Near-drowning.

Near-drowning and drowning are major causes of neurologic injury and death in young persons. Most victims aspirate water, and pulmonary edema develops in many of these cases. Prolonged submersion causes cerebral asphyxia and adversely affects the brain within five minutes. Immediate ventilation and oxygenation are essential in the reversal of cerebral anoxia. During field resuscitation, precautions should be taken to protect the cervical spine because of the possibility of injury. The Heimlich maneuver is used only after unsuccessful attempts at ventilation suggest foreign-body obstruction. Pr…

Evaluation of sudden death in psychiatric patients with special reference to phenothiazine therapy: forensic pathology.

The investigation of sudden unexpected death in psychiatric patients and the ensuing litigation has brought to our attention many unusual features important in the evaluation of such deaths. Certain pathophysiologic mechanisms of death, rarely encountered in routine forensic science practice, may be important in determining the cause of death in psychiatric patients, especially in cases where the autopsy is unrevealing. Of particular concern is a tendency in the current literature to implicate phenothiazines as a cause of death when the death investigation and the autopsies are incomplete. Thu

[Documentation of clinical death in preliminary death certificates].

The emergency physician acting within the framework of organised first-aid service, is not obliged to perform a post-mortem examination or to hold a coroner's inquest and to issue a death certificate. It is easy to understand why the relevant authorities must insist on the immediate submission of a death certificate. Such a certificate can be issued only if unequivocal signs of death definitely confirm that the decrease is an indisputable fact. To cope with this situation, a printed form has been designed which is used to document the occurrence of clinical death.

External counterpressure and the MAST suit: current and future roles.

External counterpressure devices, such as the MAST suit, may be lifesaving not only in acute hypovolemia secondary to abdominal, pelvic and lower extremity trauma, but in a number of other shock states as well. Expanded use of the MAST suit may well include not only supra-diaphragmatic injuries, but diverrse entities such as pericardial tamponade, tension pneumothorax, and leaking aortic aneurysm, and as an adjunct to cardiopulmonary resuscitation. The limited adverse effects of the MAST suit, in conjunction with its rapidly favorable and rapidly reversible hemodynamic effect, make it an impor…

Early neurophysiological assessment after insult to the central nervous system.

Cerebral insult is a generic term which includes many causes of altered cerebral function. Neurophysiological investigations, closely integrated with clinical and metabolic studies, should begin at an early stage after any such insult (namely in the first few hours) to provide information on the type, severity and evolution of latered brain function. The present contribution is based on prospective studies on 200 children with closed head injuries and 500 children who had been resuscitated following cardiocirculatory or respiratory arrest. In addition to electroencephalography, other neurophys

Acute Hyponatremia as a Telling Complication of Necrotizing Pancreatitis and Perforated Cholecystitis After Percutaneous Cholecystostomy.

Acute hyponatremia is an uncommon complication of percutaneous cholecystostomy drainage with minimal literature to date. We present a case of a patient with necrotizing pancreatitis causing perforated cholecystitis, requiring percutaneous cholecystostomy that was complicated by acute hyponatremia. High biliary solute and volume loss led to the hyponatremia and elucidated downstream biliary obstruction. Fluid resuscitation, biliary stenting, and endoscopic drainage corrected the patient's sodium and symptoms in this multidisciplinary case.

Navigating coagulopathy in obstetric hemorrhage: The role of point-of-care testing.

Postpartum hemorrhage (PPH) remains the leading cause of maternal mortality and morbidity worldwide. Recent advances in understanding the hemostatic changes of pregnancy and PPH have led to the development of obstetric-specific approaches to resuscitation. This article aims to examine. 1) changes in the coagulation system during pregnancy. 2) types of coagulopathy associated with PPH, and. 3) the role of point of care tests of coagulation in PPH.

Esophageal Dieulafoy Lesion: A Rare and Potentially Fatal Entity.

A 35-year-old gentleman presented acutely to the emergency department with large volume haematemesis and melena. Following adequate initial resuscitation, the patient underwent emergency upper gastrointestinal endoscopy which revealed a dilated tortuous submucosal vessel which was actively bleeding at the midpoint of the esophagus. This was consistent with a Dieulafoy lesion. However, its position in the midpoint of the esophagus is rare. Our patient recovered well following intervention.

[Nasopharyngeal hairy polyp in a newborn with intermittent airway obstruction. Case report].

Nasopharyngeal hairy polyps are rare benign tumors. We present a newborn case with a hairy polyp mass causing cyanosis and respiratory distress due to obstruction of the upper airway during the first 24 hours of life. The patient required resuscitation and endotracheal intubation. Diagnostic studies confirmed the presence of a mass in the lateral pharyngeal wall. Surgical treatment and removal of the mass was performed with satisfactory evolution of the patient.

Unexpected Complete Heart Block and Anesthetic Implications.

A healthy, active duty military 25-year-old female with a history of congenital complete heart block presented for a routine septorhinoplasty. During the preoperative interview, she did not disclose her heart condition. A preordered electrocardiogram was not available. During induction of anesthesia, she became extremely bradycardic, approaching asystole, requiring resuscitation. This case highlights the potential anesthetic risks in individuals with a history of congenital heart rhythm disease.
