Articles that update the state of knowledge regarding osteoporosis run the risk of quickly becoming obsolete because research and studies on osteoporosis today are arousing great interest among researchers, the pharmaceutical and medical equipment industries, governments and even WHO. All orthopedists know about osteoporosis because of its most deleterious effect: osteoporotic fracture. Osteoporosis without fractures does not arouse suspicion because this is a pathological condition with a nonspecific clinical profile. Osteoporotic fractures have an economic cost (from treatment), a social cos

The GH/IGF-I axis has essential roles in regulating bone and vascular status. The age-related decrease in GH secretion ("somatopause") may contribute to osteoporosis and atherosclerosis, commonly observed in the elderly. Adult-onset GH deficiency (GHD) has been reported to be associated with reduced bone mineral density (BMD), increased risk of fractures, and premature atherosclerosis. We have shown the young adult individuals with isolated GHD (IGHD) due to a homozygous for the c.57+1G>A GHRH receptor gene mutation have normal volumetric BMD (vBMD), and not develop premature atherosclerosis,

AIM: The aim of the study was to evaluate the application of a locked internal fixator in complex fractures of the proximal femur, in which the internal fixation with standard implants was not possible due to poor quality of bone or already failed internal fixation in the past.
METHOD: Ten patients suffering from a pertrochanteric (n = 5), periprosthetic (n = 1) or subtrochanteric (n = 4) femural fracture between 2003 and 2008 were prospectively registered, underwent open reduction and internal fixation with an "upside-down" femur LISS (less invasive stabilisation system) and were followed up

The majority of nasal fractures have been managed by using closed reduction and intranasal packing. In comminuted nasal fractures, open reduction and internal fixation may be indicated for accurate reduction and rigid fixation, but it is a very aggressive procedure. We developed a new technique for comminuted nasal fractures: indirect open reduction and intranasal Kirschner wire splinting. A periosteal elevator is used to elevate the mucoperiosteum posterior to the nasal bone through intercartilaginous incision and to reduce accurately the nasal bone, at the same time detecting the fracture li

Vitamin D and calcium are indispensable for the maintenance of bone mass and integrity. Active vitamin D3 has been most widely used in Japan for the treatment of osteoporosis. These drugs mildly increase bone mineral density, but it has little consistent effects on bone metabolic markers. Although weak, there is also some clinical evidence for their efficacy as a fracture-preventing drug. Recent reports suggest that vitamin D3 not only enhances intestinal calcium absorption but may also improve neuromuscular function to reduce the number of falls, thereby contributing to fracture prevention. Osteoporosis is a well-known complication of paralytic diseases. We investigated 11 patients with chronic spinal cord injury who have been paralyzed for 20 years or more. History of fracture was searched in these 11 patients. Mineral density in the lower limbs was measured, and the bone metabolism was examined. Of the total of 8 fractures, 87.5% occurred in the relatively low muscle tonus group. Patients showed decreased bone mineral density of the femoral neck, and this was negatively correlated with period of paralysis. One female patient showed an abnormal rise in bone absorption markers. Since the introduction of automotive airbag technology, patterns of airbag-induced injuries have emerged. Various types of upper-extremity trauma including fractures have been described. Previous reports have focused on the location of the forearm before deployment as the major determinant of fracture. We describe a case of forearm fracture resulting from an airbag deployment in a lupus patient with documented osteoporosis. Bone strength has recently been determined in laboratory testing to be an important factor determining airbag-induced fracture risk. An airbag-induced forearm fracture in a

OBJECTIVE: To evaluate the ability of self-reported risk factors to identify postmenopausal women likely to have extant vertebral fractures because approximately two thirds of women with radiographic evidence of vertebral fracture are unaware of the fracture.
PATIENTS AND METHODS: Questionnaire and spinal radiographic data were collected from postmenopausal women with a femoral neck bone mineral density T score of -1.6 or lower during screening for the Fracture Intervention Trial. Logistic regression was used to identify risk factors for extant vertebral fractures and to derive a final multiv

In a 3-year study with 2.5 mg of ibandronate daily in postmenopausal women with at least 1 pre-existing vertebral fracture, the drug reduced new vertebral fractures by 50%, but had no effect on non-vertebral fractures. The 150-mg once-monthly formulation of ibandronate was as effective as the daily one in increasing bone mineral density and decreasing bone turnover markers. It appears to be similar to alendronate and risedronate in efficacy and tolerability, but head-to-head comparisons are lacking.

Hypercalcemia and osteolytic bone lesion are important complications in the prognosis of patients with adult T cell leukemia/lymphoma (ATL). We report a 61-year-old Japanese woman who died of ATL and had multiple osteolytic lesions and pathological fractures of her extremities. Highly increased serum levels of Interleukin-6 (IL-6) and a parathyroid hormone-related protein (PTHrP) together with a high level of serum calcium observed at the time of fractures suggested their contribution to the formation of the bone lesions.

BACKGROUND: Pathological fractures result from abnormal bone remodeling caused by local bone lesions, such as malignant tumors, compromising bone strength. Tumor-derived factors disrupt the balance between bone resorption and formation, leading to osteolytic and osteosclerotic lesions that weaken bone integrity. While surgical treatments, including limb amputation and limb-sparing surgery, are commonly used, the therapeutic efficacy of fracture repair remains unclear due to limited evidence.
AIM: This study characterized pathological fractures and evaluated survival time and functional progno

Type 2 diabetes (T2D) is associated with increased risk of fractures. However, it is unclear whether current osteoporosis treatments reduce fractures in individuals with diabetes. The aim of the study was to determine whether presence of T2D influences the efficacy of antiresorptive treatment for osteoporosis using the Foundation for the National Institutes of Health (FNIH)-American Society for Bone and Mineral Research (ASBMR)-Study to Advance Bone Mineral Density (BMD) as a Regulatory Endpoint (SABRE) cohort, which includes individual patient data from randomized trials of osteoporosis thera

Erythropoietin (EPO) is the primary regulator of bone marrow erythropoiesis. Mouse models have provided evidence that EPO also promotes bone remodeling and that EPO-stimulated erythropoiesis is accompanied by bone loss independent of increased red blood cell production. EPO has been used clinically for three decades to treat anemia in end-stage renal disease, and notably, although the incidence of hip fractures decreased in the United States generally after 1990, it rose among hemodialysis patients coincident with the introduction and subsequent dose escalation of EPO treatment. Given this cli

For conditions like osteoporosis, changes in bone pore geometry even when porosity is constant have been shown to correlate to increased fracture risk using techniques such as dual-energy x-ray absorptiometry (DXA) and computed tomography (CT). Additionally, studies have found that bone pore geometry can be characterized by ultrasound to determine fracture risk, since certain pore geometries can cause stress concentration which in turn will be a source for fracture. However, it is not yet fully understood if changes in pore geometry can be detected by ultrasound when the porosity is constant.

CONTEXT: Epilepsy and osteoporosis are closely related. The detrimental effect on bone by older generation of antiepileptic drugs (AEDs) is well known, but newer AEDs can also cause a decline in bone health.
OBJECTIVE: To provide a review on the impact of AEDs on bone mineral density, fractures and bone turnover markers and to analyze the effect of bone active treatments in epileptic patients.
METHODS: Medline (PubMed) and EMBASE were searched for studies about AEDs and bone health. The PRISMA statement was used.
RESULTS: Chronic use of AEDs is associated with alterations in bone metabolism

To prevent osteoporotic fracture occurrence, a variety of treatment regimens with different mechanisms of action is available. The antiresorptive bisphosphonate drugs are currently the most commonly prescribed agents in the management of patients with osteoporosis. The recombinant amino-terminal fragment of human parathyroid hormone (Teriparatide) is a bone anabolic agent which reduces fracture risk by increasing bone mass and improving bone microarchitecture. Teriparatide treatment reduces vertebral and non-vertebral fracture risk markedly in women and men with idiopathic osteoporosis, or wit
