Complications of fluid therapy.

The intravenous administration of fluids is one of the most important aspects of patient care in hospitalized animals. Intravenous fluids are administered to replace or prevent dehydration, treat hypovolemic shock and intravascular volume depletion, correct acid-base and electrolyte abnormalities, and maintain vascular access for administration of drugs, blood product components, and parenteral nutrition. Intravenous catheterization also can provide a means of blood sample collection, thus avoiding frequent and uncomfortable venipunctures in critically ill animals. Although the benefits of int

Potassium additive algorithm for use in continuous renal replacement therapy.

The performance of continuous renal replacement therapy (CRRT) brings about alterations in the serum potassium levels in patient. Potassium is an electrolyte essential for the regulation of nerve conduction and muscle contraction, particularly important in the case of cardiac muscle. This article describes the physiological mechanisms that affect potassium distribution throughout the body and also describes the effects of hypokalaemia or hyperkalaemia on the heart. This article justifies the need for serum potassium control during CRRT, which recognizes and reconciles the differing areas of re

[Medical treatment of hypertension--what treatment for what patients?].

The antihypertensive agents of first choice include ACE-inhibitors, angiotensin receptor blockers, beta blockers, calcium antagonists and diuretic agents. For the selection of medicaments, the individual patient risk profile of decisive importance. In particular a metabolic syndrome, diabetes mellitus, disturbed renal function and/or a disturbed electrolyte household must be considered. For initial treatment monotherapy or a low-dose combination regime is suggested. If the response is inadequate, possible options include increasing the dose, changing the medicament, (sequential monotherapy) or

Perioperative care of the patient with nephropathic cystinosis.

Cystinosis is an autosomal recessive disease that results in a defective integral membrane protein responsible for the transport of cystine out of lysosomes. This results in the accumulation of cystine in the lysosomes of almost every tissue, with subsequent formation of damaging crystals. The cystinotic process may affect every system of the body, but is particularly damaging to the kidneys and eyes. We discuss the perioperative care of a cystinotic patient with renal insufficiency, Fanconi's syndrome, and photophobia. Other organ systems affected by the cystinotic process are also discussed,

Formulation of amphotericin B as nanosuspension for oral administration.

Amphotherin B was formulated in a nanosuspension as a new oral drug delivery system for the treatment of experimental visceral leishmaniasis. Amphotericin B (AmB) nanosuspensions were produced by high pressure homogenisation obtaining particles with a PCS diameter of 528 nm. Environmental stability was determined in artificial gastrointestinal fluids at different pH and electrolyte concentrations. In vivo efficacy was determined in a mouse model of visceral leishmaniasis. Following oral administration (5 mg kg(-1)), micronised amphotericin B did not show any curative effect. However, administr

Two-stage management of mega occipito-encephalocele.

We report three cases of large occipito-encephaloceles that were managed in the Neurosciences Unit, Hospital Universiti Sains Malaysia over the last 5 years. All patients had pre-operative MRI (magnetic resonance imaging) evolution and mapping of the sagittal sinus tract. The cerebrospinal fluid (CSF) of all three patients were initially diverted by means of ventricular shunt two weeks prior to shunt removal. The slow drainage of CSF prevented electrolyte and volume disturbances due to sudden decompression during their definitive occipital encephalocele operations. After 3 years follow-up, all

Nephrotic syndrome and interstitial nephritis associated with celecoxib.

Nonsteroidal anti-inflammatory drugs (NSAIDs) are well known to cause fluid and electrolyte abnormalities and renal failure. NSAIDs also may cause an acute allergic interstitial nephritis (AIN) and the nephrotic syndrome, characterized by histologic pathology consistent with minimal change disease in patients with previously normal renal function. The nephrotoxic potential of cyclooxygenase 2 (COX-2) inhibitors has not been established because AIN associated with nephrotic syndrome has not been reported secondary to the COX-2 inhibitors. This case report describes the first case of AIN associa

[Effect of furosemide on the intrarenal hemodynamics and excretory renal function depending on salt regime].

The experiments on rats showed that the dietary mineral composition influences renal circulation and saluretic function and modifies the related effects of furosemide. In particular, a high NaCl intake reduces the blood perfusion in the renal cortex and increased that in the renal medulla, thus increasing sodium excretion with urine. The use of hyposol, a table salt substituent, enhances the blood flow in the renal medulla and leads to a significant increase in the diuresis, natriuresis, and kaliuresis. Furosemide (10 mg/kg) sharply increases the medullar circulation and electrolyte excretion

Application of heparin to chiral separations of antihistamines by capillary electrophoresis.

A study of the chiral separations of antihistamines, including pheniramine, chlorpheniramine, brompheniramine, carbinoxamine and doxylamine in capillary electrophoresis (CE) was accomplished using heparin as a chiral additive (CA) and phosphate buffer as the background electrolyte. Several factors were shown to affect both the selectivity and the migration time, including concentration of heparin, concentration of buffer, and the pH. A dual mechanism involving both inclusion complexation and ionic interactions with heparin is thought to be responsible for the chiral recognition. In the pH rang

[Severe massive pseudomembranous colitis with a fulminant course].

Pseudomembranous colitis is an inflammatory disease of rectal and colonic mucosa caused by Clostridium difficile produced toxin. The inflammation is produced as the consequence of a non-specific response to several agents. It usually presents with abdominal pain and mild watery diarrhea which used to decrease when removing the antibiotic or when starting the therapy with metronidazole or vancomycin. In aged patients, with severe concomitant diseases, may appear complications such as dehydration, electrolyte imbalance, hypotension and toxic megacolon, which occasionally may lead to fatal outcom

Use of 4-chloro-7-nitrobenzofurazane as a derivative reagent for the determination of amino acids by cathodic stripping square wave voltammetry in biological fluids.

The rapid and simple reaction of 4-chloro-7-nitrobenzofurazane with amino acids allows the determination of amino acids in urine using cathodic stripping square wave voltammetry. The obtained compounds are adsorbed on a hanging mercury drop electrode for determination by voltammetric methods using phosphate buffer at pH 2.0 as supporting electrolyte. The proposed method allows determination with an error of 5.68%. A limit of detection (3sigma) of 3.64 nM (0.766 ng ml(-1)) and a limit of determination (10sigma) of 12.12 nM (2.55 ng ml(-1)) are obtained for arginine determination. The proposed m

Endosaccular treatment of inoperable intracranial aneurysms with platinum coils.

Endosaccular packing of inoperable aneurysms with electrolytic platinum coils was performed in 50 patients. Complete embolization of the aneurysms was achieved in 100% of small, 95% of large and 85% of giant aneurysms, with combined procedural and periprocedural morbidity and mortality rates of 6 and 4%, respectively. Follow-up angiography of 42 aneurysms demonstrated some degree of refilling in 17% of small, 19% of large and 50% of giant aneurysms. The risks associated with the procedure were similar in aneurysms of the carotid and vertebrobasilar circulations. This treatment should therefore

Effects of gentamicin on rat submandibulary gland functions.

1. Submandibular saliva was collected from anesthetized gentamicin-treated and control rats using carbachol, isoproterenol and pilocarpine as secretagogue. 2. Intraperitoneal injection of a large single dose (80 mg/kg) of gentamicin caused marked changes in saliva flow rate, protein and electrolyte concentrations in the presence of parasympathetic or sympathetic agents used as stimulants. 3. The secretion of N-acetyl-beta-D-glucosaminidase in saliva was determined and there was a marked increase in the enzyme activity of saliva of gentamicin-treated rats in comparison to that of controls. 4. [Epileptogenic complications of neurosurgical interventions].

Epileptogenic complications after supratentorial intracranial surgery are, strictly defined, the events which promote a further complication, i.e. seizures in any form, partial, simple or complex, or generalized. The most common epileptogenic complications are, therefore, electrolyte imbalance, hypoxia, hypoglycemia, hypocapnia, and intracerebral or extradural hematoma. However, also inadequate anticonvulsant prophylaxis and use of convulsant drugs can yield to the onset of post-operative seizures. Anesthetic drugs have been each time defined pro or anti-convulsant: however, data from the lite

Rhabdomyolysis accompanying thyroid crisis: an autopsy case report.

Rhabdomyolysis is occasionally associated with metabolic disorders such as diabetic coma, severe electrolyte disturbances and myxedema coma. We describe rhabdomyolysis accompanying thyroid crisis. A 50-year-old man with Graves' disease developed rhabdomyolysis, congestive heart failure and hepatic failure during the course of thyroid crisis and then died of acute renal failure. Postmortem examination revealed rhabdomyolysis in the cardiac and psoas muscles, old myocardial infarction, hepatic centrilobular necrosis, renal cortical necrosis, and follicular hyperplasia in the thyroid. Circulatory

The assessment of dehydration in congenital pyloric stenosis.

In a retrospective review of the records of 88 infants who underwent a Ramstedt pyloromyotomy for pyloric stenosis, biochemical and clinical measures were used to determine the most useful guide to the degree of dehydration on admission. The only measure which correlated to the dehydration was the clinical assessment (r = 0.62). Apart from indicating the presence of alkalosis and hypochloraemia, serum chloride and serum bicarbonate levels were not useful measures. While clinical measures show only a degree of linear correlation with dehydration, they do present the most useful baseline to judg

Effects of PGE1 alone and on sympathetically mediated Na, K transport in the perfused main submandibular duct of rat.

PGE1 was administered in 5 micrograms/kg doses every 5 min over a period of 70 min and Na and K transport in the perfused main duct of rat submandibular gland was examined during a period without stimulation of the sympathetic nerve as well as during stimulation of the sympathetic nerve. A 25% decrease in the amount of Na absorbed and a 42% decrease in the amount of K secreted occurred when PGE1 alone was administered; the same change occurred when the sympathetic nerve was stimulated in the presence of PGE1. These data show, for the first time, an effect of PGE1 alone on electrolyte transport

Reverse pinocytosis induced in cerebral endothelial cells by injection of histamine into the cerebral ventricle.

Histamine dihydrochloride (10 micrograms of 500 micrograms/ml) was infused during 1 min into the lateral cerebral ventricle of rats, which resulted in a significant stimulation of pinocytosis in the endothelial cells. Systemic injections of mepyramine or metiamide could not prevent this activation. In contrast, ranitidine, injected with histamine was able to inhibit the stimulation of pinocytosis. Albumin exudation from the blood was not found. There was also no change in water and electrolyte contents of the brain tissue. The results suggest that histamine reaching the abluminal membrane can

Polydipsia in chronic psychiatric patients. Body weight and plasma sodium.

Eight chronic psychiatric in-patients with polydipsia, and polyuria, up to 22 litres per 24 hours, were studied by frequent timed body weight and urine volume measurements, and episodic plasma electrolyte estimations. During the day they all showed irregular water retention, with hyponatremia in proportion to the weight gain. During the night they always lost water and weight, returning to their individual lowest weights and to normal plasma sodium. Measurement of weight in chronic psychiatric patients can be used to identify patients with significant polydipsia, to monitor those with the diso

Acute renal failure.

ARF is a common clinical problem associated with significant morbidity and mortality. Therefore, rapid and accurate diagnosis is imperative. Initial diagnostic strategies should be directed toward distinguishing among prerenal, renal, and postrenal causes of ARF. This can be done by the use of urinary diagnostic indices in concert with radiologic investigation, renal biopsy, and hemodynamic monitoring when appropriate. Treatment of established ARF should be directed toward maintaining fluid and electrolyte homeostasis and controlling complications resulting from retained nitrogenous waste prod
