Examining the relationships between prenatal methamphetamine exposure, early adversity, and child neurobehavioral disinhibition.

Methamphetamine use is a growing problem among pregnant women in the United States. Many negative consequences of methamphetamine use have been documented for the users, but little research has examined the long-term association between prenatal methamphetamine exposure (PME) and childhood outcomes. The current study examined the extent to which PME was predictive of childhood neurobehavioral disinhibition (ND), as well as the extent to which early adversity mediated this relationship. A sample of 320 mother-infant dyads (162 PME) was followed from birth through 6.5 years of age. ND was concep

Sleep disturbances associated with Parkinson's disease.

Sleep disturbances are common problems affecting the quality life of Parkinson's disease (PD) patients and are often underestimated. The causes of sleep disturbances are multifactorial and include nocturnal motor disturbances, nocturia, depressive symptoms, and medication use. Comorbidity of PD with sleep apnea syndrome, restless legs syndrome, rapid eye movement sleep behavior disorder, or circadian cycle disruption also results in impaired sleep. In addition, the involvement of serotoninergic, noradrenergic, and cholinergic neurons in the brainstem as a disease-related change contributes to

Sleep disturbances in children with attention-deficit/hyperactivity disorder.

In this article, we advocate the need for better understanding and treatment of children exhibiting inattentive, hyperactive, impulsive behaviors, by in-depth questioning on sleepiness, sleep-disordered breathing or problematic behaviors at bedtime, during the night and upon awakening, as well as night-to-night sleep duration variability. The relationships between sleep and attention-deficit/hyperactivity disorder (ADHD) are complex and are routinely overlooked by practitioners. Motricity and somnolence, the most consistent complaints and objectively measured sleep problems in children with AD

[Pathological buying. A review of the current knowledge regarding this condition of behavioral excess].

Compulsive buying is characterized by frequent excessive purchasing of items that are primarily not needed or used. The compulsive buying behavior results in mental, social, financial and often legal problems. Although compulsive buying affects a significant percentage of the general population and has received increasing attention in research, it has largely been ignored in clinical practice. Compulsive buying disorder is currently conceptualized as an"impulse control disorder not otherwise specified". However, the appropriate classification continues to be debated. Compulsive buying is assoc

Impulsivity in mania.

Impulsivity, a breakdown in the balance between initiation and screening of action that leads to reactions to stimuli without adequate reflection or regard for consequences, is a core feature of bipolar disorder and is prominent in manic episodes. Catecholaminergic function is related to impulsivity and mania. Manic individuals have abnormal dopaminergic reactions to reward and abnormal responses in the ventral prefrontal cortex that are consistent with impulsive behavior. Impulsivity in mania is pervasive, encompassing deficits in attention and behavioral inhibition. Impulsivity is increased

Examining the relationship between binge eating and coping strategies and the definition of binge eating in a sample of Spanish adolescents.

The present study had two major goals: to explore the relationship between binge eating and coping strategies in a sample of Spanish adolescents and to examine the adolescents' concept of binge eating. Two hundred and fifty-nine adolescents from a secondary school completed the Adolescent Coping Scale (ACS; Frydenberg & Lewis, 1993) and the Bulimic Investigatory Test, Edinburgh (BITE; Henderson & Freeman, 1987), as well as additional questions regarding the binge-eating episodes. The results show that the adolescents who reported binge eating used more avoidance coping strategies than those wh

Validation of the multiple language versions of the Hopkins Symptom Checklist-37 for refugee adolescents.

The objective of this study is to provide preliminary psychometric properties of the Hopkins Symptom Checklist-37 (HSCL-37A) for refugee adolescents. The HSCL-37A is a modification of the well-known HSCL-25 and assesses symptoms of internalizing and externalizing problems that have been associated with reactions to trauma. Four independent heterogeneous samples (N=3890) of unaccompanied refugee minors, immigrants, and native Dutch and Belgian adolescents were assessed at school. The confirmative factor analyses, per language version, support the two-factor structure of internalizing and extern

The assessment of alexithymia in medical settings: implications for understanding and treating health problems.

The construct of alexithymia encompasses the characteristics of difficulty identifying feelings, difficulty describing feelings, externally oriented thinking, and a limited imaginal capacity. These characteristics are thought to reflect deficits in the cognitive processing and regulation of emotions and to contribute to the onset or maintenance of several medical and psychiatric disorders. In this article, we review recent methods for assessing alexithymia and examine how assessing alexithymia can inform clinical practice. Alexithymia is associated with heightened physiological arousal, the te

Susceptibility of middle adolescent females to sexually transmitted infections: impact of hormone contraception and sexual behaviors on vaginal immunity.

PROBLEM: The resistance and/or susceptibility to infections can be influenced by patterns of immunomodulators. Based on this and the high rate of sexually transmitted infections (STIs) in adolescents, we examined the longitudinal relationship between sexual behaviors, hormonal contraceptive use, and bacterial vaginosis (BV) with vaginal-associated immunomodulators in adolescent females. METHOD OF STUDY: Over 27 months, subjects completed detailed questionnaires, and consented to vaginal swabs for STI testing, and vaginal lavages for identification of immunomodulators including T-helper, proin

Advantages of mixed effects models over traditional ANOVA models in developmental studies: a worked example in a mouse model of fetal alcohol syndrome.

Developmental studies in animals often violate the assumption of statistical independence of observations due to the hierarchical nature of the data (i.e., pups cluster by litter, correlation of individual observations over time). Mixed effect modeling (MEM) provides a robust analytical approach for addressing problems associated with hierarchical data. This article compares the application of MEM to traditional ANOVA models within the context of a developmental study of prenatal ethanol exposure in mice. The results of the MEM analyses supported the ANOVA results in showing that a large propo

Association of violence victimization with inconsistent condom use in HIV-infected persons.

The association of violence victimization with current condom use in HIV-infected persons was examined in this cross-sectional study. The HIV-Alcohol Longitudinal Cohort (HIV-ALC) recruited HIV-infected participants with a history of alcohol problems. Interviews assessed violence histories and current sexual behaviors. Of the 349 participants (79% men), 38% reported inconsistent condom use and 80% reported a violence history. Lifetime sexual violence was reported by 40% and lifetime physical violence (without sexual violence) by 40%. Participants reporting lifetime sexual violence had greater

Emerging treatments for child and adolescent social phobia: a review.

Social anxiety disorder, or social phobia (SP), is an anxiety disorder characterized by excessive fear of exposure to situations that involve potential scrutiny by others. SP is a common psychiatric problem in children and adolescents, often presenting with comorbid anxiety and mood disorders. Although the onset of SP is typically in late childhood or early adolescence, most afflicted individuals go undiagnosed for years, not seeking treatment until adulthood. First-line treatments for SP in adults support the use of pharmacotherapy and cognitive behavioral therapy. There is new and emerging d

Cognitive versus behavioral treatment of concurrent alcohol dependence and agoraphobia: a pilot study.

With the growing awareness of the prevalence of anxiety disorders among alcohol abusers there is a need for effective cognitive--behavioral treatments (CBTs). This study is a pilot investigation comparing two treatments for concurrent alcohol dependence and panic disorder with agoraphobia. A 10-session behavioral treatment (BT), consisting of five sessions treating alcohol dependence and five sessions treating panic disorder with agoraphobia, was compared to a 10-session cognitive treatment (CT) that addressed the dysfunctional cognitions mediating the alcohol problem and anxiety symptoms. Understanding relationships among trauma, post-tramatic stress disorder, and health outcomes.

Since Selye first published his classic work, The Stress of Life, a great deal of research has investigated the relationship between stress and physical health. Most of this research has focused on stressors such as divorce, bereavement, and job loss, but some has examined the health effects associated with extreme stressors, including war, sexual victimization, disasters, and serious accidents. The evidence presented here shows that poor physical health should be recognized, along with mental health problems and impaired psychosocial functioning, as an outcome of traumatic exposure. PTSD and

Understanding and caring for the child with Asperger syndrome.

Nurses in a variety of settings encounter children with the unfamiliar diagnosis of Asperger syndrome (AS). This disorder, which falls clinically along the autism spectrum, is receiving increasing attention because of its inclusion in the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) as one of the pervasive developmental disorders. The characteristic features of AS include deficits in social skills, atypical understanding of and use of pragmatic language, behavior problems, and a restricted set of interests. Cognitive abilities vary, and some children with AS have high intelli

Skin signs of sexually transmitted diseases in adolescents.

Sexually transmitted diseases (STDs) continue to be a significant and prevalent problem among adolescents. One in four sexually active teenagers are infected with one or more STDs. Certain STDs, including gonorrhea, chlamydia, and possibly human papillomavirus infections have the highest prevalence in this age group. The determinants of risk for STDs in adolescence encompass a range of behavioral, biological, and social factors. This review focuses on the skin signs of STDs in adolescents. Recognition of these findings is important since they may represent the most prominent, earliest, or only

The Kleine-Levin syndrome as a neuropsychiatric disorder: a case report.

The Kleine-Levin syndrome (KLS) is characterized by periodic, sudden-onset episodes of hypersomnia, compulsive hyperphagia, and behavioral-emotional disorders (typically indiscriminate hypersexuality, irritability, impulsive behaviors), lasting from a few days to a few weeks, with almost complete remission in the intercritical periods. Depression, confusion, and thought disorders are frequently associated with the critical symptomatology, and they may suggest other psychiatric diagnoses (schizophrenia, mood disorder, conversion disorder) or a substance abuse. Couples' drinking patterns, intimate partner violence, and alcohol-related partnership problems.

PURPOSE: Past research has revealed a strong correspondence between alcohol use and family dysfunction (e.g., Straus and Gelles in 1986 and 1990). Unfortunately, much of this research has relied on reports from only one family member. METHODS: Here, we present the results from a recent, nationwide study of alcohol-consumption patterns, alcohol-related partnership problems, and intimate partner violence (IPV) based on reports from both romantic partners of 1,615 married and cohabiting couples. RESULTS: Using the "drinking partnerships" construct developed by Roberts and Leonard in 1997, we fo

The behavioral morbidity of obstructive sleep apnea.

The behavioral morbidity associated with obstructive sleep apnea (OSA) includes symptoms of excessive daytime sleepiness (EDS), neurocognitive deficits, psychological problems, and possibly an increased chance of accidents. EDS is among the most frequently reported symptoms in patients diagnosed with OSA. The available data suggest that the primary cause of EDS is sleep fragmentation. The subjective measures of sleepiness include the sleep wake activity inventory and the epworth sleepiness scale. Sleepiness can also be evaluated objectively in the sleep laboratory using the multiple sleep late

The Psychological Maltreatment Rating Scales.

Psychological maltreatment is gaining recognition as one of the core concepts in child welfare, however, its utility has been limited by definitional problems and the absence of operationalized and validated instruments. These Psychological Maltreatment Rating Scales (PMRS) were developed for assessing psychological maltreatment in mother-child interaction, and were used to rate the videotaped interaction of 49 high-risk mother-child dyads and make predictions of child protective service involvement with the dyads. These predictions are compared with predictions based upon mothers' personal re
