Registry of transcatheter aortic-valve implantation in high-risk patients.

BACKGROUND: Transcatheter aortic-valve implantation (TAVI) is an emerging intervention for the treatment of high-risk patients with severe aortic stenosis and coexisting illnesses. We report the results of a prospective multicenter study of the French national transcatheter aortic-valve implantation registry, FRANCE 2. METHODS: All TAVIs performed in France, as listed in the FRANCE 2 registry, were prospectively included in the study. The primary end point was death from any cause. RESULTS: A total of 3195 patients were enrolled between January 2010 and October 2011 at 34 centers. The mean (

Allograft aortic root replacement in complex prosthetic endocarditis.

OBJECTIVE: To evaluate perioperative and long-term results of complex prosthetic valve endocarditis treated by allograft aortic root replacement. METHODS: From April 1988 through February 2006, 41 patients diagnosed as prosthetic valve endocarditis (PVE) complicated by root abscess and/or periprosthetic leak, underwent fresh allograft valve replacement by root replacement. There were 37 males (89.7%) and the mean age was 51.5+/-13.7 years. The NYHA functional class was 3.0+/-0.1. Thirty-seven patients (90%) had a mechanical prosthesis and in 10 (25.6%) the PVE was recent (< 3 months). Ten pat

Aortoventriculoplasty after a prior posterior root enlargement (Manougian): case report.

A patient with congenital bicuspid aortic stenosis had an open commissurotomy followed 3 years later by a Manougian's operation along with an aortic valve replacement. Nine years later, due to residual aortic stenosis, a Konno's anterior root enlarging procedure with an aortic valve replacement was successfully done. Good relief of aortic and subaortic stenosis and the absence of significant gradient across the left ventricular outflow tract led us to believe that successful and effective relief could be obtained by adding a Konno-type of enlargement to a previously performed posterior enlarge

Usefulness of intracardiac echocardiography for the diagnosis of infective endocarditis following percutaneous pulmonary valve replacement.

Infective endocarditis is a serious complication occurring after percutaneous pulmonary valve replacement with a Melody valve; it is of uncertain incidence, and presents a challenging diagnosis. Vegetations may be difficult to visualise with transthoracic and transoesophageal echocardiography. We report the role of intracardiac echocardiography in this condition by confirming the prosthesis involvement.

Ever heard of the obturator vein?

We report a complication related to advancement of a large diameter long sheath in the obturator vein. In an adult with a previously thrombosed femoral vein, the obturator vein was inadvertently cannulated. During percutaneous pulmonary valve implantation, the Ensemble(R) delivery system could not be advanced after repositioning attempt. The obturator vein had been perforated and sheath was coiled up in the pelvis leading to a retroperitoneal pelvic hematoma. Continuity of the vein was established using two self-expanding covered stents. The obturator vein runs dorsally and joins the inferior

Infective Endocarditis After Melody Valve Implantation in the Pulmonary Position: A Systematic Review.

BACKGROUND: Infective endocarditis (IE) after transcatheter pulmonary valve implantation (TPVI) in dysfunctioning right ventricular outflow tract conduits has evoked growing concerns. We aimed to investigate the incidence and the natural history of IE after TPVI with the Melody valve through a systematic review of published data. METHODS AND RESULTS: PubMed, EMBASE, and Web of Science databases were systematically searched for articles published until March 2017, reporting on IE after TPVI with the Melody valve. Nine studies (including 851 patients and 2060 patient-years of follow-up) were in

Decellularized aortic conduits: could their cryopreservation affect post-implantation outcomes? A morpho-functional study on porcine homografts.

Decellularized porcine aortic valve conduits (AVCs) implanted in a Vietnamese Pig (VP) experimental animal model were matched against decellularized and then cryopreserved AVCs to assess the effect of cryopreservation on graft hemodynamic performance and propensity to in vivo repopulation by host's cells. VPs (n = 12) underwent right ventricular outflow tract substitution using AVC allografts and were studied for 15-month follow-up. VPs were randomized into two groups, receiving AVCs treated with decellularization alone (D; n = 6) or decellularization/cryopreservation (DC; n = 6), respectively

Bioprosthetic valve thrombosis and degeneration following transcatheter aortic valve implantation (TAVI).

Bioprosthetic valve thrombosis (BPVT) is a recognised complication of prosthetic aortic valves and can be found in up to 13% of patients after transcatheter implantation. The mechanism of BPVT is not well known, abnormal flow conditions in the new and native sinuses and lack of functional endothelialisation are suspected causes. BPVT may result in valve dysfunction, possibly related to degeneration, and recurrence of patient symptoms, or remain subclinical. BPVT is best diagnosed at multiphase gated computed tomography (CT) angiography as the presence of reduced leaflet motion (RELM) and hypoa

Modes of failure in explanted mitroflow pericardial valves.

BACKGROUND: Bioprosthetic heart valves are a treatment option for patients with significant valvular disease. We present the first morphologic analysis of a series of explanted Mitroflow pericardial valves (model A12) and discuss their modes of failure. METHODS: Between 2001 and 2010, 12 Mitroflow (A12) valves (Sorin Group Inc, Vancouver, Canada) were examined, grossly and microscopically, for postimplantation changes. Cusp tears and thickening, calcification (at radiography), pannus formation, inflammation, and thrombus deposition were noted. Microscopically, tissue degeneration including th

Fungal meningitis and intracerebral hemorrhage complicating prosthetic valve endocarditis.

A 50 yr old man presented with meningitis 163 days following aortic valve replacement for congenital bicuspid aortic stenosis. One week later he died of intracerebral hemorrhage and autopsy revealed fungal prosthetic valve endocarditis. Colonies of branching septate hyphae were adherent to the oculomotor nerves and left posterior communicating artery. This pattern of fungal basal meningeal involvement complicating prosthetic valve endocarditis has not been previously described.

Spontaneous host endothelial growth on bioprostheses. Influence of fixation.

BACKGROUND: Neither homografts nor bioprostheses have previously been seen to acquire a host endothelium. We previously reported a direct relation between aldehyde tanning and bioprosthesis calcification and the absence of calcification in the absence of aldehyde. METHODS AND RESULTS: Bovine pericardium was 1) treated with 0.625% glutaraldehyde and stored in 4% formaldehyde, 2) treated with 99.5% glycerol, and 3) treated with 99.5% glycerol and stored in formaldehyde (0.25-4%). The treated pericardium was used to construct stentless mitral valve prostheses (of a single pattern) that were impl

[A technique for distal anastomosis in replacement of the ascending aorta and aortic valve with a composite graft: application of an open aortic anastomosis under retrograde cerebral perfusion and deep hypothermic systemic circulatory arrest].

Between 1987 to 1990, six patients underwent replacement of the ascending aorta and aortic valve with a composite graft which was not wrapped with aneurysmal aortic wall; annuloaortic ectasia in 4, aortitis with acute onset of aortic regurgitation in 1, and acute aortic dissection with aortic regurgitation in 1. In 2 patients, "open aortic anastomosis" under deep hypothermic systemic circulatory arrest and retrograde cerebral perfusion was utilized for distal anastomosis of the graft to the resected aortic arch. Three patients required no transfusion. One patient with annuloaortic ectasia and
