Oculovagal reflex in paediatric orbital floor fractures mimicking head injury.

BACKGROUND: The oculovagal reflex is well described in ophthalmic surgery, but may be caused by any manner of pressure on the globe. Children with orbital blowout fractures present in a different manner from adults. The classic presentation in children is a white eye injury (ie, no subconjunctival haemorrhage) with upgaze diplopia and general malaise. METHODS: A retrospective audit is presented of paediatric and young adult patients referred to the Orbital Service at St George's Hospital who required surgical intervention. RESULTS: One-third of children with orbital blowout fractures are adm…

Unusual orbital lymphoma undetectable by magnetic resonance imaging: a case report.

INTRODUCTION: We report the case of a patient with orbital malignant lymphoma that was not detected by imaging studies when she presented with impaired vision, which lead to her eventual loss of sight. CASE PRESENTATION: A 71-year-old Japanese woman complained of deteriorating vision in her left eye. On examination, papilledema was detected, but magnetic resonance imaging only showed slight thickening and enhancement of the left optic nerve. A diagnosis of idiopathic optic neuritis was made and corticosteroid pulse therapy was administered. During the next four months, the patient received a …

Ectopic orbital brain diagnosed 20 years after symptomatic presentation.

Ectopic brain in the orbit is a rare entity with only 14 other case reports in the literature. This case is unique in that symptomatic presentation occurred 20 years prior to diagnosis. Symptoms are generally due to mass effect. We report an unusual case of diplopia secondary to ectopic orbital brain and a review of the literature.

A consecutive case review of orbital blowout fractures and recommendations for comprehensive management.

BACKGROUND: The orbital blowout fracture is a common facial injury, but full consensus has not been reached regarding its optimal management. The authors retrospectively explored consecutive cases of blowout fractures and proposed new recommendations for treatment. METHODS: Two hundred eight newly registered patients were selected from the database of Nagasaki University Hospital over the past 5 years. One hundred nine patients in the authors' department were then reviewed regarding computed tomographic classification of fracture types, preoperative complaints, and outcomes. RESULTS: Of the …

New technique for reconstructing the affected cranium and orbital rim in unicoronal craniosynostosis.

Of the single-suture craniosynostoses, unicoronal synostosis (UCS) is widely acknowledged to represent the most varied and complex set of craniofacial deformities. This is attributed to the endocranial base being affected along with the coronal suture. This deformity may present with both coronal and frontosphenoidal synostosis and has been associated with elevated intracranial pressure; brain morphological abnormalities; vertical dystopia; ambylopia; malformations of the forehead, orbit, midface, and mandible; and behavioral, cognitive, and speech abnormalities. It is the variable structural …

Unilateral orbital inflammation in a child after a jellyfish sting to the lower extremities.

A 3-year 10-month-old child initially developed locally recurrent cutaneous eruptions within the first 2 weeks after sustaining a jellyfish sting to her lower extremities. After 5 asymptomatic weeks, she developed unilateral orbital inflammation that did not respond to systemic antibiotics, antihistamines, or steroids. Imaging revealed a rapidly growing mass of the right lacrimal gland. Urgent anterior orbitotomy was performed and the lacrimal gland was biopsied. Histopathologic diagnosis revealed sclerosing dacryodenitis consistent with orbital inflammatory syndrome and/or an immune response …

Orbital complications due to an acute odontogenic focus in a child. A case report.

The full clinical manifestations of complications due to odontogenic foci are rarely seen in daily dental practice and can take a clinically foudroyant course of development in young people owing to anatomic conditions, as demonstrated in this clinical case in a 12-year-old girl. Endodontic treatment of the first right upper molar was started owing to increasing toothache and swelling of fossa canina and the periorbital region. During the course of treatment, the patient exhibited an acute increase in orbital inflammation, which required immediate surgical intervention with postsurgical intrav…

Rhino-orbital-cerebral mucormycosis: a lethal complication of body dysmorphic disorder.

Young diabetics can sometimes have difficulty with insulin compliance for various reasons. This can be exacerbated if associated with body dysmorphic disorder or other eating disorders. The manipulation of insulin to lose weight can have devastating consequences. The authors describe a fatal case of rhino-orbital-cerebral mucormycosis secondary to insulin manipulation for the purpose of weight loss.

Cemento-ossifying Fibroma Of Paranasal Sinus Presenting Acutely As Orbital Cellulitis.

Fibro-osseous lesions of the face and paranasal sinuses are relatively uncommon. These lesions have overlapping clinical, radiologic and pathologic features causing difficulty in diagnosis. Neoplastic fibro-osseous paranasal sinus lesions can be benign or malignant. The benign fibro-osseous lesions described are: ossifying fibroma (and its histologic variants) and fibrous dysplasia. The variants of ossifying fibroma differ in the nature of calcified material (i.e. cementum versus bone), in the location of the lesion (oral versus paranasal sinus or orbital), other morphologic variations (presen…

Dentigerous cyst of the maxilla with impacted tooth displaced into orbital rim and floor.

We report a case of dentigerous cyst of the maxilla and maxillary sinus that caused the ectopic displacement of an unerupted tooth into the orbital rim and floor. After an incisional biopsy, marsupialization of the lesion promoted its involution and stimulated osteogenesis. This in turn simplified the surgical enucleation of the specimen and removal of the unerupted tooth without the excessive loss of the bony contours of the maxilla.

Use of high-resolution microscopy coil MRI for depicting orbital anatomy.

OBJECTIVE: High-resolution MRI (HR-MRI) is a powerful non-invasive tool that provides images of higher spatial resolution and enables visualization of tissues previously unidentified with conventional techniques. The utilization of HR-MRI in the eye and orbit is essential due to the minute structure with great tissue diversity. The purpose of this study is to investigate the use of a novel surface coil and to explore the potential of this approach to depict normal anatomy. METHODS: MR images were acquired using a commonly available 1.5T scanner. Ten normal volunteers were imaged using a surfa…

Combined central retinal artery and vein occlusion complicating orbital cellulitis.

Orbital Cellulitis is a dreaded ophthalmologic disease. It may destroy vision and the eye and may even become life threatening. Often visual loss is the result of exposure and subsequent destruction of ocular tissue commonly the cornea and the uvea. We report a case of combined central retinal artery and vein occlusion complicating orbital cellulitis in a 35 year old patient who was 37 weeks pregnant resulting in loss of vision in the affected eye. There have been few case reports of this type of complication of orbital cellulitis.

Preseptal and orbital cellulitis: 15-year experience with sulbactam ampicillin treatment.

The infection of the orbita and ocular tissues can result in severe local and systemic complications. We aimed to determine the predisposing factors for preseptal and orbital cellulitis, the clinical and routine laboratory differences between orbital and preseptal cellulitis, and the change in the spectrum of the pathogens and the antibiotics used in the last 10 years. One hundred thirty-nine patients, hospitalized in Hacettepe University Faculty of Medicine Children's Hospital between 1 January 1990 and 31 December 2003 with diagnosis ofperiorbital or orbital cellulitis, were reviewed retrosp…

Transconjunctival orbital fat repositioning for tear trough deformity in young Asians.

BACKGROUND: Tear trough deformity is a primary complaint in those who believe it causes an old and tired appearance. This deformity naturally increases with aging, but there are younger people that also complain about the appearance of their lower eyelid area. Recently, many authors have reported good results treating this deformity with transconjunctival orbital fat repositioning. Although Kawamoto has pointed out a possible need for a procedure modification in Asians with a brachycephalic face, thus far, there have been no reports evaluating such procedures in Asians. OBJECTIVE: In November…

[Surgical treatment of diffuse adult orbital lymphangioma: two case studies].

PURPOSE: Orbital lymphangioma is a rare vascular malformation; it is a benign but severe anomaly because of its infiltrative, diffuse, and hemorrhagic nature, and its high morbidity rate. Surgical resection is a real challenge on account of the intricate architecture of the lesion. The authors report their surgical experience concerning two cases of diffuse orbital lymphangioma whose diagnosis was established in adulthood and whose surgical treatment was successful. CASE REPORTS: Two patients presented with adult orbital lymphangioma. Progression was slow during the first decade and then was …
