Hirudin is the most potent and specific known inhibitor of thrombin, the enzyme that plays a key regulatory function in hemostasis and blood coagulation. The importance of thrombosis in cardiovascular disease has recently highlighted the limitations of existing antithrombotic drugs and the potential value of direct thrombin inhibition as an effective approach to antithrombotic therapy. Hirudin and a small peptidomimetic analog--hirulog--are being developed as alternatives to heparin for the treatment of unstable angina, for prevention of abrupt closure and restenosis following coronary angiopl

In 34 patients with non-A non-B, 28 with type B and 11 with autoimmune chronic hepatitis, anti-neutrophil antibodies were investigated using indirect immunofluorescence and anti-myeloperoxidase antibodies by enzyme-linked immunosorbent assay. Granulocyte-specific antinuclear antibodies, were detected in 14 patients with advanced stages of non-A, non-B hepatitis (41%). Their presence correlated with histological features of disease activity but not with response to interferon therapy. Within 24 h after the first dose of interferon, 9 of these became negative and 3 more became negative after 1,

Studies were undertaken to evaluate the role of interleukin 6 (IL-6) in the pathogenesis of chronic hepatitis B. Using recently developed sensitive and specific enzyme-linked immunosorbent assay, we determined serum levels of IL-6 in patients with chronic active hepatitis B (B-CAH), chronic persistent hepatitis B (B-CPH), and primary biliary cirrhosis (PBC). Serum IL-6 activity tended to increase in patients with B-CAH (38.4 +/- 68.0 pg/ml) (+/- SD), compared with controls (9.7 +/- 6.8 pg/ml), although the difference was not significant because of the wide scatter in values. IL-6 levels in B-C

The benefits of treating hypertension have been documented by several long-term studies that have shown a decreased incidence of morbidity and mortality associated with stroke, left ventricular failure, and renal insufficiency. With the large number of antihypertensive drugs currently available, several safety factors need to be considered when initially choosing a regimen so as not to adversely influence the potential benefits of blood pressure control. Antihypertensive agents should be chosen based on their hemodynamic profile, the absence of adverse metabolic effects and subjective side eff

Chemiluminescence is a fascinating phenomenon that evolved in nature and has been harnessed by chemists in diverse ways to improve life. This Account tells the story of our research group's efforts to formulate and manifest spiroadamantane 1,2-dioxetanes with triggerable chemiluminescence for imaging and monitoring important reactive analytes in living cells, animals, and human clinical samples. Analytes like reactive sulfur, oxygen and nitrogen species, as well as pH and hypoxia can be indicators of cellular function or dysfunction and are often implicated in the causes and effects of disease

Carbonic anhydrases are ubiquitous metalloenzymes which catalyze the CO2 hydration to bicarbonate and proton. β-Carbonic Anhydrases from fungi, such as Saccharomyces cerevisiae, Candida spp. and Cryptococcus neoformans, have been widely investigated as potential targets for antifungal therapies. In this chapter, we provide a comprehensive overview on their properties highlighting their role as CO₂-sensing enzymes. We survey functional, biochemical, structural, and kinetic features, summarize inhibition and activation studies, and review in vitro experiments. Taken together, these data undersco

Radiation therapy (RT) is a mainstay of treatment for several sarcomas. Radiation-induced fibrosis (RIF) is a common side effect of RT with an incidence of approximately 80%. RIF is caused by excessive proliferation of myofibroblasts, which produce collagen and extracellular matrix components, coupled with a reduction of remodeling enzymes. Targeting this pathway with pravastatin to prevent and ameliorate RIF has been shown to improve outcomes in patients with head and neck cancer. We share our single-center experience using pravastatin to treat RIF involving the joints in children with sarcom

INTRODUCTION: Heterogeneity of patients with late-life depression (LLD) makes it difficult to predict their responses to treatment.
METHODS: Hundred patients with LLD were stratified into clusters based on ten blood biochemical and immunological parameters measured at baseline. The treatment responses and clinical characteristics of the patients in the resulting clusters were compared.
RESULTS: The resulting 4 clusters of patients significantly differed in treatment responses and in a number of clinical traits.
CONCLUSION: The study confirms the link between the activities of enzymes involv

Phosphoglycerate kinase 1 (PGK1) is a highly conserved enzyme that catalyzes the initial ATP-producing step in glycolysis. Improving cellular energy production by increasing PGK1 activity may be beneficial in multiple neurological conditions where cell metabolism is dysregulated, including Parkinson's disease (PD) and motor neuron disease (MND). This review examines recent evidence that suggests increasing PGK1 activity may be beneficial in multiple neurological conditions and discusses the current challenges surrounding the development of PGK1-focused therapies. PGK1 has considerable therapeu

Aminoacyl-tRNA synthetases (aaRSs) are essential enzymes to support protein synthesis in all organisms. Recent studies, empowered by advancements in genome sequencing, have uncovered an increasing number of disease-causing mutations in aaRSs. Monoallelic aaRS mutations typically lead to dominant peripheral neuropathies such as Charcot-Marie-Tooth (CMT) disease, whereas biallelic aaRS mutations often impair the central nervous system (CNS) and cause neurodevelopmental disorders. Here, we review recent progress in the disease onsets, molecular basis, and potential therapies for diseases caused b

Inflammatory pseudotumors (IPTs) of the liver can mimic malignant lesions. As the name implies, they are usually associated with an inflammatory process and usually regress with the treatment of the underlying pathology. We report a case of a 67-year-old female who presented with right upper quadrant pain, deranged liver enzymes, elevated tumor markers [alpha-fetoprotein (AFP) and CA 19-9], and a large liver mass on imaging, suspected to be hepatocellular carcinoma (HCC). She was eventually diagnosed with IPT complicating the liver inflammation due to autoimmune hepatitis (AIH). She responded

A 28-year-old woman was referred to our hospital for liver dysfunction and neck pain. Blood tests revealed elevated liver enzymes and eosinophilia. Ultrasonography, computed tomography, and magnetic resonance imaging showed a mass lesion near the hepatic hilus. The tumor was considered to be an inflammatory pseudo-tumor or malignancy. A liver-mass biopsy was performed and led to a diagnosis of inflammatory pseudo-tumor. In the present case, a markedly elevated eosinophil count was a characteristic clinical feature, and the patient underwent steroid therapy. Treatment resulted in a reduced eosi

We present the case of an 86-year-old African American gentleman who presented with fatigue, diarrhea, and weight loss. He had elevated liver enzymes in an obstructive pattern. A magnetic resonance cholangiopancreatography scan showed edema around the stomach and duodenum, which prompted evaluation with an esophagogastroduodenoscopy. A large enteric fistula between the duodenum and colon was visible, and biopsies returned positive for cytomegalovirus (CMV). The patient did not have any known risk factors for immunodeficiency and was successfully treated with medical therapy. Our case is unique

The cytochrome P450 is a superfamily of isoenzymes that are responsible for the metabolism of many drugs. Significant changes in pharmacokinetics and drug interactions may be due to induction of hepatic cytochrome P450 enzymes. Rifampicin is a common inducer of CYP3A4. We report a case of a 57-year-old woman who was suspected for endocarditis and therefore treated with rifampicin. Due to previous mechanical aortic valve replacement, she also received phenprocoumon for anticoagulation. Although continuing anticoagulant therapy, antibiotic coadministration led to normal international normalised

Mucopolysaccharidosis (MPS) are a group of lysosomal storage disorders that are caused by the deficiency of enzymes involving in the catabolism of glycosaminoglycan (GAGs). GAGs incompletely degraded accumulate in many sites, damaging tissues and cells, leading to a variety of clinical manifestations. Many of these manifestations are painful, but few data are available in the literature concerning the prevalence, etiology, and pathogenesis of pain in children with MPS. This review, through the analysis of the data available the in literature, underscores the relevant prevalence of pain in MPSs

Pyruvate kinase (PK) deficiency is a rare genetic disease that causes chronic hemolytic anemia. There are currently no targeted therapies for PK deficiency. Here, we describe the identification and characterization of AG-348, an allosteric activator of PK that is currently in clinical trials for the treatment of PK deficiency. We demonstrate that AG-348 can increase the activity of wild-type and mutant PK enzymes in biochemical assays and in patient red blood cells treated ex vivo. These data illustrate the potential for AG-348 to restore the glycolytic pathway activity in patients with PK def

Myopathy is a known complication of hypothyroidism, commonly characterized by an elevation in Creatine Kinase (CPK) due to increase capillary permeability proportional to the hypothyroid state. Thyroid hormone is important for the expression of fast myofibrillar proteins in the muscle. In hypothyroidism the expression of these proteins are deficient and there is an increase accumulation of slow myofibrillar proteins. A rapid or abrupt descend in thyroid hormones caused by radioiodine therapy after prolonged hyperthyroidism can lead to local hypothyroid state within the muscle tissue, resulting

A 45-year-old woman visited our hospital due to upper left quadrant pain and melena. Colonoscopy revealed longitudinal ulcers in the transverse colon. The endoscopic findings and pathological examination of a biopsy specimen led to diagnosis of Crohn disease, and mesalazine was administered. Although the colorectal lesions showed improvement with mesalazine, a blood test revealed elevation of biliary enzymes. Endoscopic retrograde cholangiopancreatography showed diffuse narrowing of the main pancreatic duct and smooth stricture of the distal bile duct. Steroid therapy improved the pancreatic l

Tyrosine kinases (TKs) are enzymes that catalyze the phosphorylation of tyrosine residues on protein substrates. They are key components of signaling pathways that drive an array of cellular responses including proliferation, differentiation, migration, and survival. Specific TKs have recently been identified as critical to the pathogenesis of several autoimmune and inflammatory diseases. Small-molecule inhibitors of TKs are emerging as a novel class of therapy that may provide benefit in certain patient subsets. In this review, we highlight TK signaling implicated in inflammatory dermatologic
