A protocol for a randomized comparison of extended-release versus sublingual buprenorphine among pre-trial detainees in jail.

CONTEXT/BACKGROUND: The high prevalence of opioid use among jailed adults offers an unmatched opportunity to identify and treat those with opioid use disorder (OUD), a population that is at a substantial risk for post-release overdose. From a public health perspective, jails are critical touchpoints, as these facilities typically admit more than 7 million adults per year. One clinical consideration is whether pre-trial detainees with OUD would benefit from early induction onto extended-release buprenorphine (XRB). METHODS/STUDY DESIGN: In this 3-year randomized controlled trial, we will ident

Allocative efficiency analysis of strategies to reduce overdose deaths among people with opioid use disorder and history of incarceration in Connecticut.

BACKGROUND: While Connecticut has successfully slowed overdose death (ODD) rates, additional progress is necessary. We examined policies that allocate resources with maximal efficiency to reduce ODDs. METHODS: We developed a mechanistic simulation of overdose policies, including medications for opioid use disorder (OUD) among people involved in the criminal justice system both during incarceration (MOUD-INC) and post-release in the community (MOUD-COM), and naloxone in the community (NLX) to determine how maximally scaling all permutations compared to current MOUD and NLX levels (i.e., status

The Intersecting Effects of Lifetime Experiences of Homelessness and Incarceration on Cognitive Aging and Dementia Risk Factors in the United States.

BACKGROUND AND OBJECTIVES: Alzheimer's disease and related dementias (ADRD) remain a pressing concern in the United States, which also has one of the highest incarceration rates worldwide. Existing research has analyzed dementia risk and care among currently incarcerated and homeless populations; this paper fills a gap by examining later-life cognitive disparities facing formerly incarcerated and/or homeless individuals. RESEARCH DESIGN AND METHODS: Using Health and Retirement Study data (1998-2018), we tested whether formerly homeless and/or incarcerated people had earlier onset of cognitive

Role of homelessness and incarceration in time-to-decision on veterans affairs disability benefits for a mental health condition.

Homelessness and incarceration are major psychosocial concerns in the United States that may be mitigated by access to disability benefits. Data from the Veterans Benefits Administration (VBA) provides a unique opportunity to study how the process of obtaining disability benefits and time-to-decision might be affected by homelessness and/or incarceration among veterans in the Veterans Health Administration (VHA). The purpose of this study was to examine how homelessness and/or incarceration status among VHA-enrolled U.S. veterans might affect outcomes of VBA disability benefits claims for a me

Opioid-related harms and experiences of care among people in justice settings in New South Wales, Australia: evidence from the National Ambulance Surveillance System.

BACKGROUND: People in justice settings experience higher rates of psychiatric morbidity, including alcohol and drug use disorders, compared with the general population. However, our understanding of opioid-related harms in justice settings is limited. This study used ambulance data to examine opioid-related harms and experiences of care in New South Wales (NSW), Australia, during periods of incarceration or detention. METHODS: This mixed-methods study used data from the National Ambulance Surveillance System (NASS) for patients aged 18 and older with an opioid-related ambulance attendance bet

PrEP attitudes, willingness, and preferences among men incarcerated in jail in Massachusetts.

BACKGROUND: People who inject drugs (PWID) are both disproportionately incarcerated and affected by HIV infection. Systemic inequities perpetuate the cyclic nature of injection drug use (IDU) and incarceration, and both IDU and incarceration are linked to higher rates of HIV infection. Pre-exposure prophylaxis (PrEP) is highly effective in HIV prevention and is currently available as a daily oral pill. Longer-acting PrEP options, such as injectables and implants, are also in development to improve accessibility and adherence. Despite these advancements, PrEP uptake remains low among PWID and i

"Don't Tell Nobody Nothing": A Phenomenological Study on Fear of HIV Disclosure Among Older African Americans During Incarceration Living in Kentucky, USA.

The intersections of HIV with aging, incarceration, and racial disparities have been well studied, but little is known about the lived incarceration experiences of older African American persons living with HIV. This phenomenological study examined the fear of HIV disclosure among older African Americans living in Kentucky who have experienced incarceration. Based on the interviews analyzed, five main themes were identified: experiences with fear, experiences pertaining to confidentiality, experiences with disclosure, experiences with stigma, and the desire to be accepted. These themes demonst

Emergency department utilization by individuals with opioid use disorder who were recently incarcerated.

INTRODUCTION: Individuals with opioid use disorder (OUD) are highly represented among the incarcerated population and are frequent utilizers of the emergency department (ED). Medications for opioid use disorder (MOUD) are a recognized treatment option for individuals with OUD. Although the field recognizes the benefits of MOUD, we know little about what mitigating effects MOUD offered in jail might have on post-release ED utilization. METHODS: In this retrospective cohort analysis, we searched electronic medical records (EMR) for incarcerations in the Santa Clara County jail between 8/1/2019

Incarceration and adversity histories: Modeling life course pathways affecting behavioral health.

A consequence of a growing incarceration rate is that an increasing number of children face having an incarcerated household member, a known contributor to diverse lifelong behavioral health risks such as substance use and mental health impairment. Few studies have explored how household incarceration uniquely contributes to these subsequent behavioral health concerns, nor mediational contributors to these associations, within a theoretical framework. Using state Behavioral Risk Factor Surveillance System survey data (n = 14,001), this study tests pathways of household incarceration and Advers

Pregnancy Status, Risk Factors, and Opportunities for Referral to Care Among Reproductive-Aged Women With Newly Reported Chronic Hepatitis C Virus Infection in Tennessee.

OBJECTIVES: From 2006 through 2012, the number of acute hepatitis C virus (HCV) infections increased 364% among persons aged ≤30, including reproductive-aged women, in Central Appalachian states. Outreach to reproductive-aged women with newly diagnosed HCV infection affords a unique opportunity to provide counseling, further testing, and linkage to treatment. We modeled a centrally located statewide effort to reach this population and their health care providers to ascertain pregnancy status, HCV risk factors, and opportunities for linkage to additional services. METHODS: Using the Tennessee

A New Kind of Nursing: Walking the labyrinth of Cook County Jail.

This paper examines the application of public health nursing education with an emphasis on the social determinants of health within a correctional facility. The innate tension between public health practice and the punitive nature of correctional confinement is discussed through the perspective of student involvement. Exemplars of unexpected contingencies endemic to this practice setting are provided as are insights gleaned from this pilot collaboration between a college of nursing and a county correctional system.

Acceptability of HIV Testing Among Jail Inmates When Combined With a Blood Test for Tuberculosis.

Incarcerated individuals are at high risk of HIV and tuberculosis (TB), though combined screening in the criminal justice system is limited. Newer blood tests for TB provide a unique opportunity to combine HIV and TB testing in corrections. Entering jail inmates were offered routine opt-out HIV testing along with TB screening, both via blood draw. Of the 529 individuals enrolled, 524 (99%) agreed to HIV testing. Testing identified 13 (2.5%) previously diagnosed HIV infections and one new positive. One quarter (138, 26%) of participants had never been tested for HIV and were more likely to be y

Using a community-engaged health informatics approach to develop a web analytics research platform for sharing data with community stakeholders.

Marginalized communities are rarely included in the planning of research relevant to their own health or have access to research data. We implemented a community-based participatory research approach in developing a new health informatics system, WARP (Web Analytics Research Platform), to enable stakeholders to access and analyze research data. We leveraged data from a cohort study of 751 patients in the Transitions Clinic Network (TCN), a network of clinical programs serving patients with a history of incarceration. WARP holds de-identified, patient data, streamlines data processing (i.e. tra

Promoting Recovery Identities Among Mothers with Histories of Addiction: Strategies of Family Engagement.

Changes in identity are critical to managing transitions to recovery from substance and alcohol addictions. Identity change is particularly important for mothers, whose recovery processes are often in the context of critical but complex family relationships and societal expectations. But research and practice often underestimate the relational dimensions that promote or inhibit changes in one's identity during recovery. Here we analyze data from a study that involved interviews with 30 formerly incarcerated women participating in a community-based substance use treatment program in the Midwest
