Erythropoietin and erythropoiesis in renal transplantation.

This report reviews the features of erythropoietin (Epo) production after renal transplantation. Successful kidney transplantation leads to the correction of renal anaemia over an 8-10 week period. An early ineffective peak of serum Epo may occur when there is delayed graft function. A late peak follows the decrease in serum creatinine and this is associated with a rise in haemoglobin. Serum Epo returns to normal when the haematocrit reaches 32%. Acute early rejection causes a striking reduction in serum Epo and reticulocytosis. In some patients the haematocrit continues to increase after comp

[Renal transplantation in children under five years of age. Experience at the Hopital des Enfants-Malades].

From 1976 through 1991, 50 renal transplants (with 7 kidneys from living related donors and 43 cadaver kidneys) were performed in 47 children under five years of age (range 11 to 59 months) at the Enfants-Malades Hospital, Paris, France. Donor age ranged from 3 months to 53 years. Six of the seven kidneys from living related donors are currently functioning after a follow-up of 6 months to 8 years, whereas actuarial survival of cadaver kidneys was 70% at one year and 66% at five years. The main cause of graft loss was vascular thrombosis (40% of lost kidneys). In the most recent years of the s

Improved allograft survival of strong immune responder-high risk recipients with adjuvant antithymocyte globulin therapy.

The pretransplant cellular immune responsiveness of 90 renal failure patients was correlated with subsequent allograft survival. Patients were subdivided in two bases: whether the pretransplant immune parameter values were above (strong responder) or below (weak responder) the group median, and whether they were responsive or anergic to recall skin test antigens. In a group of 72 cadaveric renal allograft recipients, treated with only Imuran and prednisone, the overall 1-year graft survival was 48%. Pretransplant immunocompetence correlated with graft survival: factors predicting longer allogr

Comparison of outcomes of living and deceased donor kidney grafts surviving longer than 5 years in Korea.

BACKGROUND: It is generally recognized that living donor kidney transplantation (LDKT) grafts are superior to deceased donor kidney transplantation (DDKT) grafts. We compared survival and functional outcomes of LDKT and DDKT grafts. METHODS: Among 1000 kidneys transplanted from 1995 to 2008, we selected grafts surviving >5 years, excluding pediatric, multi-organ transplantation, and retransplantations (n=454). RESULTS: There were 179 kidneys from deceased donors and 275 from living donors. Recipients showed no difference in age, gender, or cause of renal failure. Donors were younger in the D

Non-HLA antibodies worsen the histological phenotype and prognosis of antibody mediated rejection in kidney allografts.

INTRODUCTION: Antibody-mediated rejection (AMR) remains a leading cause of kidney allograft failure, with both HLA and non-HLA antibodies implicated in its pathogenesis. The contribution of non-HLA antibodies (non-HLA Abs) to microvascular inflammation (MVI) and graft outcome, particularly in cases lacking donor-specific anti-HLA antibodies (HLA-DSAs), remains incompletely understood. METHODS: We analyzed 571 post-transplant serum samples from 326 patients with histological features of AMR (AMRh) and 164 stable control individuals. Non-HLA Abs were detected using the previously developed Non-

Total intravenous anesthesia in kidney transplantation: A narrative review.

BACKGROUND: Kidney transplantation is the treatment of choice for end-stage renal disease. Whether propofol-based total intravenous anaesthesia (TIVA) confers advantages over volatile anaesthesia for perioperative stability, recovery, and graft outcomes remains debated. AIM: To synthesise the currently available comparative evidence on propofol-based TIVA versus volatile anaesthesia in adult kidney transplantation-focusing on haemodynamic stability, renal injury biomarkers, early graft function, recovery, postoperative nausea and vomiting (PONV), immunologic outcomes, and safety-and to identi

Engineering Immune-Protective Stem Cell-Derived β-Cell Grafts for Type 1 Diabetes: Encapsulation Technologies and the Emerging Graft Ecosystem.

BACKGROUND: Stem cell-derived β-cell replacement has emerged as a promising strategy for the treatment of type 1 diabetes (T1D), potentially addressing the limitations of exogenous insulin therapy and donor-dependent islet transplantation. However, despite substantial progress in stem cell differentiation and transplantation technologies, long-term graft survival remains constrained by immune rejection, recurrent autoimmunity, fibrosis, hypoxia, and inadequate vascularization. Encapsulation technologies have therefore become central to the development of clinically viable bioartificial pancrea

Incidence, risk factors and survival outcomes of post-transplant tertiary hyperparathyroidism in kidney recipients.

BACKGROUND: Post-transplant tertiary hyperparathyroidism (PT-tHPT) is a well-recognized complication following kidney transplantation, characterized by persistent excessive secretion of parathyroid hormone (PTH) despite improved renal function. It is potentially associated with an increased risk of cardiovascular events, renal osteodystrophy, pathologic fractures, graft loss, and mortality. AIM: To evaluate the incidence, risk factors, and outcomes of PT-tHPT amongst kidney transplant recipients. METHODS: A total of 887 transplant recipients who underwent transplantation between 2000 and 202

[ROBOTIC-ASSISTED KIDNEY TRANSPLANTATION - IMPLEMENTATION AND INITIAL EXPERIENCE AT BEILINSON TRANSPLANT CENTER].

INTRODUCTION: Robotic-assisted kidney transplantation (RAKT) has brought the benefits of minimally invasive surgery to the field of renal transplantation. The robotic platform overcomes some of the major limitations of the laparoscopic approach facilitating complex surgical procedures such as kidney transplantation with non-inferior or improved outcomes, particularly in obese patients. In this article we describe the implementation of the RAKT program, donor and recipient selection, our unique surgical technique, and analyze early results. METHODS: This is a retrospective study of all RAKTs p

Long-Term Outcome of Kidney Transplant Among Iranian Children: A Systematic Review and Meta-Analysis.

OBJECTIVES: End-stage kidney disease has dramatic health effects and life consequences in children. Presently, kidney transplant has been globally accepted as a treatment of choice for end-stage kidney disease in both children and adults, leading to better quality of life and longer patient survival. Because of lack of comprehensive information on the outcome of kidney transplant among children in Iran, we aimed to present a proper vision of pediatric kidney transplant in Iran by systematically reviewing the current literature. MATERIALS AND METHODS: Major databases were searched, including M

Long-Term Outcomes of Kidney Transplants from Older/Marginal Donors: A Cohort Study.

INTRODUCTION: To safely expand the donor pool, we introduced a strategy of biopsy-guided selection and allocation to single or dual transplantation of kidneys from donors >60 years old or with hypertension, diabetes, and/or proteinuria (older/marginal donors). Here, we evaluated the long-term performance of this approach in everyday clinical practice. METHODS: In this single-center cohort study, we compared outcomes of 98 patients who received one or two biopsy-evaluated grafts from older/marginal donors ("recipients") and 198 patients who received nonhistologically assessed single graft from

Femtosecond Laser-Assisted Small Incision Endokeratophakia Using a Xenogeneic Lenticule in Rhesus Monkeys.

PURPOSE: To evaluate the feasibility and safety of xenogeneic endokeratophakia in rhesus monkeys and to explore the effects of a concave lenticule on refractive power. METHODS: Three adult New Zealand white rabbits and 6 healthy rhesus monkeys were used. The xenogenic concave grafts were created from the rabbits using a modified small incision lenticule extraction technique; after being cryopreserved in glycerol for 1 week, the grafts were implanted into the monkey recipient corneas. Spherical equivalent (SE), central corneal thickness, and keratometry curvature were assessed preoperatively,

Renin-Angiotensin System Blockage and Avoiding High Doses of Calcineurin Inhibitors Prevent Interstitial Fibrosis and Tubular Atrophy in Kidney Transplant Recipients.

OBJECTIVES: Chronic allograft dysfunction is a complex and multifactorial process characterized by progressive interstitial fibrosis and tubular atrophy. The finding of interstitial fibrosis and tubular atrophy is prevalent among kidney transplant patients receiving a calcineurin inhibitor-based immunosuppressive regimen and may be considered as a surrogate of allograft survival. Both immune (acute rejection episodes, sensitization, and HLA incompatibility) and nonimmune (donor age, delayed graft function, calcineurin inhibitor toxicity, infections, and hypertension) mechanisms play a role in

Surgical Complications After Pediatric Renal Transplant.

OBJECTIVES: Renal transplant is the most appropriate treatment for both adult and pediatric patients with end-stage renal failure. Here, we analyzed surgical complications after pediatric renal transplant at our center. MATERIALS AND METHODS: We retrospectively analyzed data from patient files and hospital charts of pediatric patients who had renal transplant at our center (Gazi University, Ankara, Turkey). Our immunosuppression protocol, a calcineurin inhibitor-based triple regimen, was applied to all recipients (steroids, mycophenolic acid). For neoureterocystostomy anastomosis, we used the

C1q-fixing human leukocyte antigen antibodies are specific for predicting transplant glomerulopathy and late graft failure after kidney transplantation.

BACKGROUND: Human leukocyte antigen (HLA) antibodies, especially those that fix complement, are associated with antibody-mediated rejection and graft failure. The C1q assay on single antigen beads detects a subset of HLA antibodies that can fix complement and precede C4d deposition. The aim of this study was to determine whether C1q-fixing antibodies distinguish de novo donor-specific antibodies (DSA) that are clinically relevant and harmful. METHODS: We retrospectively studied 31 of 274 kidney transplant recipients who had pretransplant and concurrent biopsy and serum specimens, 13 with C4d-

Increasing levels of hemoglobin improve renal transplantation outcomes.

INTRODUCTION: Several studies have reported various data on prevalence of posttransplant anemia (PTA). We have little information about its impact on long-term graft outcomes and few studies of the optimal hemoglobin (Hb) target in kidney transplantation. METHODS: We examined retrospectively 144 kidney transplant recipients of mean age 44.4 ± 12.3 years and follow-up of 40.5 ± 4.6 months. Exclusion criteria were age below 18 years, multiorgan transplantation, and graft failure in the first year. Using simple and multiple linear regression models, we evaluated the potential prediction of a ser

Thrombotic microangiopathy associated with parvovirus B 19 infection after renal transplantation.

Human parvovirus B19 is considered an etiologic agent of aplastic anemia in immunosuppressed patients. Microscopic vasculitis, with or without renal involvement, has recently been attributed to this viral infection in immunocompetent patients. This study describes four cases of thrombotic renal graft microangiopathy presumably secondary to B19 infection. Twelve to 50 days after transplantation, four patients presented a renal graft dysfunction with creatinine rising to 360 to 1088 micromol/L and requiring hemodialysis in three cases. Renal involvement appeared after a systemic illness characte
