Medical and surgical considerations in patients with Samter's triad.

BACKGROUND: The aim of this study was to report on objective and subjective outcomes of patients with Samter's triad (ST) treated with functional endoscopic sinus surgery (FESS), and correlate these results with aspirin desensitization (DS) in patients. METHODS: We performed a retrospective analysis of prospectively collected data in 15 patients requiring revision FESS after failing maximum medical therapy and prior sinus surgery for chronic rhinosinusitis in the context of ST. Five patients underwent aspirin DS and 10 patients did not (non-DS). These patients represent a subset of patients p

Involvement of IL-9 in the bronchial phenotype of patients with nasal polyposis.

BACKGROUND: Nasal polyposis (NP) is frequently associated with asthma. In this disease, asymptomatic bronchial hyperresponsiveness (BHR) is thought to precede the development of asthma. IL-9 and its receptor have been reported as candidate genes for asthma and to be associated with BHR. OBJECTIVE: The objective of this study was to assess the contribution of 11-9 to the pathogenesis of BHR in NP by comparing the expression of IL-9 and its receptor in bronchial biopsy specimens from three groups of patients with NP: NP without BHR, NP with asymptomatic BHR, and NP with BHR and asthma. METHODS

[Olfactory disorders].

Smell and taste problems are of major importance to those who suffer from olfactory disorders. The inability to determine the presence of odors in the home and the markedly reduced capacity or incapacity to appreciate food flavors are key reasons given for limited social interaction. Patients experiencing distorted smells and tastes may avoid food, which results in weight loss and possible malnutrition. We present an overview of smell disorders, based on physiological considerations, with specific attention to clinical characteristics of conditions most commonly causing smell disorders.

Functional endoscopic sinus surgery in children and adults.

Functional endoscopic sinus surgery (FESS) has become a popular procedure for the treatment of adults with chronic sinusitis. Recently, it has also been applied in the pediatric population. It has been used to treat chronic sinusitis in our department since April 1988. As of March 1998, 1,112 patients were available for analysis by file and questionnaire review. Of the 1,112 patients, 104 patients (9.4%) who were 16 years of age or younger at the time they first underwent FESS in our department were included in the pediatric endoscopic sinus surgery (PESS) group. The other 1,008 patients (90.6

Phenotypes of alpha-1-antitrypsin in intrinsic asthma and ASA-triad patients.

The frequency of presence of various phenotypes of alpha 1-antitrypsin was studied in 31 patients with intrinsic asthma and 11 with ASA-Triad, and compared to a group of 200 people representative of the general population. The MZ and SZ phenotype is more frequent in intrinsic asthmatics (p < 0.00001) and MZ in ASA-Triad (p < 0.0005) than in the control group. No differences were found between the intrinsic asthmatics and ASA-Triad. All the patients were divided into groups according to their clinical characteristics and an increase of the MZ phenotype was observed (p < 0.001) in patients with:

The Past, the Present, and the Future of Periostin in Allergy.

Periostin is a matricellular protein that binds to its receptors, which include several integrin molecules but mainly αvβ3 integrin on cell surfaces, thereby transducing its signals into cells. Almost 20 years have passed since we found that periostin is involved in the pathogenesis of asthma, which marked the first evidence of its involvement in allergic diseases. Over the past 2 decades, cumulative evidence has demonstrated that periostin plays important roles in the onset or progress of many allergic diseases such as chronic rhinitis with nasal polyp, atopic dermatitis (AD), allergic conjun

Is asthma with hypersenitivity to non-steroidal anti-inflammatory drugs is a risk factor for depression?

UNLABELLED: The respiratory tract disease exacerbated by non-steroidal antiinflammatory drugs (NSAIDs) is characterized by the Widal triad, which includes the presence of bronchial asthma, hypersensitivity to NSAIDs (acetylsalicylic acid) and chronic eosinophilic rhinitis and sinusitis (frequently also coincident with nasal polyps). Idiosyncratic hypersensitivity to NSAIDs in the course of bronchial asthma affects approximately 15% of patients. AIM: The aim of the study is to assess depressiveness and severity of anxiety in the population of patients with partially and poorly controlled asthm…

Expression pattern of two related cystic fibrosis-associated calcium-binding proteins in normal and abnormal tissues.

This paper reports further study of the identity and function of a protein shown to be elevated in serum from cystic fibrosis (CF) patients and clinically normal heterozygotes. Monoclonal antibodies, specifically recognizing the tentatively named cystic fibrosis antigen (CFAg), were produced. Immunoaffinity purification of CFAg from several sources revealed two components: 11 x 10(3) and 14 x 10(3) Mr protein. cDNA clones corresponding to each protein have been isolated. Data-base comparisons of the deduced amino acid sequences suggest that both genes encode related but distinct calcium-bindin

Human circulating group 2 innate lymphoid cells can express CD154 and promote IgE production.

BACKGROUND: Protection against helminths consists of adaptive responses by TH2 cells and innate responses by group 2 innate lymphoid cells (ILC2s), with these latter being well characterized in mice but less so in human subjects. OBJECTIVE: We sought to characterize human circulating ILC2s and compare their functional profile with that of autologous TH2 cells. METHODS: Circulating ILC2s and TH2 cells were isolated by means of fluorescence-activated cell sorting and magnetic cell sorting and expanded in vitro. ILC2s were then stimulated with phorbol 12-myristate 13-acetate plus ionomycin, IL-

Effect of sodium hyaluronate added to topical corticosteroids in chronic rhinosinusitis with nasal polyposis.

BACKGROUND: Available medical treatments for chronic rhinosinusitis (CRS) with nasal polyposis (CRSwNP) comprise systemic and topical therapies. Although topical corticosteroids are effective in the treatment of CRS, they are not completely devoid of adverse effects. Thus, care has to be taken when long-term treatments are prescribed. There is recent evidence that sodium hyaluronate (SH), the major component of many extracellular matrices, promotes tissue healing, including activation and moderation of the inflammatory responses, cell proliferation, migration, and angiogenesis. OBJECTIVE: The

Increased expression of α7nAChR in chronic rhinosinusitis: The intranasal cholinergic anti-inflammatory hypothesis.

OBJECTIVE: Chronic rhinosinusitis results from a dysfunctional host-environment interaction at the site of interface, in the nose and paranasal sinuses. A parasympathetic-mediated anti-inflammatory reflex is known to have a pivotal role in the control of damage induced by immune response to injury and infection; acetylcholine released by peripheral nerves interacts with nicotinic acetylcholine receptor subunit α7 - α7nAChR - of innate immune cells, inhibiting pro-inflammatory signalling. This work aims to investigate whether cholinergic function is implicated in chronic rhinosinusitis. METHOD

Galectin fingerprinting in naso-sinusal diseases.

Galectins, a family of endogenous lectins, are multifunctional effectors that act at various sites and can be used in immunohistochemical localization studies of diseased states. Since they form a potentially cooperative and antagonistic network, we tested the hypothesis that histopathological fingerprinting of galectins could refine the molecular understanding of naso-sinusal pathologies. Using non-cross-reactive antibodies against galectin-1, -3, -4, -7, -8 and -9, we characterized the galectin profiles in chronic rhinosinusitis, nasal polyposis, inverted papillomas and squamous cell carcino

Toward photorealism in endoscopic sinus surgery simulation.

BACKGROUND: Endoscopic sinus surgery (ESS) is the surgical standard treatment for chronic rhinitis/rhinosinusitis and nasal polyposis. There is a reported complication rate of 5-10% associated with this type of surgery. Simulation has been advocated as a means to improve surgical training and minimize the rates of complication and medical error. This study aimed to show how a virtual reality ESS simulator was developed, with particular emphasis on achieving satisfactory photorealism and surgical verisimilitude. METHODS: Sinus computed tomography scans were processed to create a triangle-based
