Clinical utility of molecular profiling in rare lung neuroendocrine neoplasms.

Rare lung neuroendocrine neoplasms (LNENs), including pulmonary carcinoids and large-cell neuroendocrine carcinomas (LCNEC), are heterogeneous tumors. Current World Health Organization (WHO) classification primarily relies on morphological features, leading to important inter-observer variability and sub-optimal prognostic accuracy. This review highlights the clinical utility of molecular profiling to overcome these diagnostic and prognostic challenges. Recent multi-omics studies have demonstrated that pulmonary carcinoids are not a uniform entity but comprise distinct molecular subgroups (A1,

Treatment of younger patients with mantle cell lymphoma.

Mantle cell lymphoma (MCL) is an individualized entity that is well characterized at the molecular level and considered to be a disease of elderly patients. However, about half of patients are less than 65 years of age and may benefit from intensive therapies. Although MCL has been considered during the last three decades as an incurable disease with current chemotherapy regimens, in young patients recent intense chemo-immunotherapy (CIT) induction regimens including high-dose cytarabine with consolidation with autologous stem cell transplantation (ASCT) have increased significantly the outcom

Nuclear molecular imaging of venous thromboembolism: targeting clot-specific molecular components for improved diagnosis and management.

Venous thromboembolism (VTE), manifesting as deep vein thrombosis and pulmonary embolism, is a common, recurrent and potentially fatal condition. Anticoagulation remains the cornerstone of therapy, yet determining optimal treatment duration (eg, decision to shorten or extend anticoagulant treatment) is difficult due to the uncertain balance between long-term bleeding risk and recurrence. Future advances in vascular nuclear molecular imaging could critically help support this decision-making process. Current VTE diagnostic imaging relies on structural or perfusion-based techniques to infer thro

Successful antemortem diagnosis and treatment of pulmonary tumor thrombotic microangiopathy.

Pulmonary tumor thrombotic microangiopathy (PTTM) is a fatal cancer-related complication with rapid progression. Although the underlying molecular mechanisms of PTTM remain unclear, platelet-derived growth factor (PDGF) and vascular endothelial growth factor (VEGF) may play important roles in the pathogenesis of PTTM. We herein report a case of PTTM that was diagnosed antemortem by pathology using pulmonary wedge aspiration. The patient was treated with a combination therapy of imatinib (PDGF receptor antagonist), bevacizumab (VEGF receptor inhibitor), S-1, and cisplatin. These findings sugges

Molecular markers for incidence, prognosis, and response to therapy.

Lung cancer is the most common malignancy in the United States and worldwide. In 2011, it is estimated that more than 221,000 people in the United States will be diagnosed with cancer of the lung and bronchus. For patients with early-stage disease, 5-year survival approaches only 50%. Recent advances using molecular, genetic, and proteomic profiling of lung tumors have enabled refining the prognosis for patients with non-small cell lung cancer. With targeted therapies, there is an opportunity to enhance long-term survival. This article discusses several key molecular markers used in the progno

[Electrocardiogram in channel disorders].

The ion channel diseases of the heart are a collection of genetically distinct arrhythmogenic cardiovascular disorders resulting from mutations in fundamental cardiac ion channels that orchestrate the action potential of the human heart. Our understanding of these genetic "channelopathies" has increased dramatically from electrocardiographic depictions of QT prolongations, ST-T alterations and torsades de pointes and clinical descriptions of people experiencing syncope and sudden death to molecular revelations of perfurbed ion channel genes. These exciting molecular breakthroughs have provided

Gene therapy and nanomedicine for meningioma treatment.

Meningioma, a prevalent central nervous system (CNS) tumor, exhibits a broad spectrum of clinical behavior, ranging from benign to aggressive. Although surgical resection remains a viable treatment option for most cases, higher-grade meningiomas often require a multifaceted therapeutic approach. Gene therapies have shown potential for use in clinical intervention for the treatment of several types of cancer. Small nucleic acid molecules such as small interfering RNAs (siRNAs) and microRNAs (miRNAs) can restore aberrant gene expression and selectively silence oncogenic drivers. However, their c

From cross cancer transcriptomics to therapeutics: WGX-50 target hub genes in breast cancer and non-small cell lung carcinoma.

Breast cancer (BC) and non-small cell lung carcinoma (NSCLC) are two of the most prevalent and lethal malignancies, with shared molecular alterations that could serve as biomarkers for diagnosis and therapy. We employed integrated bioinformatics approaches to identify common hub genes in both cancers by analyzing eight GEO transcriptomic datasets. The protein-protein interaction network revealed ten hub genes: five upregulated (TOP2A, CCNB1, CCNB2, MELK, and UBE2C) and five downregulated (CDH5, MMRN2, RAMP2, CCM2L, and CD36), which were validated through transcriptomic analyses. These genes we

Differentiating IL-23 Inhibitors in Crohn's Disease.

Interleukin-23 (IL-23), a member of the IL-12 family of cytokines, plays a critical role in intestinal homeostasis and inflammation and is strongly implicated in the pathogenesis of inflammatory bowel disease (IBD). Therapies targeting the p19 subunit of IL-23 have recently expanded the therapeutic options for IBD demonstrating efficacy and safety for the treatment of moderate to severe Crohn's disease (CD). Thus, in this review, we provide an overview of agents targeting the IL-23 pathway in CD, highlighting similarities and differences of specific IL-23 inhibitors. Furthermore, we summarize

Beyond Swelling: Clinical Insights into the Diagnosis and Management of Hereditary Angioedema.

Hereditary angioedema (HAE) is a rare genetic disorder characterized by recurrent episodes of severe, non-pitting edema, most commonly caused by a deficiency or dysfunction of C1 esterase inhibitor (C1-INH), resulting in excessive bradykinin production. HAE subtypes differ according to C1-INH levels, functional activity, underlying genetic variants, and increasingly recognized molecular mechanisms. Awareness of HAE outside specialized medical fields remains limited, and recurrent angioedema may arise through different pathophysiological mechanisms, which together contribute to frequent misdiag

[Recent Advances in the Classification and Treatment of Pediatric Gliomas].

Pediatric gliomas constitute the largest group of pediatric brain tumors and are biologically distinct from adult gliomas. The 2021 fifth edition of the World Health Organization Classification of Tumors of the Central Nervous System (WHO CNS5) introduced a molecularly integrated classification system and recognized pediatric-type gliomas as distinct entities. In pediatric low-grade gliomas, activation of the MAPK pathway is a common molecular hallmark, with BRAF V600E mutations, BRAF fusions, FGFR1 alterations, and NF1-associated abnormalities representing major driver events. These discoveri

[Preventive and therapeutic strategies for relapse after hematopoietic stem cell transplant for pediatric AML (SFGM-TC)].

Treatment of pediatric high-risk acute myeloid leukemia (AML), defined either on molecular or cytogenetic features, relies on bone marrow transplant after cytologic remission. However, relapse remains the first post-transplant cause of mortality. In this 13th session of practice harmonization of the francophone society of bone marrow transplantation and cellular therapy (SFGM-TC), our group worked on recommendations regarding the management of post-transplant relapse in AML pediatric patients based on international literature, national survey and expert opinion. Overall, immunomodulation strat

[Ductal adenocarcinoma of the pancreas: subtypes and molecular pathology].

Ductal adenocarcinoma is the most common tumor of the pancreas. Although relatively rare, it poses one of the greatest oncological challenges because of its poor prognosis, which has so far only slightly improved. Progress has been made in the more precise classification and standardization of the morphological assessment. In the current WHO classification, prognostically relevant subtypes are clearly delimited among themselves and from ductal adenocarcinoma not otherwise specified (NOS). In the recent TNM classification, a size-based T‑category was introduced. Diagnostically, the histological

Hepatitis B Virus Pre-S Gene Deletions and Pre-S Deleted Proteins: Clinical and Molecular Implications in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is one of the most frequent and fatal human cancers worldwide and its development and prognosis are intimately associated with chronic infection with hepatitis B virus (HBV). The identification of genetic mutations and molecular mechanisms that mediate HBV-induced tumorigenesis therefore holds promise for the development of potential biomarkers and targets for HCC prevention and therapy. The presence of HBV pre-S gene deletions in the blood and the expression of pre-S deleted proteins in the liver tissues of patients with chronic hepatitis B and HBV-related HCC h
