The primary care physician and Alzheimer's disease: an international position paper.

This paper aims to define the role of the primary care physician (PCP) in the management of Alzheimer's disease (AD) and to propose a model for a work plan. The proposals in this position paper stem from a collaborative work of experts involved in the care of AD patients. It combines evidence from a literature review and expert's opinions who met in Paris, France, on July 2009 during the International Association of Geriatrics and Gerontology (IAGG) World Congress. The PCP's intervention appears essential at many levels: detection of the onset of dementia, diagnostic management, treatment and …

Association between the electronic frailty index and mortality in older adults: a systematic review and meta-analysis.

OBJECTIVE: Frailty has become an important health concern in older adults in the context of global population aging. The electronic frailty index (eFI), derived from routinely collected electronic health record data, has been increasingly used to assess frailty in clinical practice and research. However, no meta-analysis has systematically synthesized the association between eFI-defined frailty and mortality in older adults. METHODS: We searched PubMed, Embase, Web of Science, Cochrane Library, ProQuest, Ovid MEDLINE Complete, CINAHL, and Scopus for studies examining the association between e

Risk prediction models for hospitalization-associated disability among older adults: A systematic review and meta-analysis.

BACKGROUND: Hospitalization-associated disability is a significant adverse outcome in older adults, leading to prolonged hospital stays and increased healthcare burden. Although several risk prediction models have been developed to identify hospitalization-associated disability, their quality, risk of bias and clinical applicability remain unclear. OBJECTIVES: To systematic review existing evidence on risk prediction models for hospitalization-associated disability among older adults. METHODS: Eight databases were searched from inception to October 15, 2025. Data from the included studies we

Frailty and Oncology: An Increasingly Common Combination.

BACKGROUND: Frailty is defined by a reduction in physiological reserve and an increased vulnerability to stressors. In oncology, frailty is highly prevalent and has been consistently associated with a worse prognosis. The aim of this manuscript is to understand the interaction between frailty and cancer to optimise therapeutic decision-making and improve patient-centred outcomes. METHODS: A narrative literature review was conducted using the PubMed database, with articles published up to July 2025 included. The search terms used included "frailty", "oncology", "cancer", "malignancy", "diagnos

[Evaluation of the medical and social status of hundred-year centenarians in the Chelyabinsk region.].

The analysis of demographic trends reveals a sustained pattern of population aging. This process is associated with both declining fertility rates and increasing life expectancy. Despite these well-documented global demographic shifts, the health status of centenarians remains understudied. The aim of this research is to assess the medical and social status of centenarians in the Chelyabinsk Region. To investigate the health of centenarians, data extraction was performed for patients aged 100 years and older. The final database comprises 72 records detailing the health status and parameters of

Does the frailty index applied to randomised controlled trials really measure frailty?

BACKGROUND: Cumulative deficit frailty indices from randomised controlled trials (RCT) are increasingly used to assess whether trial findings are applicable to people living with frailty. The aim of this paper was to examine the range and type of deficits included in these frailty indices and compare these to those from cohort studies. METHODS: We identified RCTs assessing treatment effect modification using the cumulative deficit frailty index, as well as cohort studies assessing mortality risk associated with frailty, from recent systematic reviews. We extracted the deficits included in the

Improving staff awareness of sensory aid needs and dementia status in an old age ward.

BACKGROUND: Having initially done our critical appraisal of various studies, we found that geriatric wards have a higher prevalence of delirium and dementia. Among this older population, sensory aids were found to be important in orientating delirious patients, and deficits in vision/hearing were associated with an increased risk of delirium. Delirium and dementia are associated with increased morbidity and mortality. This quality improvement project aimed to increase staff awareness of patient sensory aid needs and thus improve patient care as a result. METHODS: We started off with a driver

Applying the Age-Friendly Health Systems Framework to Promote Adoption and Sustainability of Interventions for Older Adults in the Intensive Care Unit.

Older adults hospitalized in an intensive care unit (ICU) frequently experience adverse outcomes, including delirium, functional decline, medication-related harm, and goal-discordant care. Despite the availability of evidence-based interventions to address these risks, adoption and sustained implementation remain limited. The Age-Friendly Health Systems 4Ms Framework offers a policy-aligned structure for advancing adoption and sustainability of gerontological nursing interventions in the ICU. The Centers for Medicare & Medicaid Services announcement of the 2025 Age-Friendly Hospital Measure ma

Targeting oral frailty indicators of late-life cognitive disorders and depression: a systematic review.

BACKGROUND: Oral frailty is an age-related gradual loss of oral function together with a cognitive and physical function decline. Previous systematic reviews and meta-analyses examined the association of some oral frailty indicators with late-life cognitive and late-life depression (LLD). However, none of these studies investigated a large series of possible indicators and outcomes. OBJECTIVE: To clarify the impact of oral frailty indicators on late-life cognitive disorders and LLD. DESIGN: Systematic review. SUBJECTS: Sixty-three studies (56,520,662 subjects) with 11 oral frailty indicator

[A Study of Local Anesthesia Surgery Cases for Elderly Breast Cancer Patients].

The standard treatment for breast cancer surgery is for the patient to be under general anesthesia, but many elderly patients are at high risk under general anesthesia. In some cases, it is possible to do surgery under local anesthesia, but there have been few reports evaluating the usefulness of such surgery. Therefore, we investigated this in 17 patients at our hospital. The mean age was 78 years old(median 83, 42-98), and mean tumor diameter 14 mm(median 13, 3-40). The reasons why local anesthesia was selected were due to old age, comorbidities, diagnostic treatment, and difficulty of hospi

Frailty Focus: Empowering Rural Health With Advanced Nurse Practitioners: A Discussion Paper.

BACKGROUND: Frailty is a multidimensional condition with symptoms relating to falls, immobility, incontinence, impaired memory and medication side-effects. With increasing numbers of frailty, particularly in rural areas, healthcare systems are being challenged globally. Moreover, frailty may be more common in rural communities as a consequence of transportation issues, limited access to healthcare services and health promotion activities. Advanced Nurse Practitioners are ideally placed to undertake comprehensive geriatric assessments and identify frailly syndromes. AIMS: Explore the function

The effects of remimazolam and propofol on delirium following anesthesia and sedation in elderly patients: a systematic review and meta-analysis.

BACKGROUND: Delirium is a prevalent and serious postoperative complication in elderly patients, with an incidence reaching up to 70%, particularly in intensive care settings. It is associated with prolonged recovery, increased healthcare costs, and heightened mortality risk. While multifactorial etiologies involving neurotransmitter dysregulation and inflammatory cascades underline its pathogenesis, anesthetic agent selection has emerged as a modifiable risk factor requiring rigorous evaluation. OBJECTIVE: This meta-analysis aims to compare the effects of remimazolam and propofol on the incide

Clinical Value of Nutritional Assessment Tools in Older Adults with Chronic Obstructive Pulmonary Disease: An Integrative Review.

Chronic Obstructive Pulmonary Disease (COPD) is a progressive respiratory disorder frequently observed in older adults and is often accompanied by aging-related conditions such as sarcopenia, cognitive impairment, and immune dysfunction. Malnutrition is a major yet underrecognized contributor to poor outcomes in this population, accelerating disease progression and functional decline. This integrative review critically examines the clinical value of three widely used nutritional assessment tools-the Mini Nutritional Assessment (MNA), the Patient-Generated Subjective Global Assessment (PG-SGA),
