Acute Effects of Multi-Joint Eccentric Exercise on Lower-Extremity Muscle Activation Measured During Land and Water Walking.

Falls are a leading cause of injury and loss of independence in older adults, often linked to deficits in lower-limb muscle function and gait mechanics. Eccentric exercise can improve muscular resilience, while aquatic walking offers a safe, supportive environment to retrain gait; however, little is known about how these modalities interact at the neuromuscular level. This study compared lower-limb muscle activation during gait on land and in water, before and after an acute bout of eccentric exercise, in healthy young adults. Surface electromyography was collected from the tibialis anterior (…

Variations in knee compressive force profiles in patients with osteoarthritis: the absence of the first peak in knee compressive force during walking.

This study aims to investigate the gait strategies employed by some individuals with knee osteoarthritis (KOA) who walk without the first peak of the knee contact force (KCF). Additionally, it seeks to analyze the differences between those who walk with and those who walk without the first peak. In this cross-sectional study, 55 individuals with KOA were categorized into two groups (no first peak and two peaks). Gait data were collected with a motion capture system, and individualized musculoskeletal models calculated the KCF. MRI scans were performed to compare knee joint structures and score…

Ultrasound-guided acupotomy for post-stroke equinovarus foot beyond the rehabilitation golden period: Two CARE-compliant case reports.

BACKGROUND: Post-stroke equinovarus foot (EVF) is a common and disabling complication of stroke, characterized by ankle plantarflexion and inversion due to muscle spasticity. It often results in abnormal gait patterns, increased risk of falls, and diminished quality of life. Although various interventions have been proposed, targeted and effective treatments remain limited. CASE PRESENTATION: We report two patients with post-stroke EVF: a 54-year-old male 10 months after ischemic stroke, and a 37-year-old female 2 years after hemorrhagic stroke. Both exhibited spasticity of the gastrocnemius …

Utilizing mobile robotics for pelvic perturbations to improve balance and cognitive performance in older adults: a randomized controlled trial.

Late-life balance disorders remain a severe problem with fatal consequences. Perturbation-based balance training (PBT), a form of rehabilitation that intentionally introduces small, unpredictable disruptions to an individual's gait cycle, can improve balance. The Tethered Pelvic Assist Device (TPAD) is a cable-driven robotic trainer that applies perturbations to the user's pelvis during treadmill walking. Earlier work showcased improved gait stability and the first evidence of increased cognition acutely. The mobile Tethered Pelvic Assist Device (mTPAD), a portable version of the TPAD, applies…

Slower than normal walking speeds involve a pattern shift in joint and temporal coordination contributions.

Kinematic and spatiotemporal gait parameters are known to scale with gait speed, though inter-joint coordination during swing remains consistent, at least across comfortable speeds. The purpose of this study was to determine whether coordination patterns serving limb clearance and shortening change across a range of gait speeds. We assessed 17 healthy adults walking overground at their self-selected speed and multiple, progressively slower speeds. We collected lower extremity kinematics with 3D motion analysis and quantified joint influence, or relative joint contributions, to limb clearance a…

The Heidelberg Functional Foot Model-Application to Cavovarus and Equinovarus Feet.

Multisegment foot models have become increasingly important in biomechanical research and clinical gait analysis but often face limitations in defining joint positions. Often, they rely on simplified methods, such as using the midpoint between two markers to represent a joint, which lacks functional verification. In contrast, phenomenological angles, such as the medial arch angle, bypass joint center calculations, and offer sensitive, radiologically aligned indicators of foot mechanics. The Heidelberg functional foot model (HFFM) integrates functionally verified joint positions in combination …

Levodopa influence on turning dynamics in people with Parkinson's disease.

Turning behaviors are affected in people with Parkinson's disease (PwPD) leading to falls and Freezing of Gait (FOG). Levodopa therapy is commonly recommended for PwPD to help alleviate motor symptoms. Knowledge of turning dynamics with levodopa therapy is limited and could improve clinical care. We enrolled 44 PwPD already on levodopa therapy and analyzed their turns on an instrumented gait mat. Participants made 180-degree turns in their OFF-state (> 8 h off levodopa) then ON-state (1 h after levodopa). Thirteen turn measures were assessed for changes between the OFF- to ON-state. Compared t…

Concurrent validity of Protokinetics movement analysis software for estimated centre of mass displacement and velocity during walking.

BACKGROUND: The possibilities of out-of-lab gait analysis are ever increasing, with a recent development from Protokinetics movement analysis software (PKMAS) which, among other parameters, proposes an estimation of centre of mass (COMe) displacement. This measure can be useful in both clinical and sports performance environments. RESEARCH QUESTION: Does PKMAS provide an accurate estimation of the COM displacement and velocity? METHODS: Fifteen participants completed 10 walking trials over the Zeno™ Walkway while simultaneously being equipped with a full-body marker set for VICON analysis. T…

Ability of a multi-segment foot model to measure kinematic differences in cavus, neutrally aligned, asymptomatic planus, and symptomatic planus foot types.

BACKGROUND: Multi-segment foot models (MFMs) provide a better understanding of the intricate biomechanics of the foot, yet it is unclear if they accurately differentiate foot type function during locomotion. RESEARCH QUESTION: We employed an MFM to detect subtle kinematic differences between foot types, including: pes cavus, neutrally aligned, and asymptomatic and symptomatic pes planus. The study investigates how variable the results of this MFM are and if it can detect kinematic differences between pathologic and non-pathologic foot types during the stance phase of gait. METHODS: Independe…
