Determinants for Quantitative Sensory Testing and the Association with Chronic Musculoskeletal Pain in the General Elderly Population.

OBJECTIVE: Chronic musculoskeletal pain is accompanied by central sensitization, which can be determined with quantitative sensory testing (QST). In this study, we aim to investigate whether central sensitization, as measured by thermal QST, is detectable in community-dwelling elderly individuals suffering from self-reported chronic pain and identify determinants influencing thermal QST measurement analyses and interpretation. METHODS: In 3,936 participants of the Rotterdam Study, cold and warmth sensitivity and heat pain thresholds were determined using the thermo-sensory analyzer TSA II (Me

Obesity Is Associated With Increased Complications After Operative Management of End-Stage Ankle Arthritis.

BACKGROUND: Total ankle arthroplasty (TAA) and ankle arthrodesis (AA) are two operative options for the management of end-stage ankle arthritis that has failed conservative interventions. Obesity is associated with a greater incidence of musculoskeletal disease, particularly osteoarthritis of the weight-bearing joints, including the ankle. The objective of the present study was to use a national database to examine the association between obesity and postoperative complications after TAA and AA. METHODS: The PearlDiver database was queried for patients undergoing AA and TAA using Internationa

Further delineation of Loeys-Dietz syndrome type 4 in a family with mild vascular involvement and a TGFB2 splicing mutation.

BACKGROUND: The Loeys-Dietz syndrome (LDS) is a rare autosomal dominant disorder characterized by thoracic aortic aneurysm and dissection and widespread systemic connective tissue involvement. LDS type 1 to 4 are caused by mutations in genes of the TGF-β signaling pathway: TGFBR1 and TGFBR2 encoding the TGF-β receptor (LDS1 and LDS2), SMAD3 encoding the TGF-β receptor cytoplasmic effector (LDS3), and TGFB2 encoding the TGF-β2 ligand (LDS4). LDS4 represents the mildest end of the LDS spectrum, since aneurysms are usually observed in fourth decade and the progression of the disease is slower tha

Assessing the outcomes of spine surgery using global positioning systems.

STUDY DESIGN: Pilot study. OBJECTIVE: To examine whether surgical outcomes can be assessed objectively by advanced tracking technology, based on Global Positioning Systems (GPS). SUMMARY OF BACKGROUND DATA: Outcome studies are the commonest way to assess the results of surgical procedures. The success of these efforts is impeded by a number of factors, including the lack of valid outcome measures, difficulty in assessing changes in patients' expectations (response shift) and confounding effects of secondary gains. METHODS: The measurement of walking speed, distances, and number of walking e

Comparative safety of atorvastatin 80 mg versus 10 mg derived from analysis of 49 completed trials in 14,236 patients.

Atorvastatin has been shown to reduce coronary events and revascularization procedures in patients with multiple risk factors for coronary heart disease. Recent studies with atorvastatin 80 mg support the overall safety of this dose during long-term treatment. However, physicians appear reluctant to use high doses of statins. A retrospective analysis of pooled data from 49 clinical trials of atorvastatin in 14,236 patients treated for an average period of 2 weeks to 52 months was conducted. The study compared the safety of atorvastatin 10 mg (n = 7,258), atorvastatin 80 mg (n = 4,798), and pla

Malignant lymphoma-associated autoimmune diseases--a descriptive epidemiological study.

Lymphoproliferative disorders and autoimmune diseases have some common aspects in their clinical appearance. We reviewed 940 patient charts with malignant lymphomas to assess the rate of associated autoimmune diseases. Of 421 non-Hodgkin's lymphoma (NHL) patients (230 males, 191 females), 32 (7.6%) had an autoimmune disease (26 females, six males, mean age 48.3 years). The most common diagnosis was Sjögren's syndrome. The other cases were autoimmune skin diseases (5), thyroiditis (3), polymyositis (2), scleroderma (2), other musculoskeletal disorders (2), rheumatoid arthritis (1), vasculitis (

Back pain.

Among the multitude of potential etiologies for low back pain in the elderly, cauda equina syndrome is the only genuine surgical emergency. The consequence of delayed recognition, referral, and decompression may be permanent neurologic deficit. Fortunately, this event is rare. Certain other warning signs and symptoms (as outlined in Fig. 1) should alert the clinician to possible systemic disease and warrant earlier, more aggressive evaluation. Careful history-taking and focused physical examination remain the cornerstones of clinical decision making in managing the geriatric patient with back

A Primer on COL4A1/COL4A2-Related Cerebral Small Vessel Disease for Neurologists.

COL4A1 and COL4A2 encode type IV collagen, a principal structural component of basement membranes, and pathogenic variants in these genes cause a multisystem small vessel vasculopathy with prominent cerebrovascular involvement. Despite increasing recognition with the expansion of genetic testing, COL4A1/2-related cerebral small vessel disease remains underrecognized in clinical practice, in part due to its striking phenotypic heterogeneity. Presentations span a broad clinical spectrum-from asymptomatic individuals or incidental imaging findings to recurrent ischemic events, intracerebral hemor

How are we representing the trunk? A narrative review on marker sets for kinematics analysis of neurological conditions.

BACKGROUND: Trunk movement plays a vital role in daily activities, influencing efficiency, balance, and musculoskeletal health-especially in individuals with neurological conditions. However, three-dimensional (3D) motion analysis of the trunk lacks standardisation, limiting both cross-study comparisons and clinical application. This review synthesises current approaches to trunk modelling, including marker setups, segment definitions, and kinematic outcomes, in studies involving participants with neurological conditions. METHODS: A systematic search of six databases, including MEDLINE and Sc

The gut-Joint axis: investigating functional gastrointestinal disorders as risk factors for prosthetic joint infections a retrospective case-control study.

BACKGROUND: Periprosthetic joint infection (PJI) is a severe complication of total joint arthroplasty (TJA), with traditional risk factors including diabetes and obesity. Emerging evidence suggests functional gastrointestinal disorders (FGIDs) may influence systemic inflammation and infection susceptibility. This study investigated whether preexisting FGIDs are independent risk factors for PJI. METHODS: A retrospective 1:1 matched case-control study analyzed 896 patients (448 PJI vs. 448 non-PJI) undergoing primary TJA (2013-2023). PJI was diagnosed using modified 2018 Musculoskeletal Infecti

Bile acid signaling in health and disease.

Bile acids (BAs), cholesterol-derived amphipathic metabolites which are traditionally regarded as digestive surfactants, are now recognized as pleiotropic signaling molecules that act through ligand-, receptor-, and tissue-specific mechanisms to maintain systemic homeostasis. They exert biological functions via nuclear receptors (FXR, PXR, VDR, CAR) and membrane receptors (TGR5, S1PR2, MRGPRX4). These receptors activate transcriptional and membrane-initiated programs, including FXR-SHP/FGF19 feedback, TGR5-cAMP-PKA signaling, S1PR2-ERK/AKT/RhoA pathways, MRGPRX4-dependent Ca2⁺ signaling, PXR/C

Artificial Intelligence-assisted MRI and CT-based diagnostic anatomical evaluation of experimental rat disease models.

Artificial intelligence (AI) emerged as the transformative technology in biomedical imaging to improve the accuracy, efficiency and reproducibility of disease diagnosis. Integration of multimodal imaging data for the automated anatomical assessment in preclinical disease models remains insufficiently explored. This study aimed to develop and validate an AI-assisted framework integrating the magnetic resonance imaging (MRI) and computed tomography (CT) for comprehensive anatomical evaluation of the experimental rat disease models. Count of 180 Sprague-Dawley rats were allocated into healthy con
