Mediastinal staging with video-assisted mediastinoscopic lymphadenectomy after endobronchial ultrasound-guided transbronchial needle aspiration: real-world evidence in 228 patients.

BACKGROUND: Accurate staging of mediastinal lymph nodes in patients with non-small-cell lung cancer (NSCLC) is essential to determine further management. Current guidelines recommend confirmatory mediastinoscopy for patients with suspicious mediastinal lymph node metastases in image-based staging but negative findings on endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). In clinical practice, adherence to these recommendations is low and many physicians omit invasive surgical staging. This study aimed to assess the results of subsequent video-assisted mediastinoscopi

Selected infectious disease topics in the ED: Optimizing logistics to optimize care.

PURPOSE: Emergency medicine pharmacists (EMPs) are involved in many clinical decisions made at the bedside in the emergency department (ED). Along with answering clinical questions, pharmacists are often relied on to optimize logistical issues requiring coordination among multiple departments and clinical applications, such as the electronic medical record and automated dispensing cabinets. This article discusses 3 common infectious disease-related issues that pharmacists can assist with to ensure care is optimized during and after a patient's ED visit. SUMMARY: This article first discusses a

Polypharmacy in HIV: Rethinking what counts and why it matters.

Polypharmacy, the concurrent use of multiple medications, presents a growing challenge in HIV care as people living with HIV age and experience earlier onset of age-related co-morbidities. However, how polypharmacy is defined and assessed in HIV research remains inconsistent. The commonly used threshold of five or more medications, often derived from geriatric medicine, may not adequately reflect the clinical complexity of HIV care, where lifelong antiretroviral therapy (ART) forms the foundation of treatment. This review examines how polypharmacy has been defined and operationalized in HIV st

From whole genomes to probiotic candidates: A study of potential lactobacilli strains selection for vaginitis treatment.

Vaginitis, characterized by pathogenic invasion and a deficiency in beneficial lactobacilli, has recognized lactobacilli supplementation as a novel therapeutic strategy. However, due to individual differences in vaginal microbiota, identifying universally effective Lactobacillus strains is challenging. Traditional methodologies for probiotic selection, which heavily depend on extensive in vitro experiments, are both time-intensive and laborious. The aim of this study was to pinpoint possible vaginal probiotic candidates based on whole-genome screening. We sequenced the genomes of 98 previously

Pharmacist-driven outreach initiative to increase prescribing of sodium-glucose cotransporter-2 inhibitors in eligible VHA patients with chronic kidney disease: a study protocol.

BACKGROUND: Patients with chronic kidney disease (CKD) are at increased risk for multiple adverse events, several of which have been proven to be less likely with the use of sodium-glucose cotransporter-2 inhibitors (SGLT2i). As a result, guidelines now recommend SGLT2i be given to those with mild to moderate CKD and type 2 diabetes. The objective of this study is to evaluate if a pharmacist-driven SGLT2i prescribing initiative among eligible patients with CKD and diabetes within the VA could more rapidly improve the adoption of SGLT2i via a pragmatic approach aligned with learning health syst

Disparities in Vaccination Amongst Socially Vulnerable Patients with Inflammatory Bowel Disease.

INTRODUCTION: Social determinants of health (SDOH) have a known impact on disparities in vaccination. Despite an increased risk for infection in patients with inflammatory bowel disease (IBD), SDOH and vaccination in this population have not been studied. Using census tract-level data from the Centers for Disease Control's social vulnerability index (SVI), we aimed to understand the relationship between SDOH and adherence to guideline-recommended vaccinations in patients with IBD. METHODS: A single-center retrospective cohort of patients with IBD was used to geocode patient addresses to their

Postarrest Neuroprognostication: Practices and Opinions of Canadian Physicians.

BACKGROUND: Objective, evidence-based neuroprognostication of postarrest patients is crucial to avoid inappropriate withdrawal of life-sustaining therapies or prolonged, invasive, and costly therapies that could perpetuate suffering when there is no chance of an acceptable recovery. Postarrest prognostication guidelines exist; however, guideline adherence and practice variability are unknown. OBJECTIVE: To investigate Canadian practices and opinions regarding assessment of neurological prognosis in postarrest patients. METHODS: An anonymous electronic survey was distributed to physicians who…

Quality of care and risk of incident atrial fibrillation in patients with newly diagnosed heart failure: a nationwide cohort study.

AIMS: Incident atrial fibrillation (AF) is an adverse prognostic indicator in heart failure (HF); identifying modifiable targets may be relevant to reduce the incidence and morbidity of AF. Therefore, we examined the association between quality of HF care and risk of AF. METHODS AND RESULTS: Using the Danish Heart Failure Registry, we conducted a nationwide registry-based cohort study of all incident HF patients diagnosed between 2008 and 2018 and without history of AF. Quality of HF care was assessed by seven process performance measures, including echocardiographic examination, New York Hea

Persistence of uncontrolled hypertension post-cardiac rehabilitation in stable coronary patients.

In stable coronary heart disease, uncontrolled risk factors are strongly associated with incident myocardial infarction. We analysed the management of hypertension in 746 stable coronary patients recruited between 2005 and 2015 in a single-centre prospective study. Risk factors and pharmacological treatments were documented prior to and immediately after cardiac rehabilitation, and 1 year later. One year post-cardiac rehabilitation, all cardiovascular risk factors were significantly better controlled with the notable exception of hypertension: blood pressure (BP) <140/90 mmHg in 60% of the tot
