Four cases of unreduced (2-11 months postinjury) anterior hip dislocation are reported. One public dislocation 2 months postinjury was treated by open reduction with a fair result. A unique case of iliac dislocation 11 months postinjury, overlooked due to an associated femoral shaft fracture, was treated by valgus/derotation osteotomy. Two obturator dislocations 7.5 and 9.5 months postinjury were treated by a subcapital osteotomy and displacement of the femoral neck into the acetabulum (modified excision arthroplasty). Both of these patients had fairly stable, painless, and mobile hips at 2.1

BACKGROUND: Metatarsal stress fractures account for approximately 28% of all stress fractures reported in the literature. Second and third metatarsal stress fractures are most common, followed by fractures of the fourth and fifth metatarsal. Reports in the literature suggest that proximal fourth metatarsal stress fractures have the same propensity for delayed healing and nonunion as do fifth metatarsal stress fractures. There is a paucity of literature on operative management of fourth metatarsal fractures. This study reports the results of operative management of fourth metatarsal stress frac

UNLABELLED: This report cites new developments in the treatment of extra-articular hand fractures in adults. Recent reports confirm that small amounts of metacarpal shortening or dorsal angulation cause minimal functional impairment. Unilateral excision of the lateral band and oblique fibers of the extensor apparatus of the metacarpophalangeal joint facilitates proximal phalangeal fracture exposure and may improve functional recovery. Results using open mini screw fixation of oblique extra-articular metacarpal and phalangeal fractures may be comparable to those of percutaneous Kirschner wire f

Tibial plateau fractures (TPFs) are orthopedic challenges with multiple injury modalities and clinical presentations. TPFs are often classified using the Schatzker classification system, which can dictate management. In our case, a 37-year-old male presented at an orthopedic specialty hospital with right knee pain after a fall from a truck ramp. X-rays and CT imaging demonstrated a comminuted bicondylar TPF in the emergency room with metaphyseal dissociation. The patient was placed in a knee immobilizer, made non-weight bearing, and scheduled for outpatient follow-up with a local orthopedic su

This study was undertaken to document whether a basic set of a new unilateral external fixation device could provide sufficient stability in complex musculoskeletal injuries involving upper and lower extremities and in patients with unstable pelvic ring disruptions. The initial clinical evaluation was performed in Sweden (stage 1) followed by field evaluation at the Swedish Field Hospital during the Mogadishu conflict in Somalia (stage 2). In stage 1, there were 90 patients with 116 acute limb injuries and six patients with an unstable pelvic ring disruption. Ninety limb injuries and all six p

We report the results of treatment in eight patients who had posteromedial dislocation of the ankle without fracture. The ages of seven of the eight patients ranged from twenty-three to thirty-six years. Four patients had increased generalized ligamentous laxity. One-half of the injuries were sustained during athletic competition. The mechanism of injury appeared to be forced inversion of the foot when it was maximally plantar flexed and axially loaded. Five of the eight patients had an open injury, and four of the five patients had an associated injury to the anterolateral neurovascular struc

BACKGROUND: Post-traumatic stiffness after open reduction and internal fixation of fractures about the knee can have dramatic effects on function. Traditionally, open quadricepsplasty has been the treatment of choice, but is associated with significant morbidity.
PURPOSE: The purpose of this study is to examine the immediate and sustainable range of motion (ROM) changes after surgical arthroscopic lysis of knee adhesions (SALKA) for post-traumatic knee stiffness after open reduction internal fixation (ORIF).
METHODS: We retrospectively reviewed a consecutive series of patients at a single in

UNLABELLED: Complex intra-articular calcaneal fractures often resulted in secondary pain and deformity, requiring subsequent subtalar arthrodesis1-3. The literature suggests that primary subtalar arthrodesis in the acute period has good functional results2,3. The literature also demonstrates that posterior arthroscopic subtalar arthrodesis for chronic arthritis has favorable results5. Thus, we propose an approach to treating these difficult intra-articular calcaneal fractures that utilizes a posterior arthroscopic primary subtalar arthrodesis technique-aptly named Calcaneus Fracture and Poster

Fractures of the trapezium are rare and are usually treated by open reduction and internal fixation. The trapeziometacarpal joint is difficult to visualize with standard radiographic techniques; proper visualization of the joint surface of the scaphometacarpal and trapeziometacarpal joints is paramount. A 36-year-old male patient complained of isolated wrist pain and had limitation of wrist function after a motor vehicle accident. The injury was closed without obvious deformity. Radiographs showed a longitudinal shear fracture of the trapezium. Five days later, the patient was taken to surgery

This paper describes an image analysis method that evaluates the glenoid and humerus bone morphology to automate the gleno-humeral (GH) joint diagnoses and surgical managements. This method uses radial B-spline curves to approximate ellipse-like shoulder structures including the humeral stem, tubercle and contact joint as well as the glenoid on every CT transverse section. Radius changes from structure centers to bone boundaries are recognized as convex, concave, separate and hole features that are then identified as pathological spurs, fractures and tumors. The centers and radii of these stru

A retrospective review of 46 consecutive patients with complex (Schatzker V and VI) tibial plateau fractures treated at Harborview Medical Center between 1984 and 1989, disclosed a subset of 14 grade II or III (Gustilo) open injuries. We wished to determine the incidence of infection, union rate, and the number of operations required to achieve a satisfactory result, based on a treatment protocol: alignment and splinting of fracture at the scene of injury if possible, antibiotics administered in the emergency room (ER) and continued for 48 h, and admission of patient to the operating room as q

BACKGROUND: This study investigates whether open distal radius fractures (ODRFs) treated after 24 hours from time of injury have an increased risk of infection or overall complication profile compared with those treated within 24 hours.
METHODS: Retrospective review was performed of all patients treated for ODRF over a 6-year period at a single large academic institution. Postoperative complications included surgical site infections, need for revision irrigation and debridement, delayed soft tissue healing, loss of reduction, nonunion, and malunion.
RESULTS: One-hundred twenty patients were

BACKGROUND: The most common first-line fixation technique for simple Weber B fibula fractures is a lag screw with lateral neutralization plate. The most common surgical technique for unstable Weber B fibula fracture is one-third semi-tubular plate and cortical screws, implemented with lag screw when appropriate. However, the lag technique can be technically challenging in osteoporotic bone or within fibulas of smaller diameter, and in some cases can result in fragmentation at the fracture site, malreduction, or peroneal irritation. The purpose of this study is to examine an alternative first-l

The induced membrane technique was initially described by Masquelet et al. in 1986 as a treatment for tibia nonunion; then, it became one of the established methods in the management of bone defects. Several changes have been made to this technique and have been used in different contexts and different methodologies. We present the case of a 16-year-old girl admitted to our department for a polytrauma after a motorcycle accident. She presented a Gustilo III-A open fracture of the right femoral shaft with a large bone defect of 8 centimeters that we treated with a modified Masquelet technique.

Thoracic spine fracture-dislocations are typically the product of high-energy injuries, and their neurological sequelae are usually devastating. For a patient to experience such an injury from a minor trauma and preserve all neurological function is rare and is often credited to the paradoxical decompression effect that follows a traumatic fracture of the posterior elements of the spine. This is a case of a 31-year-old female patient who suffered a severe thoracic spine fracture-dislocation from a low-energy fall from a manually operated "merry-go-round" in a kids' park. Despite the apparent i

BACKGROUND Lateral condyle fracture of the humerus in children should be diagnosed and treated quickly to avoid the complications of malunion and varus deformity of the elbow. Worldwide, pediatric orthopedic departments experienced delays in patient diagnosis and treatment during the COVID-19 pandemic. This retrospective study from a single center in Turkey aimed to compare outcomes from early treatment, elective treatment, and delayed treatment in 140 children with lateral condyle fracture of the humerus between 2013 and 2021. MATERIAL AND METHODS In the study, 140 patients with Milch type 2
