Comparison of Postoperative Outcomes of Open, Laparoscopic, and Robotic Surgery for Pediatric Choledochal Cyst Excision.

BACKGROUND: Choledochal cyst is treated with extrahepatic bile duct resection and hepaticojejunostomy. Minimally invasive approaches, including laparoscopic surgery (LS) and robotic surgery (RS), offer potential advantages over open surgery (OS), but their impact on perioperative and short-term postoperative outcomes remains unclear. METHODS: This retrospective study analyzed 201 pediatric patients who underwent OS (n = 83), LS (n = 89), or RS (n = 29). OS was performed from 2004 to 2013, LS from 2013, and RS from 2021. Perioperative outcomes and early postoperative complications within one y

Tubeless ureteroenteric anastomosis in robot-assisted radical cystectomy with intracorporeal urinary diversion does not increase the risk of anastomotic stenosis or postoperative complications.

The objective is to demonstrate that omitting ureteral stent placement in robotic intracorporeal urinary diversion does not lead to increased risk of perioperative complications, namely ureteral strictures or anastomotic leaks. We retrospectively reviewed the records of 68 consecutive patients who underwent robotic radical cystectomy with ileal conduit creation or orthotopic neobladder by a single surgeon between January 2020 and September 2023. Chronologically, the first cohort of patients had ureteral stents placed to bridge the ureteroenteric anastomosis, and in the second cohort, stenting

Ureteric stricture rates and management after robot-assisted radical cystectomy: a single-centre observational study.

OBJECTIVE: Benign ureterointestinal anastomosis (BUIA) stricture is a recognized complication after open radical cystectomy. The evidence for stricture rates following robot-assisted radical cystectomy (RARC) is limited. This article reports stricture rates from a single high-volume RARC centre. MATERIALS AND METHODS: Between December 2003 and December 2015, 371 patients underwent RARC with a totally intracorporeal urinary diversion. All patients received a ureteric anastomosis utilizing the 'Wallace plate' with a running suture technique. Monofilament suture was used in the first 81 patients

Transmural migration of azygous vein Hem-O-lok clip causing food bolus 3 months following uneventful minimally invasive oesophagectomy.

BACKGROUND: Hem-o-lok clips are widely deployed in various laparoscopic and robotic operations. Their migration is not very common and majority of reported cases are limited to biliary, prostatic and vesico-urethral cases. METHODS: Herein, we would like to report the first case of transmural migration of Hem-o-lok clip from azygous vein stump following totally minimally invasive two-stage oesophagectomy for squamous cell carcinoma of the distal oesophagus, into the gastric conduit 3-months following uneventful discharge. The patient presented with 5-days history of worsening dysphagia. RESUL

Intravesical Hemostatic Clip Migration After Robotic Prostatectomy: Case Series and Review of the Literature.

INTRODUCTION: The Weck Hem-o-lok™ Ligating clip is a routinely used hemostatic tool in robotic and laparoscopic surgery. It has been the practice in our institution to use such clips for hemostasis of the vascular bundles during robotic prostatectomy. Migration of such clips has been reported in the literature as single case reports. In this study, we present a case series of intravesical Weck clip extrusions presenting as bladder calculi. Such events have led to a change in our practice, and more research is needed to assess the impact of this change. MATERIALS AND METHODS: A retrospective c

Short- and long-term outcomes of minimally invasive total mesorectal excision in obese versus nonobese patients with rectal cancer: a propensity score matched study.

BACKGROUND: Obesity adds technical complexity to colorectal surgery and has been linked to higher rates of perioperative complications and poorer long-term outcomes. Its prevalence is increasing among patients undergoing minimally invasive total mesorectal excision for rectal cancer; however, its impact on perioperative and oncologic outcomes remains controversial. Therefore, the aim of this study was to compare short- and long-term outcomes between obese (body mass index (BMI) ≥ 27 kg/m2) and nonobese patients undergoing minimally invasive total mesorectal excision. METHODS: Retrospective re

Lateral lymph node dissection via robotic surgery: technical feasibility and patterns of lateral lymph node recurrence in rectal cancer.

BACKGROUND: Total mesorectal excision (TME) with bilateral lateral lymph node dissection (BLLND) is a standard surgical approach for low advanced rectal cancer (LARC) in Eastern countries. Although robotic surgery has been increasingly adopted for rectal cancer, its impact on lateral lymph node recurrence (LLNR) after BLLND remains unclear. METHODS: We retrospectively reviewed 180 patients with pathological stage II/III LARC who underwent TME with BLLND between 2009 and 2019. Of these, 149 patients underwent open surgery and 31 underwent robotic surgery. Perioperative outcomes, long-term onco

Comparison of Therapeutic Benefit of Bupivacaine HCl Transversus Abdominis Plane (TAP) Block as Part of an Enhanced Recovery Pathway versus Traditional Oral and Intravenous Pain Control after Minimally Invasive Colorectal Surgery: A Prospective, Randomized, Double-Blind Trial.

Enhanced recovery pathways (ERPs), when combined with transversus abdominis plane (TAP) blocks, have been proven to reduce the length of stay (LOS) and improve quality outcomes. Nonopioid pain management is an essential component of this pathway, leading to a reduction in immobility, postoperative ileus, and an increase in patient satisfaction. TAP block variations have been studied in general and gynecologic surgery. This study evaluates the effectiveness of laparoscopic TAP blocks in conjunction with the benefit of an ERP. One hundred thirty-seven consecutive laparoscopic and robotic-assiste

Transoral robotic surgery for the treatment of laryngeal chondrosarcoma: A case report.

IMPORTANCE: Transoral robotic surgery has revolutionized the practice of head and neck surgery over the past decade, with indications now expanding to include laryngeal pathology. Although laryngeal chondrosarcoma is a rare entity, trends in otolaryngology literature suggest that it can frequently be managed with conservative approaches. We hope to inspire other head and neck surgeons to consider transoral robotic surgery for the treatment of such tumors so that outcomes data can be collected and studied. OBSERVATION: An 87-year-old male presented with a left-sided supraglottic mass that was

Combined endoscopic transcervical-transoral robotic approach for resection of parapharyngeal space tumors.

BACKGROUND: Parapharyngeal space tumors (PPSTs) pose exposure challenges; they are routinely resected by the transcervical approach using blunt/blind finger dissection, increasing the risk of tumor spillage and of neurovascular injury. Large PPSTs or those located high at the skull base often require mandibulotomy or an infratemporal fossa approach, baring considerable morbidity. METHODS: The novel minimally invasive approach described, utilizes endoscopic equipment introduced transcervically for circumferential separation of the tumor from the neurovascular structures of the skull base.
