EEG-X: An Integrated Framework for Automated Quantitative EEG Analysis in Epilepsy Diagnosis.

Accurate interpretation of electroencephalography (EEG) remains a major challenge in epilepsy diagnosis, particularly given that patient heterogeneity hinders the cross-subject generalization of artificial intelligence based algorithms. To address the complex spatio-temporal characteristics and long-range contextual dependencies of EEG signals, we propose a novel EEG decoding frame work. The framework employs a graph convolutional neural network to capture the spatial dependencies across EEG channels, integrated with a multiresolution transformer block to capture short-term dynamics while pres

Etiology of active epilepsy in adults: A Latin American multicenter hospital-based study.

PURPOSE: Etiologic patterns of epilepsy in low- and middle-income countries (LMICs) remain insufficiently characterized. We aimed to describe the causes and clinical features of active epilepsy in adults from seven Latin American (LA) countries using the 2017 ILAE etiologic classification. METHODS: We performed a retrospective multicenter case series based on medical records from 12 tertiary hospitals in Argentina, Brazil, Chile, Colombia, Ecuador, Mexico, and Uruguay (2019-2023). Adults (≥16 years) with active epilepsy, defined as at least one seizure in the past five years or ongoing antise

Seizure and developmental outcomes following epilepsy surgery for children with Sturge-Weber syndrome.

OBJECTIVE: Sturge-Weber syndrome (SWS) is a rare neurocutaneous disorder. Up to 90% of children have epilepsy. Epilepsy surgery is a treatment option. However, the seizure and neurodevelopmental outcomes following this are not well defined. This study aimed to determine the outcomes for children undergoing epilepsy surgery for SWS in relation to seizure control, cognition, language, motor function and social communication. METHODS: A retrospective case-note review was completed for children with SWS undergoing epilepsy surgery at Great Ormond Street Hospital from 1993 to 2022. Results of stan

Parameter-efficient convolutional neural network for drug treatment outcome studies of pediatric epilepsy.

This research aimed to develop and validate a convolutional neural network leveraging T2-weighted (T2W) and FLAIR MRI images to predict pharmacological treatment outcomes for pediatric epilepsy in tuberous sclerosis complex (TSC) patients. Using the EfficientNet3D-B0 architecture, we developed Efficient Tuberous Sclerosis Complex-Net (eTSC-Net), a weighted ensemble network that trains binary classification models with T2W and T2FLAIR images to differentiate between controlled and non-controlled TSC patients based on one-year post-anti-seizure medication (ASM) outcomes. Of the 95 patients, 39 (

Mechanistically informed circulating biomarkers are associated with acquired epilepsy after neonatal brain injury.

BACKGROUND: Acute provoked neonatal seizures are a major risk factor for acquired epilepsy, yet clinicians lack reliable tools to identify neonates at highest risk. Preclinical data implicate innate immune activation and neuronal injury as key drivers of epileptogenesis, suggesting blood-based biomarkers could provide mechanistic insight and prognostic utility. We sought to identify biomarkers of epileptogenesis in neonates with acute provoked seizures after brain injury using multicenter cohorts. METHODS: We conducted a prospective, multi-cohort analysis across two independent studies. NSR-R

Early predictors for later epilepsy after therapeutic hypothermia for neonatal hypoxic-ischemic encephalopathy.

BACKGROUND AND OBJECTIVE: Therapeutic hypothermia (TH) is the standard of care for neonatal hypoxic-ischemic encephalopathy (HIE), yet the risk of neonatal epilepsy remains a significant burden for families and clinicians. We aimed to observe and describe clinical, electrographic, and neuroimaging factors associated with the development of epilepsy by age 2 years in neonates with HIE treated with TH. METHODS: We conducted a retrospective observational study of neonates (≥36 weeks' gestational age) with moderate-to-severe HIE. Neonatal evaluations included clinical staging, continuous aEEG dur

Artificial intelligence in epilepsy diagnosis: Clinical readiness, failure modes, and standards for Implementation.

Epilepsy diagnosis is not a single classification task but a sequence of clinically consequential decisions: whether a paroxysmal event is epileptic, whether the patient meets the definition of epilepsy, how seizures and epilepsy should be classified, and what etiology or epileptogenic network explains the disorder. Artificial intelligence (AI) can support these decisions through natural language processing, conversational systems, computer vision, wearable sensors, electroencephalography (EEG), magnetic resonance imaging (MRI), genetics, and multimodal models. The evidence is uneven. Automate

Multi-glial inhibitory plasticity in epilepsy and sleep: implications for post-traumatic epileptogenesis.

Traumatic brain injury (TBI) is a leading cause of acquired epilepsy, and the latent period before the first spontaneous seizure offers a window for anti-epileptogenic intervention. Glial cells are active participants in the pathophysiology of epilepsy and play a role beyond the classical supportive roles and in the control of inhibitory networks. Cumulatively, astrocytes, microglia and oligodendrocytes contribute to the GABAergic signal transduction via orchestrated pathways comprising neurotransmitter uptake, cytokine-mediated inflammation and myelin remodeling. GABA transporters (GAT-1, GAT

Volumetric Analysis of Thalamic Nuclei in Epilepsy Patients: A Sex-Based Comparative Study.

Background/Objectives: Epilepsy is a long-term condition marked by recurring seizures that greatly affect daily life. The thalamus, with its extensive connections to the cortex, plays a vital role in epileptic processes. Changes in the volume of the thalamic nuclei are believed to occur in epilepsy. Our study aimed to examine the volumetric characteristics of thalamic nuclei in individuals diagnosed with generalized epilepsy and to explore the potential role of changes in these nuclei in the process of epileptogenesis. Methods: In this retrospective study, a total of 232 participants were incl

Epilepsy as a Multiscale Network Disorder: Integrating Precision Therapeutics and Emerging Experimental Platforms.

Background/Objectives: Epilepsy remains a major neurological disorder, with approximately one-third of patients continuing to experience pharmacoresistant seizures despite the availability of numerous antiseizure medications (ASMs). While current therapies primarily target neuronal hyperexcitability through modulation of ion channels and neurotransmitter systems, increasing evidence suggests that epileptogenesis arises from multiscale interactions involving molecular, cellular, circuit, network, neuroinflammatory, and neurovascular mechanisms. Although therapeutic strategies have diversified,

Facial emotion recognition in temporal lobe epilepsy: Behavioral performance, reaction time and alexithymia.

OBJECTIVE: Facial emotion recognition (FER) deficits are well documented in temporal lobe epilepsy (TLE), but most studies have focused on recognition accuracy, with limited attention to reaction time (RT) and its relationship with alexithymia. This study aimed to compare FER accuracy, RT, and alexithymia levels between patients with TLE and healthy controls (HCs) and to examine associations between FER performance and alexithymia. METHODS: Twenty-two patients with unilateral TLE and 30 age-, sex-, and education-matched HCs were included. FER accuracy and RTs were assessed using a standardize

Beyond seizures: eye tracking evidence of cognitive dysfunction in epilepsy - A systematic review.

BACKGROUND: Cognitive dysfunction is a common and disabling feature of epilepsy, affecting attention, memory, executive function, and socioemotional processing. Conventional neuropsychological assessments may lack sensitivity to subtle deficits and are influenced by educational and linguistic factors. Eye tracking provides high-temporal-resolution quantification of oculomotor behavior linked to distributed cognitive networks; however, its relationship to traditional neuropsychological performance in epilepsy has not been systematically synthesized. METHODS: This systematic review followed PRI

Language network reorganization in temporal lobe epilepsy: insights from task-based functional MRI.

OBJECTIVE: In temporal lobe epilepsy (TLE), recurrent seizures can cause structural and functional disruptions within the temporal lobe and nearby language regions, resulting in neural reorganization. In this study, we leverage task-based functional magnetic resonance imaging (tb-fMRI) to understand how this language reorganization process varies based on the seizure location, the timing of seizure onset, and the chronicity of epileptic activity. METHOD: 84 drug-resistant TLE patients who completed a tb-fMRI sentence completion task were included in this study. Analysis included group compari

Small sharp spikes precede the development of epileptiform discharges in temporal lobe epilepsy.

Small sharp spikes (SSS), traditionally regarded as benign EEG variants, have increasingly been linked to temporal lobe epileptogenicity. However, their potential role as early EEG biomarkers of temporal lobe epilepsy (TLE) remains unclear. We analyzed 11 patients with or without a prior epilepsy diagnosis whose initial EEGs demonstrated isolated SSS in the absence of interictal epileptiform discharges (IEDs). The cohort included 8 females and 3 males with a median age of 35 years (IQR 26-43). Four patients had a diagnosis of epilepsy at presentation, whereas seven did not. Over a median follo

Association between disconnected networks and surgical outcome in drug-resistant temporal lobe epilepsy.

PURPOSE: To characterize alterations in structural network connection strength (SNCS), functional network connection strength (FNCS), and structural-functional correlation (SFC) of preoperative disconnected networks in temporal lobe epilepsy (TLE) and the association of these measures with postoperative seizure outcomes. METHODS: This retrospective cohort study enrolled 39 TLE patients with 29 healthy controls (HCs). Patients were divided into seizure-free (SF) (Engel Ia) and non-seizure-free (NSF) (Engel Ib-IV) groups. Each network was partitioned into predefined resection-anchored tiers, in

Association between Interictal Spike Rate and Seizure Frequency in a Large Epilepsy Cohort.

BACKGROUND AND OBJECTIVES: Tracking and predicting seizure frequency in patients with epilepsy is important for prognostication and therapy management. Interictal spikes have been proposed as a biomarker of seizure burden, but their association with seizure frequency has not been well quantified across epilepsy subtypes. Our objective was to measure the association between spike rate and seizure frequency and how this varies by epilepsy subtype. METHODS: We studied 3,614 consecutive routine outpatient EEGs from 3,245 patients with epilepsy followed for a median of 2.8 years. A validated autom

Cognitive outcomes following surgical resection in tumoral temporal lobe epilepsy: a scoping review.

BACKGROUND AND AIMS: Surgical strategy in tumoral temporal lobe epilepsy (TLE), remains controversial in the literature given the complex balance between seizure cessation and cognitive function post-operatively. Strategies range from lesionectomy to more extensive resections. This review investigates the effects of temporal lobe tumor resection strategies on cognitive function outcomes. METHODS: A comprehensive literature review was conducted using PubMed, Embase, Cochrane, and Web of Science. Search terms encompassed seizure-related conditions (e.g., epilepsy), surgical approaches (e.g., ag

Memory performance, hippocampal volumes and memory fMRI in frontal versus temporal lobe epilepsy.

PURPOSE: Patients with temporal lobe epilepsy (TLE) and patients with frontal lobe epilepsy (FLE) show an increased risk of memory impairment relative to healthy controls, alongside structural and functional encoding-related cerebral alterations. To better specify syndrome-related differences, we directly compared TLE and FLE patients. METHODS: We evaluated 56 TLE (25 right-sided) and 24 FLE patients with respect to fMRI activation during encoding of scenes, faces, and words, post-scan recognition accuracy, neuropsychological verbal and figural memory tests, and hippocampal volumes. RESULTS:

Neural correlates of emotion processing and regulation dissociate frontal and temporal lobe epilepsy.

When confronted with negative events, regulating emotions is essential for self-management and mental health. Temporal lobe epilepsy (TLE) patients, particularly right TLE (rTLE) patients, show altered responses towards emotional stimuli. However, neural correlates of emotion regulation and its interplay with stimulus-based emotion processing have not been studied in TLE so far. Therefore, we compared functional MRI activity during up- and downregulation of emotions towards negative scenes among 21 left TLE and 18 rTLE patients, 18 frontal lobe epilepsy (FLE) patients and 17 healthy controls.
