondritis dissecans of the capitellum.

BACKGROUND: Various surgical methods are used for osteochondritis dissecans of the capitellum; however, we have consistently performed a closed-wedge osteotomy of the lateral humeral condyle since 1983. The purpose of this study is to clarify the long-term results of closed-wedge osteotomy for osteochondritis dissecans of the capitellum. METHODS: Seventy-seven elbows with all lesion types of osteochondritis dissecans of the capitellum were treated with closed-wedge osteotomy. Unstable osteochondral fragments were fixed with a bone graft and bone pegs in combination with osteotomy. The mean ag

Elbow hemiarthroplasty using a "triceps-on" approach for the management of acute distal humeral fractures.

BACKGROUND: Total elbow arthroplasty is an established option for the primary treatment of acute distal humeral fractures, but there are sparse data regarding elbow hemiarthroplasty (EHA) as an alternative. We present the outcome of EHA performed with a modular anatomic prosthesis and a "triceps-on" surgical technique. METHODS: Eighteen consecutive patients underwent EHA for an acute fracture. Two patients died, leaving a study group of 16 patients with minimum 2-year follow-up. Clinical evaluation included range of motion; Mayo Elbow Performance Score; Quick Disabilities of the Arm, Shoulder

Early mobilisation versus plaster immobilisation of simple elbow dislocations: results of the FuncSiE multicentre randomised clinical trial.

BACKGROUND/AIM: To compare outcome of early mobilisation and plaster immobilisation in patients with a simple elbow dislocation. We hypothesised that early mobilisation would result in earlier functional recovery. METHODS: From August 2009 to September 2012, 100 adult patients with a simple elbow dislocation were enrolled in this multicentre randomised controlled trial. Patients were randomised to early mobilisation (n=48) or 3 weeks plaster immobilisation (n=52). Primary outcome measure was the Quick Disabilities of the Arm, Shoulder, and Hand (Quick-DASH) score.
