THA following deformities due to congenital dislocation of the hip joint.

Total hip replacement is the treatment of choice for the patient suffering from end-stage hip osteoarthritis. Excellent long-term results have been published. In the presence of deformities due to congenital hip dislocation, total hip replacement is, in most of the cases, a difficult task, since the technique of performing such an operation is demanding and the results could vary. This paper presents our experience and preferred strategies focusing on challenges and surgical techniques associated with reconstructing the dysplastic hip.

Safety and efficacy of edoxaban in patients undergoing hip fracture surgery.

INTRODUCTION: Edoxaban is an oral, direct, once-daily factor Xa inhibitor. This study evaluated the safety and efficacy of edoxaban compared to subcutaneous enoxaparin in Japanese patients undergoing hip fracture surgery. MATERIALS AND METHODS: In this multicenter, randomized, open-label, active-comparator, phase 3 trial, 92 patients were randomized 2:1 to receive edoxaban 30mg once daily (n=62) or enoxaparin sodium (enoxaparin) 2000IU (equivalent to 20mg) twice daily (n=30) for 11 to 14days. The primary endpoints were the incidence of major or clinically relevant non-major (CRNM) bleeding an…

How are dysplastic hips different? A three-dimensional CT study.

BACKGROUND: Surgical correction of acetabular dysplasia can postpone or prevent joint degeneration. The specific abnormalities that make up the dysplastic hip are controversial. QUESTIONS/PURPOSES: (1) What are the relative size, shape, and orientations of the typical nondysplastic hip? (2) How do these variables differ in the developmentally dysplastic hip? (3) Are there version differences between the acetabuli of dysplastic and nondysplastic hips? (4) Are there pairs of variables in which the change in one is always accompanied by a change in the other for both nondysplastic and dysplastic…

Prevalence of radiographic hip osteoarthritis is increased in high bone mass.

OBJECTIVE: Epidemiological studies have shown an association between increased bone mineral density (BMD) and osteoarthritis (OA), but whether this represents cause or effect remains unclear. In this study, we used a novel approach to investigate this question, determining whether individuals with High Bone Mass (HBM) have a higher prevalence of radiographic hip OA compared with controls. DESIGN: HBM cases came from the UK-based HBM study: HBM was defined by BMD Z-score. Unaffected relatives of index cases were recruited as family controls. Age-stratified random sampling was used to select fu…

Finite element prediction of transchondral stress and strain in the human hip.

Cartilage fissures, surface fibrillation, and delamination represent early signs of hip osteoarthritis (OA). This damage may be caused by elevated first principal (most tensile) strain and maximum shear stress. The objectives of this study were to use a population of validated finite element (FE) models of normal human hips to evaluate the required mesh for converged predictions of cartilage tensile strain and shear stress, to assess the sensitivity to cartilage constitutive assumptions, and to determine the patterns of transchondral stress and strain that occur during activities of daily livi…

Hips with synovial chondromatosis may display the features of femoroacetabular impingement.

BACKGROUND: Both synovial chondromatosis and femoroacetabular impingement present with hip pain and may lead to hip osteoarthritis. We present a small case series and describe the clinical presentation, investigation, and treatment of patients with synovial chondromatosis who also had cam-type femoroacetabular impingement involving the same hip. METHODS: Five patients (four men and one woman with a mean age of thirty-four years [range, thirty to thirty-seven years]) with unilateral synovial chondromatosis of the hip presented with clinical and radiographic features of ipsilateral cam-type fem…

High-grade SCFE: the role of surgical hip dislocation and reduction.

The patient with an unstable slipped capital femoral epiphysis poses a challenging problem to the treating physician to improve the position of the displaced epiphysis to avoid femoroacetabular impingement without developing avascular necrosis (AVN)-a potentially devastating complication. Although the standard operative procedure of in situ pinning following an incidental reduction while positioning the patient on the table, has been the mainstay of treatment in North America, other viable options are available including a surgical dislocation approach to the hip followed by a modified Dunn os…

Schizophrenia, antipsychotics and risk of hip fracture: a population-based analysis.

In a nationwide study using linkage of Danish hospital registers we examined predictors of hip fracture (ICD-10: S72) in 15,431 patients with schizophrenia (ICD-10: F20 or ICD-8: 295) and 3,807,597 population controls. Shorter education, disability pension, lifetime alcohol abuse, somatic co-morbidity, antipsychotics (IRR=1.19; 95% CI 1.15-1.24), antidepressant (IRR=1.18; 95% CI 1.16-1.20), anticholinergics (IRR=1.29; 95% CI 1.22-1.36), benzodiazepines (IRR=1.06; 95% CI 1.04-1.08) and corticosteroids (IRR=1.44; 95% CI 1.36-1.53) were significant predictors. In 556 persons with schizophrenia an…

Hip structural analysis in adolescent boys with anorexia nervosa and controls.

CONTEXT: We have reported lower hip bone mineral density (BMD) in adolescent boys with anorexia nervosa (AN) compared with controls. Although studies have described bone structure in girls with AN, these data are not available for boys. Hip structural analysis (HSA) using dual-energy x-ray absorptiometry is a validated technique to assess hip geometry and strength while avoiding radiation associated with quantitative computed tomography. OBJECTIVE: We hypothesized that boys with AN would have impaired hip structure/strength (assessed by HSA) compared with controls. DESIGN AND SETTING: We con…

Ectopic production of human chorionic gonadotropin by synovial sarcoma of the hip.

BACKGROUND: Human chorionic gonadotropin (hCG) is a marker of pregnancy and a tumor marker for some gynecologic malignancies, including germ cell tumors and gestational trophoblastic neoplasia. Rarely, hCG is secreted by nongynecologic tumors, confounding the diagnosis. CASE: A 45-year-old woman was evaluated for a persistently elevated β-hCG. Diagnosis of her primary malignancy, synovial sarcoma of the hip, was delayed as more common etiologies were considered, including ectopic pregnancy and gestational trophoblastic neoplasm. The workup eventually led to the diagnosis using imaging studies…

Bilateral asymmetric dislocations of hip joints: an unusual mechanism of injury.

Asymmetric bilateral dislocations of the hips are rare injuries. Among the small number of reports in the literature, most have attributed the cause to high-velocity motor crashes. These dislocations are often seen to be associated with fractures of the proximal femur or the acetabulum. We present a case of a 45-year-old man with bilateral asymmetric dislocation of hips which were purely ligamentous in nature, without any fracture. He sustained his injuries due to a fall while getting on a moving bus. It was an unusual mechanism of injury as compared to the other cases of asymmetric hip disloc…

The effect of gestational age on sonographic screening of the hip in term infants.

Several authors have suggested that the final five weeks of gestation are a critical period for the development of the hip. In order to test the hypothesis that gestational age at birth may influence the development of the hip joint, we analysed the sonographic findings in 1992 hips (in 996 term newborns) with no risk factor for developmental dysplasia of the hip. The 996 infants were born at a mean gestational age of 39 weeks (37 to 41). The mean bony roof angle (α), cartilage roof angle (β) and the distribution of the type of hip were compared between the 37th, 38th, 39th, 40th and 41st birt…

Management of developmental dysplasia of the hip in young adults: current concepts.

The term developmental dysplasia of the hip (DDH) describes a spectrum of disorders that results in abnormal development of the hip joint. If not treated successfully in childhood, these patients may go on to develop hip symptoms and/or secondary osteoarthritis in adulthood. In this review we describe the altered anatomy encountered in adults with DDH along with the management options, and the challenges associated with hip arthroscopy, osteotomies and arthroplasty for the treatment of DDH in young adults.

High failure rate of the Pavlik harness in dislocated hips: is it bilaterality?

BACKGROUND: The purpose of this study was to evaluate the efficacy of Pavlik harness treatment in patients with bilaterally dislocated Graf type IV hips and compare them to cases with unilaterally dislocated hips. METHODS: Twenty-one patients (42 hips) who presented with bilaterally dislocated hips with no prior treatment were studied. The comparison group consisted of 33 patients (33 hips) with unilateral hip dislocation treated with the same protocol. Successful treatment was defined as relocation of the hips within 3 weeks of Pavlik harness application. RESULTS: Twelve of 21 patients (57%…

The role of oxidative stress in aseptic loosening of total hip arthroplasties.

This study investigated the hypothesis that wear particle-induced oxidative stress initiates osteolysis after total hip arthroplasty (THA). Patient radiographs were scored for osteolysis and periprosthetic tissues were immunostained and imaged to quantify polyethylene wear, inflammation, and five osteoinflammatory and oxidative stress-responsive factors. These included high mobility group protein-B1 (HMGB1), cyclooxygenase-2 (COX2), inducible nitric oxide synthase (iNOS), 4-hydroxynonenal (4-HNE), and nitrotyrosine (NT). The results show wear debris correlated with inflammation, 4-HNE, NT and …

Monitoring and risk of progression of osteolysis after total hip arthroplasty.

Osteolysis after total hip arthroplasty (THA) develops in response to particulate wear debris and may not be associated with clinical symptoms. Osteolysis is associated with greater wear volume. Wear increases with the increased use and activity of the joint, so longer in vivo use of a THA implant increases the risk of osteolysis. Patients with non-cross-linked, ultra-high-molecular weight polyethylene implants and younger more active patients are at greater risk for the development of osteolysis. Routine monitoring for osteolysis 5 years after THA, with radiographic imaging every 2 to 3 years…

Revision hip replacement in patients 55 years of age and younger.

The purpose of this study was to assess the efficacy and complications associated with revision hip arthroplasty undertaken in patients under 55 years of age. A review of our revision hip database showed that 430 revision hip arthroplasties undertaken over the past 9 years had a minimum follow-up of 2 years. Of these, 84 (46 female, 38 male) patients were under 55 years of age. The index procedures were preformed at an average age of 46 years (range 15 years to 54 years). The etiology of failure was aseptic loosening (74%), followed by infection (8%) and recurrent dislocation (7%). In the majo…

The burden of hip osteoarthritis in the United States: epidemiologic and economic considerations.

Osteoarthritis (OA) is a common and disabling disease. Because of improved treatment of chronic diseases and lower mortality from infectious diseases, the US population is aging, and older Americans are living with disabling conditions, including hip OA. The projected number of older adults with arthritis or other chronic musculoskeletal joint symptoms is expected to nearly double, from 21.4 million in 2005 to 41.1 million by 2030. The burden of hip OA is increasing due to the aging population and the obesity crisis; as a result, the need for total hip arthroplasty (THA) is expected to grow 17…
