A sixteen-year follow-up of three operative techniques for the treatment of acute ruptures of the anterior cruciate ligament.

BACKGROUND: This study compares three surgical procedures that we used in the past to treat ruptures of the anterior cruciate ligament: acute primary repair, acute repair augmented with a synthetic ligament-augmentation device, and acute repair augmented with autologous bone-patellar tendon-bone graft. METHODS: This is the third report on a group of patients who were randomized to the three different procedures between 1986 and 1988. There were fifty patients in each group. The patients were evaluated prospectively at one, two, five, and sixteen years with use of the Tegner activity score and

Arthroscopic anterior cruciate ligament reconstruction with quadriceps tendon autograft: clinical outcome in 4-7 years.

Surgical reconstruction of the anterior cruciate ligament (ACL) is indicated in the ACL-deficient knee with symptomatic instability and multiple ligaments injuries. In the present study, we describe the clinical results of quadriceps tendon-patellar bone autograft for ACL reconstruction. From 1996 to 1998, the graft has been used in 38 patients. Thirty-four patients with complete final follow-up for 4-7 years were analyzed. The average follow-up time was 62 (48-84) months. Thirty-two patients (94%) achieved good or excellent results by Lysholm knee rating. Twenty-six patients (76%) could retur…

Early active extension after anterior cruciate ligament reconstruction does not result in increased laxity of the knee.

If permission of full active and passive extension immediately after an anterior cruciate ligament (ACL) reconstruction will increase the post-operative laxity of the knee has been a subject of discussion. We investigated whether a post-operative rehabilitation protocol including active and passive extension without any restrictions in extension immediately after an ACL reconstruction would increase the post-operative anterior-posterior knee laxity (A-P laxity). Our hypothesis was that full active and passive extension immediately after an ACL reconstruction would have no effect on the A-P lax

Osteogenic potential of cells in vitro derived from haemarthrosis of the knee induced by injury to the anterior cruciate ligament.

We have investigated whether cells derived from haemarthrosis caused by injury to the anterior cruciate ligament could differentiate into the osteoblast lineage in vitro. Haemarthroses associated with anterior cruciate ligament injuries were aspirated and cultured. After treatment with beta-glycerophosphate, ascorbic acid and dexamethasone or 1,25 (OH)(2)D(3), a significant increase in the activity of alkaline phosphatase was observed. Matrix mineralisation was demonstrated after 28 days and mRNA levels in osteoblast-related genes were enhanced. Our results suggest that the haemarthrosis induc

Changes in patterns of scaphoid and lunate motion during functional arcs of wrist motion induced by ligament division.

PURPOSE: To determine the in vitro motion of the scaphoid and lunate during wrist circumduction and wrist dart-throw motions and to see how these motions change after the ligamentous stabilizers of the scaphoid and lunate are sectioned in a manner simulating scapholunate instability. METHODS: Twenty-one fresh-frozen cadaver forearms were moved through a dart-throw motion and a circumduction motion using a wrist joint simulator. Scaphoid and lunate motion were measured with the wrist ligaments intact and after sectioning of the scapholunate interosseous ligament, the scaphotrapezium ligament, …

Proteoglycan fragments in anterior cruciate ligament reconstructed knees: a comparative study of two different surgical techniques.

Proteoglycan fragments (PF) as chondroitin sulphate were monitored and compared in two groups of patients who had anterior cruciate ligament reconstruction with either patellar tendon (PT) or quadruple semitendinosus tendon (ST) autografts. There were 20 ST and 21 PT reconstructions. No difference existed between the groups in terms of associated lesion and time from injury to surgery. Synovial sample was collected at 6 weeks, 3 months and 18 months after the operation. No difference was noted between the groups at 18 months in terms of activity level and functional scores. Both groups demonst

Evolgate fixation of doubled flexor graft in anterior cruciate ligament reconstruction: biomechanical evaluation with cyclic loading.

BACKGROUND: The weakest points in hamstrings anterior cruciate ligament reconstruction are its points of fixation, especially on the tibial side. Methods for graft fixation to bone should be strong enough to avoid failure, stiff enough to restore load-displacement response, and secure enough to resist slippage under cyclic loading. HYPOTHESIS: Biomechanical properties of the interference screw can be improved by reinforcing the walls of the tunnel with a metal spiral (Evolgate fixation). STUDY DESIGN: Controlled laboratory study. METHODS: Three paired tests were performed using common digit

Effects of various periodontal ligament elastic moduli on the stress distribution of a central incisor and surrounding alveolar bone.

BACKGROUND AND PURPOSE: Previous studies investigating the effects of core or post on the fracture strength of teeth often omitted the periodontal ligament (PDL) because its physical properties are difficult to reliably quantify. This study investigated the effects of various PDL elastic moduli on the stress distribution around a central incisor and surrounding alveolar bone using 3-dimensional finite element stress analyses. METHODS: Four 3-dimensional finite element models of an upper central incisor and the dento-alveolar complex were established. Five models were constructed, including: n

Surgical treatment of lumbar ossification of the posterior longitudinal ligament. Report of two cases and description of surgical technique.

The authors report two cases of patients with lumbar ossification of the posterior longitudinal ligament (OPLL). One patient underwent surgery via the single posterior approach, and the other patient underwent combined anterior-posterior surgery. The authors consider the anterior approach for excision of the ossified lesion to be the most reasonable for treatment of lumbar OPLL. It is extremely important, however, to select the surgical procedure according to the individual patient's condition.

Primary repair of posterior cruciate ligament avulsion fracture: the effect of occult injury in the midsubstance on postoperative instability.

BACKGROUND: In the posterior cruciate ligament avulsion fracture, posterior instability cannot be completely restored by the anatomical reduction and fixation of an avulsed fragment. HYPOTHESIS: The occult midsubstance injury inside the posterior cruciate ligament may affect the residual posterior instability after anatomical reduction and internal fixation of the avulsed fragment. STUDY DESIGN: Prospective comparative clinical study. METHODS: Thirty-one patients were followed for a period of 2 to 8 years. Based on magnetic resonance images taken immediately after the injury, these patients

Tibiofemoral kinematics following successful anterior cruciate ligament reconstruction using dynamic multiple resonance imaging.

BACKGROUND: The aim of anterior cruciate ligament reconstruction is to reduce excess joint laxity, hoping to restore normal tibiofemoral kinematics and therefore improve joint stability. It remains unclear if successful ACL reconstruction restores normal tibiofemoral kinematics and whether it is this that is associated with a good result. STUDY: Case series. PURPOSE: To assess the kinematics of the anterior cruciate ligament-reconstructed knee using open-access MRI. METHODS: Tibiofemoral motion was assessed using open-access MRI, weightbearing through the arc of flexion from 0 degrees to 90
