A middle-aged woman began experiencing spells of profound anterograde amnesia several months after beginning intrathecal baclofen treatment for generalized dystonia. Her spells met criteria for transient global amnesia, but were unusual because of their frequent recurrence and because their frequency was somewhat dose-dependent on baclofen. Fludrocortisone decreased the frequency of these episodes, and sublingual nitroglycerin both prevented and terminated them. Baclofen-induced amnesia in rodents is a reliable model of memory impairment. In contrast, baclofen-induced memory impairment in huma

Lupus panniculitis (LP) or lupus erythematosus profundus (LEP) is a lupus-associated dermatologic condition predominantly affecting young to middle-aged women in which the deep dermis and subcutaneous fat are mainly involved. The lesions are usually located on the forehead, cheeks, proximal aspect of the limbs, and buttocks, though cases have occasionally been reported with orbital, breast, and salivary gland involvement. Descriptions of imaging findings of LP are very scarce in the literature. We describe the CT scan imaging features of the case of a patient with head and neck LP.

Spontaneous coronary artery dissection is rare. When it does occur, it can present in a myriad of ways and can mimic more common clinical scenarios. Undiagnosed it can be fatal. In this article, the authors present a case of coronary artery dissection in a middle aged man presenting as fast atrial fibrillation with hemodynamic compromise. He was unsuccessfully cardioverted and later arrested and died. Postmortem showed a ruptured dissection of the left circumflex artery causing cardiac tamponade and death. Incidence, associations, pathophysiology, investigations, and treatment options are revi…

A middle-aged woman presented with painless hematuria and passage of large clots. Two weeks earlier, she had pyrexia with upper respiratory tract symptoms. Imaging studies revealed no lesions of the urinary tract. Cystoscopy showed clots and oozing of blood from the left ureteric orifice. Urine cytology and mucosal biopsies were normal. Left nephroureterectomy was done as an emergency procedure; the hematuria stopped following the procedure. Histopathology revealed cytomegalovirus inclusion bodies in the mucosa of the excised ureter. This unusual presentation of hematuria has not been reported

Obstructive sleep apnoea/hypopnoea syndrome affects a significant number of middle-aged people. By causing the person with the condition to wake repeatedly during the night in order to restart breathing, it causes excessive daytime sleepiness and impairs cognitive function and driving ability. It can also adversely affect family relationships, particularly with partners whose sleep may also be disturbed. Nurses have a role in the diagnosis and management of the condition through appropriate questioning, observation, advice on lifestyle choices, and support for treatment. The article also focus

An active otherwise healthy and middle-aged woman presented with left supraclavicular pulsation, right upper extremity claudication, and mild dysphagia. Evaluation revealed an aberrant right subclavian artery, Kommerell's diverticulum with aneurysmal degeneration, legamentum arteriosum completing vascular ring, and absent left pulmonary artery with multiple collateral supply to the left lung. She underwent successful surgical repair via right thoracotomy, including division of the vascular ring, resection of the diverticulum and aneurysm, and finally reimplantation of the right subclavian arte

Meliodosis, an infectious disease caused by Burkholderia pseudomallei has been recognized as an emerging infectious disease in India. The infection is under diagnosed and underreported, and hence considered a rare disease. Majority of the cases have been documented from the states with heavy rainfall. The present case being reported is a middle-aged woman who had developed a fulminant infection following exposure to stagnant floodwater in the city of Hyderabad. To the best of our knowledge, this is the first case of Melioidosis being reported from this part of the country.

Cough syncope occurs primarily in middle-aged male smokers with chronic obstructive pulmonary disease (COPD). It has also been described in children with asthma. I report the case of a 34-year-old nonsmoker who had syncope due to coughing, and who also related a 1-year history of cough with wheezing. Chest examination revealed diffuse wheezing and a prolonged expiratory phase, and pulmonary function testing with a methacholine challenge confirmed hyperreactive airways. Notably, while undergoing spirometric testing he had a recurrent syncopal episode. His syncope resolved with medical therapy f

A middle-aged woman presented with a history of constipation, easy fatigue, depressive mood, lassitude, polydipsia, and polyuria. The patient posed a challenging diagnostic dilemma due to the presence of persistent severe hypercalcemia and relative lack of clinically manifested symptoms. Clinical, biochemical, and genetic examinations confirmed the diagnosis of familial hypocalciuric hypercalcemia as a result of C562Y calcium-sensing receptor mutation, and a coexisting parathyroid adenoma. After adenectomy, the patient's clinical situation improved markedly, and a modest equilibrium hypercalce

Melioidosis is an acute infectious disease caused by a safety-pin-shaped gram-negative bacteria called Burkholderia pseudomallei. Here, we report the first case of melioidosis in a middle aged male agricultural worker, from Pondicherry. The isolation of this organism from subcutaneous nodules on the extensor aspect of his limbs underlines the diversity of its clinical presentation. Difficulty in identifying the organism which mimics any other non-fementing gram-negative bacilli (NFGNB) on cursory examination, highlights the importance of identification of NFGNB in endemic areas for specific tr…

Primary spontaneous coronary artery dissection is one of the rare causes of acute myocardial infarction. Previous studies reports that it is mostly seen in middle aged women in the last trimester of pregnancy and early postpartum period. Clinical presentation of the disease is variable in pattern and severity related to extent and development rate of dissection. In the last 2 years, nine non-pregnant primary spontaneous coronary artery dissection cases were found in coronary angiography among 3750 patients prediagnosed as coronary artery disease. The cases were presented and discussed with rev…

We report a syndrome in a middle-aged woman characterized by tender erythematous plaques with histologic evidence of dramatic dermal vessel occlusion. These cutaneous findings occurred in association with progressive inferior vena cava and portal vein thrombosis while on coumarin anticoagulation, following hepatic transplantation for Budd-Chiari syndrome. The material occluding dermal vessels was proven by immunohistochemical staining to be platelet plugs. These findings led to the diagnosis of an underlying myeloproliferative disorder explaining both her cutaneous and liver abnormalities and

Two cases of cystadenoma of the biliary tract are presented. This is a rare tumor that represents less than 5% of non parasitic cysts of biliary origin. It usually occurs in middle-aged women. Ultrasound (US) and Computerized Tomography (CT) are most important for tumor diagnosis, but differential diagnosis with other cystic tumors of the liver is very difficult preoperatively. Malignant degeneration to biliary cystadenocarcinoma is documented. Surgery is always indicated and has included aspiration, sclerosis, marsupialization, internal drainage, but the complete removal of lesion with enucle

Multiple symmetrical lipomatosis (Madelung's disease) is a rare disease of undetermined cause characterized by symmetrical deposits of non-encapsulated fat on the suboccipital area, cervical area, shoulders and trunk. The patients are usually middle-aged male alcoholics. The treatment is palliative surgical removal of excess fat from the neck and paracervical regions or any other lesion sites. Oriental report about multiple symmetrical lipomatosis (MSL) is very rare. Here a case of multiple symmetrical lipomatosis in a 43 year-old man is described. We describe his clinical course and review th

We describe a middle-aged profoundly hypogonadal man with panhypopituitarism since infancy who was treated only with glucocorticoid and thyroid replacement. A magnetic resonance imaging study (MRI) revealed absence of pituitary stalk and ectopic neurohypophysis consistent with traumatic transection, probably resulting from a traumatic birth. The hormonal consequences of this stalk lesion were recognized but inadequately treated for 45 years resulting in avoidable endocrine and psychosocial defects. Androgen replacement was started at age 45 with good initial results. The unique hormonal replac

Buerger disease is an chronic vasculitis mainly involving lower extremities. This disease is sometimes contrasted with Takayasu arteritis because both are speculated autoimmune mechanisms as an important factor in their etiology. Furthermore both are frequently encountered in Asian countries. However, patients with Takayasu arteritis were mostly young women in Japan and, on the contrary, patients with Buerger disease were mainly middle aged men. Recent HLA studies on Buerger disease are revealing the interesting results in comparison with those on Takayasu arteritis. The outline of Buerger dis…

Lymphangiomas are congenital abnormalities of the lymphatics that occur predominantly in the head and neck, most often in children. We present an unusual case of a middle-aged man who had a large septated cystic lesion adherent to the pancreas that could not be differentiated from a pancreatic cystic neoplasm despite analysis by ultrasound, computed tomography, magnetic resonance imaging, or percutaneous needle aspiration. The correct diagnosis was established only via laparotomy and subsequent pathologic interpretation. Although lymphangiomas are rare, they should be included in the different

Sleep apnea syndrome is a relatively common disorder characterized by periodic cessation of breathing during sleep because of upper airway obstruction (obstructive sleep apnea) or by reduction of ventilatory drive (central sleep apnea). We report the case of a middle-aged, obese man who underwent tracheostomy for the treatment of obstructive sleep apnea documented by polysomnography. Although tracheostomy resulted in marked clinical improvement, relief of upper airway obstruction unmasked severe central sleep apnea. We report the successful treatment of this patient's central sleep apnea syndr

1. Creutzfeldt-Jakob disease (CJD) is a progressive neurologic disease that occurs in approximately one in one million individuals annually. 2. CJD affects late middle-aged and elderly individuals, with a peak occurrence between the ages of 55 and 75. 3. The disease is thought to be caused by an unknown virus that enters the central nervous system, causing neuron destruction and replacing the destroyed neurons with plaques. 4. There is no known cure or treatment for CJD, and the time span from onset of symptoms until death is 6 to 12 months.
